Job Purpose - Monday to Friday
The post holder will work autonomously and in collaboration with other health care professionals,statutory and voluntary agencies and the Planned Care locality teams to deliver excellent standards of patient care for patients requiring support in unplanned care.
The post holder will be required to:
· To deliver high quality, skilled clinical care to patients at home and provide rapid response, rehabilitative and post-acute carein a varietyofcommunitysettingsincludingweekendsandbankholidays.The service operates through pre-rostered shift patterns spanning 8am -10pm7daysaweek.
· Toutilisespecialist skillsandknowledgetoholisticallyassessneedsand provideshort term programmes of nursing care for vulnerable adults basedonevidencebasedpractice.
· To work, as required, on a flexible basis to cover all roles required within the team including in-reach support at Watford General Hospital.
· Toworkwithinandacrossprofessional andorganisational boundaries,liaisingwithGP’s other professionalsandstatutoryandvoluntaryagenciestodeliverindividualisedand expertclinicalcaretoahighstandardtoclients.
· Totakeresponsibilityformanagingthecaseloadandtheteam,withthe supportoftheSenior Nurses.
· TosupporttheLocality Manager/Clinical Leadand colleaguesindeliveringhighqualitycarethatdeliversbestvalue,andmeetstherequirementsofcommissioners.
· To provide skilled, effective and evidence based clinical care to patients in a variety of community settings.
· To use enhanced skills and knowledge to assess physical and psycho socialneedsanddevelopacomprehensiveandconcordantplanofcarewith patients, carersandthewiderMultidisciplinaryTeamasneeded.
· To make risk assessments, establishing the safety or otherwise of patients' ability to stay at home, or return home safely from another care setting,providingandarrangingnecessarynursingandsupportivepackages.
· Provide autonomous clinical care to a range of patients presenting with undifferentiated, undiagnosed and/ or complex health needs and long termconditions.
· To support appropriate and timely case-finding of patients suitable for the service including working to support in-reach to local acute trusts as needed
· To provide evidenced based clinical/therapeutic interventions based on'bestpractice',inordertoimprovehealthoutcomesandpromote choice,e.g.Intravenoustherapy, anticoagulation monitoring, palliative care interventionsandcatheterandbowelmanagement.

Both verbal and written communication skills, and experience of team working will be essential to the role. Strong interpersonal skills will also be essential. You must be an NMC registered, UK based nurse to apply for this role, as it is 'zero-hours' it does not attract visa sponsorship. You must also have at least 6 months acute experience.
