Job Summary 1. To lead, deliver, develop and evaluate clinical pharmacy services relating to anticoagulation across the Trust, including Promote safe and effective use of anticoagulants during patient admission. To support the anticoagulation services through direct clinical care commitments for prescribing and counselling at the designated anticoagulation clinics and provision of expertise at the relevant specialist MDTs. To support the implementation of the National Commissioning Policy for immunoglobulins within the Trust, including participation in the local and sub regional panels. 2. Represent the Pharmacy department at the Thrombosis Committee and the Trust at the Immunoglobulin Assessment Panel and South London Cardiovascular Network. 3. Act as a role model and provide a consistently high standard of pharmaceutical service to patients. 4. Support divisions across the Trust in managing anticoagulant expenditure and act as a liaison for service developments involving anticoagulation services. 5. Provide operational and clinical support to the pharmacy department as needed, including supervision, training, and support to junior pharmacists and other pharmacy staff. 6. Deputise for the Principal Pharmacist for Surgery and Theatres at Surgery Divisional meetings if appropriate. Key Results Areas & Performance: Clinical Services and Patient Care 1.
