What will you be doing? Part of the role involves working alongside Childrens services with young people under 18 years to prepare them and their families for adulthood. You will support young adults through change, encourage progression, and acknowledge individual, family, and community strengths through a transforming lives approach. To lead and inspire the team to deliver a high quality and effective recovery service for service users and carers. To provide clinical leadership to ensure where appropriate the service will work effectively alongside other services, to manage risk and prevent hospital admission for service users with the most complex and acute needs. To undertake and support the specialist ongoing assessment of physiological and/or psychological functioning for service users with the most complex and health and wellbeing needs. CLINICAL To identify, prioritise and manage patients who require occupational therapy input. Including education of the multidisciplinary team (MDT) to promote appropriate referrals for both discharge planning and rehabilitation. The Senior Commissioner for Children and Young Peoples Integrated and Personalised Care within Bedfordshire, Luton and Milton Keynes Integrated Care Board (BLMK ICB) has been developed to lead on the Learning Disability and Autism Programme to better meet the needs of the most complex children and young people; creating sustainable integrated systems and processes that can transition into the new Care Alliances and Integrated Care Partnerships. Key Duties and Responsibilities The postholder will take the lead for the system wide integration and personalisation agenda relating to children and young people with complex needs that spans health, care and education, where a more integrated multiagency approach is needed to prevent/reduce crisis and manage unavoidable crisis to improve outcomes Providing professional expertise in integrated (multiagency) case management Chairing and/or participating in multiagency panels and decision-making forums as an ICB senior team member The post holder will be expected to work with children and young peoples health, social care, and educational services (including SEND) to co-ordinate packages of care for individual young people, mobilise support within the system and removing any barriers. HMP Bullingdon is a male remand prison that has a capacity for 1,152 the offers 24 hours healthcare and there is an inpatient unit on-site. Hours This will include working days shifts, night shifts, weekend work, bank holidays and evening shifts. This will be working in all areas of the prison. Salary This position is full time, youll receive an annual salary up to £52,507 per annum FTE - depending on experience. How will we support you?
