KEY PERFORMANCE AREAS Develop a detailed understanding of East Lancashire Hospice Community Model, knowing the different options and levels of provision available to provide supportive palliative care in our community including that delivered by other organisations in order to maximise available support for patients and their families Utilise outstanding organisational skills to develop, implement and maintain robust systems and processes necessary for the effective coordination of supportive palliative care. Purpose:

The role of the Advanced Practitioner is as a non-medical practitioner, working as part of the Clinical Haematology team, under the supervision of a Consultant Haematologist.

The post holder will provide clinical expertise and advice for haematology oncology chemotherapy patients. They will demonstrate a sound understanding of the issues related to the identification, assessment, diagnosis, treatment and management of non-malignant and malignant haematological conditions and pre and post chemotherapy patients. Be involved in consultant clinics and assist in pre chemotherapy assessments and planning, and emergency attendance avoidance. They will have a key role within the MDT acting as a liaison between all members of the multi-disciplinary team, to allow for seamless and efficient care.

They will monitor and improve standards of care through supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership. They will participate in clinical governance within the service by undertaking audit and research as part of the MDT.

Under the direction of a Consultant Haematologist and following a programme of training and competency assessment the Advanced Practitioner will;
• Provide advanced levels of clinical practice, knowledge and skills in Haematology
• Undertake Pre-chemotherapy assessment and physical examination as directed by the Haematology team
• Assess the need for and order investigations within scope of practice
• They will have a clear understanding of their boundaries and know when to involve the Consultant Haematologist
• Manage medications including assessment, review and non-medical prescribing where appropriate and in accordance with legislation.
• Manage own caseload with decision-making responsibilities e.g. fitness for treatment
• Undertake the process of gaining informed consent
• Prepare accurate and concise reports both verbally and
• Some technical and invasive procedures according to their scope of practice g. bone marrow biopsies
• Facilitating the continuity of care of patients
• Arranging appropriate pre and post chemotherapy investigations as part of the multidisciplinary team
• Liaising with accident and emergency for haematology emergencies and arrange admissions if required to maintain coherent service provision
• Providing ACP-led medication and review clinics

There may be times where cover will also be needed at Burnley Magistrates Court. The role will include preparing papers and files for court, tribunals, hearings and meetings; creating and updating records, including collecting information; data input, communicating with the Judiciary and other court staff and handling face to face, telephone and email enquiries from court users. The role will also consist of CVP preparation and hosting. Training will be provided on internal systems. Excellent communication and IT skills are required for this post.

• Strong administrative skills with proficiency in Microsoft Office packages
