Senior Care Workers will work with patients in their own homes/Care Homes or at Hospitals. Senior Care Workers has to do a range of activities relevant to the individual with care needs i.e.
1- Reading blood sugar monitors
2- Giving insulin
3- Pressure Ulcer care
4- Simple wound care
5- Simple medication management
6- Basic initial assessments

1. To assess patients, identifying patients problems, potential complications and contraindications and referring back for assessment by a registered practitioner when necessary. To be able to assess, evaluate, progress and report back and assist the multi disciplinary team (MDT) in identifying and planning treatment options and ongoing therapy interventions. To identify and organise/undertake appropriate follow up treatment within a delegated framework of responsibilities, reporting back to registered staff and referring on to other agencies when necessary. This could include virtual or face to face follow up, home visits and post clinic phone calls. 2. To use clinical reasoning skills to agree functional goals and develop and evaluate the treatment plan. To implement and progress treatment programmes in order to promote physical / functional independence. To include the instructing of patients on functional exercise, the assessment and issuing of equipment for activities of daily living. Assessing and instructing patients in the safe use of mobility aids and equipment for activities of daily living, and practice activities of daily living such as mobility, safety on the stairs, dressing and bathing. 3. To provide a collaborative assessment and treatment approach to reduce repetition of information and provide a seamless pathway of rehabilitation from assessment through to discharge, with appropriate follow up intervention post discharge. To improve the cross partnership working process by communicating as a key member of the multidisciplinary 4. To exercise autonomy in the management of the caseload in the clinical area, to include making decisions about allocation of work to other staff, both registered and non-registered. To undertake specific unaccompanied work and organise own caseload comprising of patients with predictable and unpredictable conditions. 5. To use verbal and non-verbal communication tools to communicate effectively with patients/carers. To undertake administrative and housekeeping tasks to support the Therapy team and ensure the ongoing care and rehabilitation of the patient. To include making accurate and legible entries in the therapy and multidisciplinary patient records. This will involve discharge summaries, reports and transfers to other agencies. This will include demonstrating compliance with the terms of all safeguarding policies and processes relevant to the safeguarding of adults and children in the care of GHNHSFT and to undergo regular mandatory training concerned with safeguarding matters. Communications and Working Relationships To provide, receive and interpret routine information to and from the following to achieve effective patient care: - Patients - Families and carers - Therapy team - Multidisciplinary team - Internal and external organisations - Voluntary organisations - Public Most Challenging Part of the Job - To embrace the principles of both Occupational Therapy and Physiotherapy, bringing them together to work in an integrated and client-centred way - To provide high quality therapy in a fast paced, changing working environment within the resources available, to ensure the delivery of Trust targets - To motivate the patient to take part in rehabilitation and to promote independence, when there may be reluctance or communication difficulties Physical Effort and Working Conditions - Frequent daily manual handling of patients with or without equipment with a range of weights and disabilities. - Daily manual handling of equipment with a range of weights, sizes and shapes and over varying distances. - Frequently carrying out tasks involving bending, kneeling, and reaching on occasions in confined spaces as part of therapy intervention in the hospital setting and occasionally in the service users home. - Daily exposure to contagious/bodily fluids and, or home environments which pose infection control risks. - Frequent exposure to emotional distress when dealing with relatives and/or carers of service users coping with uncertainty, loss of status/role and physical change. - Occasional use of VDU - Occasional exposure to intoxicated, aggressive or abusive patients or - Frequent exposure to patients with cognitive deficits leading to; agitation, lack of insight, distress, denial, anger and challenging - The ability to drive and have access to a vehicle to travel to community sites outside of GRH & CGH will be required.
