An exciting opportunity has arisen to join the complex discharge teams in the Heart, Lung Critical Care Clinical Group. We are seeking a highly motivated and enthusiastic Administrator to join the Complex discharge teams across Royal Brompton and Harefield Hospitals. The successful applicant must have excellent communication skills and an ability to work independently as well as part of teams that work on both sites (Royal Brompton and Harefield). The post is based at Harefield but will require the post holder to cover administration tasks at the Royal Brompton with occasional travel there. This is a full time position (37.5 hours). The complex discharge team comprises of social workers and clinical nurse specialist who are responsible to managing transfers of care from hospital and ensuring the provision of ongoing care. This involves liaison with multi agency and providers.

Reach Academy Feltham opened in September 2012 as a Free School and grew year-by-year. Our original Reception cohort completed their GCSEs last year and are now in our sixth form.

The SLT team at Hillingdon Hospitals are looking for an enthusiastic band 5 Speech and Language Therapist to join our friendly and supportive adult in-patient service.

If you want to develop your knowledge, skills and experience in the acute, stroke and neurorehabilitation settings, this post will offer a variety of caseload experience and professional development opportunities, including completion of the RCSLT Newly Qualified Practitioner framework and in-house dysphagia competency training.

This role is split between our acute, stroke and neuro-rehabilitation caseloadsand requires regular, joint multidisciplinary team working. There will be opportunities to gain experience across acute and rehabilitation settings including opportunities to provide regular teaching and training to nurses, medics and other AHPs.



The post holder will be required to provide specialist assessment and treatment along the various clinical pathways including neurorehabilitation, stroke, care of the elderly, general medicine, respiratory and trauma. The post holder will work as part of the SLT team and wider MDT and will be supported and supervised by a Specialist SLT as well as the Clinical Lead SLT.

You will receive regular clinical supervision, monthly CPD sessions, peer support and evidence-based practice sessions provided by our very supportive team.

The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. The Trust’s services at Mount Vernon Hospital include routine day surgery, delivered at a modern treatment centre, a minor injuries unit and outpatient clinics.

Job Purpose

To provide a high quality education service to pupils and in accordance with Company policies, procedures and practices and the standards set by the Regulatory Body by complementing the teachers’ delivery of the School’s Curriculum framework and work cooperatively with teachers, pupils and other support staff.

Responsibilities

Undertake activities with individuals, groups or a class of pupils in order to facilitate their physical, emotional and educational development within a safe environment usually under the supervision of a teacher

At Hillingdon Manor, we provide specialist education for 185 pupils, aged between 3.5 and 19. Located across two sites. Our Secondary School supports around 125 pupils. Our pupils are taught in smaller class sizes and all pupils work towards a variety of qualifications dependent on their needs and ability. This role will have responsibility for teaching pupils who are aged 11 - 14, but work at a variety of different levels, so we welcome applications from teachers with Primary level SEN experience.

Working days are mainly Monday Friday but may include some weekend work. A degree of flexibility regarding days worked is essential. This is a permanent position. Our base is at GRH but clinics are held daily around Gloucestershire and Swindon. The post holder will work as an assistant practitioner, carrying out abdominal ultrasound scans within standard operating procedures. Following successful completion of the Health Screeners Diploma (AAA), the post holder will work independently and make judgements according to experience and training. The post holder will reassure men attending the community AAA screening and surveillance clinics and explain the screening test to them. The post holder will have a key role in conducting quality assurance assessments of machines as per National Guidelines to include safe care, maintenance and handling of all equipment as used by self and others. Communicate effectively with patients, carers, relatives and all members of the multidisciplinary team. Maintain and develop high standards of care. Take on responsibility for personal development and maintaining specialist knowledge of AAA Screening Programme. Contribute towards the continuing development of professional screening practice in the clinical area. Conduct oneself in a professional manner and be a role model to others.Work proactively to ensure that every patient attendance meets the patients expectations and needs.

Possess and demonstrate clinical skills including being able to deliver specialised urogynaecology assessment and treatments. Responsible and accountable for the management of a patient caseload within the specialty. Work autonomously to carry out specialist care. Work within an agreed job plan to meet service needs (reviewed annually). A minimum of 75% of the time will be direct patient assessment, care and treatment. The remaining 25% of the time will be dedicated to the required service management activities (managing the patient help line, promoting education, leading and monitoring the patient safety agenda, participation in audit to improve care delivery and patient outcomes, managing supplies). Establish and maintain effective personal relationships with patients and relatives and ensure that the individuals physical and psychological needs are identified. Be expected to establish and maintain an effective network of communication with members of the multidisciplinary team. Be responsible for receiving and disseminating information, to all grades of staff, relating to patient care management as appropriate. Communicate with patients at the appropriate level of understanding about issues regarding their medical condition whilst maintaining patient confidentiality. Ensure that they understand the rationale for their care and its consequences and alternatives. As far as possible ensure that the patient understands the information given to them, overcoming any barriers to understanding, e.g., deafness, learning difficulties, stroke patients. Record details of any information given, always maintaining patient confidentiality. Use specialist knowledge to take Informed Consent from the patient by informing the patient and explaining the reason for the procedure, benefits, risks, complications and effects of the intended procedure, and any alternative treatments. Perform a comprehensive problem focused health history and physical examination, and be able to analyse and interpret history, presenting symptoms, physical findings and diagnostic information to develop differential diagnosis within the specialist area. Ensure patients are investigated appropriately, interpret results and communicate results to patient, carer and GP/referrer.

You will have assessment, formulation, therapy, and leadership and supervision skills; and you will have (and/or want to develop) experience, training and knowledge in working with Addictions and associated complex presentations. Although experience of working with people with addictions would be valuable, we recognise that not all applicants will have had this opportunity. Tailored induction and training will be provided to enable you to work effectively with this client group. You will have good research and audit skills, be familiar with care quality and clinical governance and be expected to contribute to service development in this way. You will be operationally responsible to the team manager and receive regular clinical supervision, support and reflective practice from your Professional Lead for Addictions, as well as peer support and guidance from other senior team members.
