Duties and Responsibilities: Audit and Guidelines Facilitate the development, implementation and monitoring of clinical guidelines across the Womens Directorate. To ensure the Directorate is aware of National and Regional guideline changes in practice that may influence local guideline development, to ensure practice remains up to date & evidence based To ensure a robust repository of guidelines within the Directorate is monitored and maintained, to provide assurance to the Division that the guidelines are up to date and in line with current practice. Each directorate has a nominated governance lead who is responsible for the coordination and monitoring of the local clinical audit program and relevant NICE guidance for the area. This role will work closely with the governance leads and/or clinical teams to actively support and monitor the progress of audit programmes, attend any relevant meetings, or forums and provide opportunities for clinical audit training as required. The postholder will be required to support Trust high priority clinical audit work such as mandatory returns, national audit, audit, and improvement work linked to safety (audits related to Patient Safety Investigation (PSI) action plans and safety incidents, Trust wide governance audits). The postholder will be expected to work autonomously on audit projects and providing support and guidance for the review and implementation of relevant NICE guidance. The postholder will lead, support and manage the Clinical Audit team including involvement in annual appraisal processes.

KEY DUTIES To support the Hearing Services management and clinical staff with clinical audit projects. To process/collate completed clinical audit data. To carry out exporting of data from Hearing Services systems into spreadsheets and reports. To perform data checks on data in excel and undertake analysis and suitable presentation of the data. To assist with the dissemination & collation of clinical audit data to and from clinicians and managers. To assist with maintaining the audit database, collate and enter data onto the database as part of the clinical audit activity. Work with clinical and management teams to assess and improve data completeness and data accuracy for quality and activity reports. Maintain the standards of Hearing Services department including confidentiality. To assist with taking calls to the team and dealing with them appropriately Identify errors and missing values in routinely collected clinical data and establish sustainable processes to reduce errors and improve data-quality. This might include identifying alternative electronic sources of information, other people with access to missing information, or in a small number of cases direct data-entry from paper records by the post holder. Develop working relationships with the informatics team to improve data-quality in data submission including checking data-files for common errors and omissions prior to submission, and answering queries after submission. Support the Hearing Services management team to refine and then document the common data-tasks currently being undertaken to allow these to be delivered by other people if required in the future. Support Hearing Service Management team develop and document new processes to minimise the use of identifiable data wherever possible for audit. Differentiate and develop separate process for requests for non-identifiable data for research to patient benefit.

The post holder will support the team to accurately monitor cleanliness in line with the NHS National Standards of Healthcare Cleanliness, via audit, analyse results, identify trends, and ensure remedial actions are taken to improve standards in a timely manner. Coordinate, administer and review the NHS National Standards of Healthcare Cleanliness audit . Develop, update, and review the audits schedule in line with the functional risk factor for each area. Plans time effectively to ensure audits are completed in line with schedule. Develop improvement plans and ensures completion where required. The role involves direct service user communication and interaction, working in challenging environments: including forensic, older people and adult mental health wards. The post holder may be subject to unpleasant working conditions whilst completing audits. Liaise with Service Managers, Ward Managers and Practice Managers and discuss audit outcomes and improvements where required. Lead on resolving operational performance issues that that have been highlighted by the audits. Training To participate in delivering assurance of continuous improvement initiatives by assisting in developing, implementing, and supervising staff training programme to all Hotel Services staff. Coordinate training requirements by organising administration frameworks and staff training programmes to ensure continuous improvement initiatives. Coordinate and complete staff training to ensure compliance with Trust and Hotel Services mandatory training targets across the Hotel Services team. This will include COSHH and SSOW compliance. Update training matrices ensuring departmental records reflect training needs accurately to provide assurance of mandatory training in line with Trust and department requirements. The post holder will lead in obtaining and submitting stroke patient data to SSNAP, this will involve liaison with clinicians and departments across the organization to obtain medical records and assess suitability for submission to SSNAP . Other responsibilities include supporting the Head of Clinical Effectiveness and Assurance by ensuring that a co-ordinated approach to managing the clinical audit programme and escalation of risks is done in a timely manner. The successful candidate will oversee participation in national clinical audits and co-ordinate high-level summaries of national clinical audit activity providing comparative analysis of Trust performance to relevant Trust committees. In addition to co-ordinating, facilitating and managing numerous clinical audit projects from a wide range of specialities; this will incorporate project design, data collection and analysis, report writing and presentation of completed projects. Co-ordinate Trust wide clinical audit activity ensuring clinical audit results are distributed widely to all relevant clinical teams and provide support in the development and implementation of subsequent action plans. Support the delivery of the trust wide quality strategy focussing on clinical effectiveness across the Trust. Co-ordinate robust and effective implementation of systems and processes developed for reviewing information and data relating to clinical effectiveness activities. Provide high level facilitation and co-ordination of action plans to support the effectiveness and assurance compliance for the Trust. Assist with advice and support to the clinical effectiveness function, Trust wide committees and specialty clinical teams to allow them to manage their clinical effectiveness activities effectively. E.g., QIPS report coordination, committee reports and group board reports. Provide support and advice for healthcare professionals in the development of clinical effectiveness project methodologies, including all aspects of project design, data collection methods and analytical support. Ensure the timely dissemination of completed clinical effectiveness reports ensuring that action plans are developed and that any actions taken are collated and recorded and, where required, re- audit activity planned. Develop clinical coding awareness presentations for non-coders/clinicians, presenting them accordingly at audit days and new consultant induction programmes. Assist the Senior Management Team with the on-going development and long term planning of the Clinical Coding service, ensuring that national clinical coding standards, rules and conventions are implemented within the agreed timescales. Lead and motivate the Site Clinical Coding Managers in developing and maintaining robust liaison and communication with a range of care group staff, clinical teams and the Information department, regarding a variety of complex matters and issues to assist with the maintenance of high quality, accurate clinically coded data Participating in the Getting it Right First Time (GIRFT) initiative, analyse the Hospital Episode Statistics (HES) data contained within the relevant report for conflicting information or indicators, identifying the potential root cause(s) for any negative trends or where the trust appears to be an outlier against national performance targets. Participating in the GIRFT initiative, communicate to the various staff disciplines how the trust recording practices regarding main and speciality treatment function codes impact upon HES data. Assist the Senior Management Team with the motivation and leadership of the trust wide clinical coding team in the production of high quality clinically coded data. Assist the Senior Management Team in the identification of areas for audit. Using specialist clinical coding knowledge, act as the professional link to the Care Groups through the provision of clinical coding reports as part of their performance management arrangements. Communicating the depth of coding versus benchmark for each specialty in the group and assisting them to agree data quality and clinical coding improvement plans.
