Investigate and take the appropriate action where pathways are incomplete to support timely patient care, as directed by the team supervisor or team leader. To assist in the creation of weekly reports ensuring any errors recorded on the patient pathway system are amended as directed by the team leader. To provide patient level detail where clinical assessment is required. Alert team leader/supervisor to any patients where appointment delay may be a cause for clinical concern. To participate fully in team meetings and suggest and implement improvements to ways of working within own area.
