The incumbent serves as the Assistant Chief of Health Information Management (ACHIM) in a medical facility of high complexity (level 1), with responsibility for the supervision of HIM functions. Incumbent is charged with full responsibility, both technical and administrative, for the daily operation of the HIM department which consists of all functions of medical record activities related to coding, analyzing, transcription, release of information, file room and scanning programs. Is a full assistant to the Chief, HIM. The incumbent shares responsibility with the CHIMS for collecting and processing data for various reports, surveys, and research projects; for preparing reports and recommendations for review and consideration by the Chief, HAS and upper management concerning the immediate needs as well as the long-range goals of the department; for recommending equipment and staffing needs; and for preparing and keeping current procedures and operation manuals.

Responsible for assisting physicians and other health professionals in obtaining health record information to be used in research projects. Advises on what material is available, how it may be obtained and the uses that can be made of the information while protecting the confidentiality of the record. The incumbent aids in a variety of projects and is responsible for screening, abstracting and tallying data from records and computer-generated reports. May serve as a consultant to provide guidance and advise researchers on methods for gathering data. Assists in selected miscellaneous retrospective health record review/data abstraction projects to support medical center Quality Management data collection activities.

Provides technical guidance to the unit supervisors and lead clerks while providing support to each unit in the absence of the section supervisor. Provides expertise in the area of subpoenas, court orders, advance directives, and general release of information, as well as other medico-legal issues.

The Nurse IV Associate Nurse Executive/Chief Nurse of Connected Care and Transitional and Community Inpatient has direct programmatic/supervisory responsibility for the Nursing Connect Care and Transitional and Community Inpatient Managers and Administrative Support functions. Ensures the application and utilization of policies, procedures, and standards of personnel are consistent. Health Records Management

• Responsible for all facets of health records management for the facility
• Directs the operation of all personnel working within the following program areas: records analysis, release of information, health record files, incomplete records/physician liaison, data validation, health records QI processes, medical transcription, scanning, outpatient/inpatient Coding and other health information projects.
• Evaluates health records program and independently determines the nature of extent of problem areas.

• Administers the health record program and assures that the facility fully accounts for workload in a timely, accurate manner
• Provides detailed technical expertise in health records science and other very technical aspects of health information

• Serves on a multidisciplinary revenue team to ensure optimal processes are in place in order to facilitate maximum reimbursement opportunities.

The Service Chief is responsible for designing the mission and vision of the Service, developing the scope of practice and/or privileging packets for all social workers, designing a Social Work service Plan of Care and assuring implementation of quality improvement measures. As the VISN 22 VA Health Connect Clinical Contact Center (CCC) Chief Nurse you will report directly to the VA Health Connect CCC Director. The Clinical Contact Center Chief Nurse will provide executive leadership and direct supervision to the Nurse Managers over the Clinical Contact Center nursing staff and other employees as assigned. Assesses, plans, and evaluates the processes and daily operations of the Clinical Contact Center, collaborating with the Nurse Managers and administrative and clinical leaders within the VISN (e.g. from Clinical Contact Centers and Tele Emergency Care), at the medical centers where patients are assigned (e.g. from Primary Care, Emergency Medicine, and Specialty Care), and with Program Offices (e.g. Homeless Program, Veterans Crisis Line) to design and implement interdisciplinary programs and processes within the service.

Chief, Clinical Contact Center is responsible for:

• Assessing and recommending the allocation of human and material resources, in determining staffing levels, managing overtime, and plays a significant role in the recruitment and retention of staff.
• Ensure competency assessment meets the learning needs of nursing staff and are completed on each employee annually.
• Reviews Clinical Contact Center Data, quality assurance findings, and business operations data related to nursing and clinical operations for trends and early intervention; formulates follow-up plans with stakeholders within and outside of the VISN.
• Evaluates findings from VISN Clinical Contact Center Quality Assurance Program and collaborates with both administrative and clinical services to monitor quality and safety within the service and during transitions in care to the medical centers.
• Functions as facilitator in implementing action plans and performance and process improvement strategies at services for follow up to include regular progress reports.
• Performance improvement projects may be co-led with leaders from medical centers.
• Promotes evidence-based practice designed to validate and/or improve patient care and health care delivery.
• Acts as a liaison with the nursing leaders in Offices of Nursing Services, Office of Integrated Veterans Care (IVC) and the Office of Nursing Services to address issues related to nursing as needed.

• Chief Nurse experience preferred
• Three plus years of experience in Emergency, Critical Care or Call Center leadership preferred
• Doctorate degree with Evidence Based Practice experience is a plus

The Mental Health Service Line provides care in several psychiatric settings: inpatient units, ambulatory programs, residential program for substance abuse, residential program for PTSD, emergency department, medical-surgical ward psychiatric consultation and geriatrics with extended programs. Psychiatrists within the Mental Health Service are able to perform and teach all of the services required in the practice of psychiatry to psychiatry residents. Educational experiences are also provided to appropriate health care personnel who seek training in mental health services (e.g. medical students, psychology and social work interns, physician assistant and nursing students, and non-psychiatric medical residents).

Section Chief duties include but are not limited to the following:

• Provide clinical and administrative coverage for Charlotte Outpatient Mental Health
• Be part of an on-call rotation of psychiatrists, which may include rounds on acute patients during weekends and holiday
• Supervise psychiatry staff
• Provide the general oversight of advanced clinical providers working in Mental Health.
• Provide clinical coverage of Inpatient Units and Outpatient Units, as necessary
• Advice and counsel workers on work and administrative matters
• Participate and coordinate Medical Center clinical committees, as required.
• Managing Psychiatry discipline - specific issues/concerns as appropriate to the authority delegated.
• Work with the other Mental Health supervisors to ensure that appropriate disposition services and meaningful therapeutic treatments are being offered throughout the department.
• Act in the absence of Mental Health Service Chief and serves on the management team of the Mental Health Service Line.
• Review and approval/disapproval of leave requests
• Assure that subordinates are trained and fully compliant with the provisions of safety regulations.

• Application of accepted protocols for assessment of the individual presenting for psychiatric care. Assessment may include gathering verbal and documented history, mental status examination and medical and laboratory evaluation.
• Diagnostic description or formulation to guide appropriate acute and secondary treatment interventions.
• Provision of psychiatric care according to his/her privileges.
• Be available for all emergency psychiatric interventions during on-call hours
• Provide or assist residents and/or Advanced Clinical Providers in the process of evaluation and treatment of the psychiatric patient, and actively participate in corrective actions against untoward responses to psychiatric interventions
• Evaluate the patient to assess changes in the physical and mental status. A progress note is written in the patient's chart. Notes also include the concurrence with notes written by the resident and attending notes as appropriate.
• Perform inpatient and outpatient psychiatry evaluations and treatment as the team leader
• Participate in various hospital committees, departmental and hospital in-service programs, and education programs of the department and the hospital. They are also involved in continuous quality improvement through the peer review process
• Initiate or participate in an ongoing research activity. Faculty appointments to affiliated medical schools are available, as teaching medical students, residents and other disciplines is essential

The Lexington VA Health Care System (LVAHCS) is a fully accredited, two-division, tertiary care health care system with an operating bed complement of 172 hospital beds. LVAHCS received a 5-star CMS rating & is 1 of 7 VA Patient Safety Programs of Excellence. The Troy Bowling Campus, located adjacent to the University of Kentucky Chandler Medical Center, offers specialty outpatient care, inpatient acute medical, neurological, surgical, & psychiatric care. The Franklin R. Sousley Campus has a 5-star Community Living Center & cottages with hospice and respite services, primary care, home-based primary care, prosthetics, women's health, optometry, podiatry, massage therapy and chiropractic, mental health, substance abuse treatment, acute rehabilitation, and Post-Traumatic Stress Disorder residential rehabilitation treatment. LVAHCS also operates four VA community clinics in Somerset, Morehead, Hazard, & Berea, KY. Lexington VAHCS is part of the VA MidSouth Healthcare Network, which consists of five medical centers in Kentucky and Tennessee.

The incumbent shares with the Executive Leadership Team a level of responsibility for the direction and management of the healthcare system and is directly responsible for all facilities operations. The incumbent has direct line authority and leadership responsibility for all administrative operation support services. The incumbents managerial duties and responsibilities involve final decisions which have a direct and substantial effect on the health care organization and programs related to facility operations. These include, but are not limited to decisions affecting the nature, scope, quality of, and emphases on, the facility's patient care programs and activities.

Duties include, but may not be limited to the following:

• Sharing with the Executive Leadership Team (ELT) a level of responsibility for the direction and management of the healthcare system and is directly responsible for all facilities operations.
• A direct line authority and leadership responsibility for all administrative operation support services.
• May serve as the Acting Executive Director in the absence of the Director.
• Taking an active leadership role in coordinating and developing customer and stakeholder support and resourcing to meet objectives of the healthcare system.
• Determining operational services and programs goals and objectives, develop short-and long-range plans for achieving them in conjunction with the overall plans of the Veterans Integrated Service Network (VISN) and consistent with the strategic planning process.
• Directing a comprehensive evaluation of operational services goals and objectives and makes necessary adjustments to improve delivery of patient care services.
• Establishing local policy in such areas impacting facility operations as program emphasis and operating guidelines.
• Administering and ensuring compliance with a system designed for the development and communication of program policies and procedures in accordance with new or existing rules and regulations.
• Administering a management reporting system which provides appropriate data for decision making for facility operations, impacting organizational improvements.
• Performs other related duties as assigned.

The physician is responsible for providing primary, secondary, and tertiary care in both hospitalized and outpatient settings, identifying patients' health problems and designing diagnostic and therapeutic plans to meet these needs.

The Section Chief is responsible for maintaining all operations and direct reports within Hematology Oncology. The incumbent will provide direct supervision to the Hem/Onc and all clinical providers within the section. The incumbent will be the point of contact for physicians and clinical staff assigned to the department. This individual will work with staff and physicians to optimize the usage of clinical systems through education and training, quality and outcomes, and providing support in accordance with organizational goals. The incumbent will work closely with hospital staff, department directors, and physicians to ensure that the highest standards of quality and services are maintained. The incumbent also serves as a liaison between customers, Information Technology staff and other departments and services. This individual will also work directly with all levels of the organization on projects, committees, and teams to meet the overall needs of the Pulmonary and the facility.

• The Associate Nurse Executive/Chief Nurse of Behavior Health Services reports to the ADPCS/NE. The ADPCS/NE provides the guidelines for the yearly Executive Career Field (ECF) performance plan.
• As a recognized authority in the analysis and evaluation of programs and issues, the ANE/Nursing Behavior Health Services is subject to administrative and policy direction concerning overall project priorities and objectives.

This MD- Chief, Geriatrics & Extended Care (GEC) position is located in the Office of the Geriatrics and Extended Care at the Providence, RI location. This position is full-time at 40 hours per week.

Included but are not limited to:

• Provides leadership in Geriatrics and Extended Care (GEC) Service Line (SL) and for the organization and delivery of these services for the Providence VA Medical Center; and is accountable for ensuring quality and cost-effective care. Geriatrics and Extended Care (GEC) Service Line (SL) is aligned under the Chief of Staff.
• Oversees all Geriatric and Extended Care operations at PVAMC.
• Reports directly to the Chief of Staff, and will work closely and effectively with the GEC clinical leadership and collaborate with other clinical and administrative leadership to accomplish the goals and mission set forth from Executive Leadership.
• Manages personal clinic time allocation negotiated with the Chief of Staff.
• Works closely with QM and is expected to support and be actively involved in systems redesign initiatives, to meet or exceed Joint Commission requirements, and to actively work on the establishment on policies and procedures.
• This is an academic position and expectation to be involved actively in research and education, in addition to the clinical activities.

  • Reviews/performs/provides duties and responsibilities of a geriatrician/internal medicine physician. - Demonstrates leadership in interpreting and supporting the VISN mission, vision and values, while operating in accordance with Veterans Health Administration (VHA) policies, facility and VISN policies, and standards of federal professional and accrediting agencies external to VHA.
  • Adheres to the Medical Staff Bylaws and Rules and Regulations as set forth by the medical staff.
  • Provides direction and support to GEC staff regarding equipment technology, policies and procedures employed by the Providence VAMC, ensures that all requirements of regulatory agencies such as the Joint Commission are met.
  • Reviews quality control and quality improvement procedures performed by VA personnel in GEC SL and makes recommendations for improvements, corrective actions, etc.
  • Participates/reviews and documents performance improvement activities, including, but not limited to, external review programs, internal peer review programs. and accreditation requirements.
  • Active member of various Medical Center committees.
  • Responsible for the financial management of the GEC SL.
  • Collaborates with the Chief of Staff to develop and execute a strategic plan and annual budget for the service line.
  • Completes reconciliations for employees who leave their service and still have purchases pending. Complete reconciliations for employees who are on leave for more than seven (7) calendar days.
