Gnalla Community Pty Ltd is currently looking for full-time disability support worker/ Aged or Disabled Carer/Personal Care Assistant with minimum 1-2 years experience. Our business is located at Northern Perth WA. We are looking for compassionate, reliable, and experienced disability support workers for our clients .
You can make a difference in our clients' lives by supporting them with daily living activities and domestic duties and empowering them to achieve their life goals.
Working in a team, you will facilitate the delivery of quality disability services and achieve positive outcomes with service users in a positive and proactive environment.
You will be actively supporting their individual needs and creating a safe environment. The annual salary is between $73,000 - $80,000.Key
Responsibilities:
Support participants in working towards achieving their individual goals. Assist with daily living activities, including cooking, housekeeping, hygiene, shopping, medication administration, and community access.
Help participants build their confidence. Liaise and collaborate with clients, their community, and community & disability support agencies.
Monitor and review participants' progress through the service and against their support plans. Utilize positive behavior strategies.
Support clients in participating in age-appropriate activities and tasks, including employment, in-home disability support, and community access.
Adhere to the medication administration policy, work health and safety procedures, and client rights policies, including privacy and confidentiality.
Monitor the safety and well-being of people with disabilities and report any concerns to the Manager.
Ensure completion of appropriate documentation; relevant information on disability support should be recorded in client notes and communication books as required. Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
Work with clients, family/carers, relevant case managers, and other professionals to support the client's individual goals.
Liaise extensively with other staff to ensure the overall smooth and consistent running of disability services.If this sounds like you,
