The Clinical Specialist Podiatrist (Diabetes) will:
Clinical Practice
Act as a recognised expert podiatric resource for specialised clinical advice to patients, peers and other medical staff including Consultants and General Practitioners.
Ensure professional standards are maintained in accordance with CORU, The College of Podiatry (UK) Guidelines on Minimum Standards of Clinical Practice for Podiatry.
Responsibility for own clinical and administrative practices in line with SCFT policies procedures and guidelines as well as national models of care and national podiatric best practice.
Treat and manage the acute diabetic foot within the community for
those with active diabetes foot disease.
Undertake specialist podiatry treatments and interventions as appropriate to the grade, skills and competencies in the community setting.
Provide standardised high-quality diabetes foot management to patients with active foot disease /in remission foot.

Adhere to national and international guidelines on diabetes foot management with emphasis on that developed by the National Clinical Programme for Diabetes/Chronic disease programme.
Develop specialised, tailored management plans following an in-depth accurate assessment and diagnosis of the patient, using highly advanced and specialist skills in patient management.
Ensure timely referral to other services in the Multidisciplinary diabetic foot team or other as required.
Identify the need for change in own clinical practice, and that of colleagues, within the context of changing demographics, economic and legislative needs.
Provide clinical leadership that will influence and assist in the development of quality improvements in diabetes foot management.
Quality, Safety and Risk Management
Work in a safe manner with due care and attention to the safety of self and others
Implement national and internationally developed standards in care for the diabetic foot and avail of evidence based interventions to achieve these standards as agreed locally.
Develop and review appropriate clinical care pathways for patients with diabetes and facilitate these with other specialists within the team and wider organisation and region.
Ensure that every patient is to be treated as an individual and provided with a high quality service in terms of dignity courtesy, kindness, interest and efficiency.
Lead and collaborate on the development and implementation of local and national standards of practice, clinical protocols and clinical pathways for diabetes foot management.

As an Intermediate Care Unit with 38 beds, we bridge the gap between home and acute hospital services. Patients who are too unwell or incapacitated in the short term to stay at home but not requiring acute intervention get admitted to our unit to help them recover and ‘return to baseline’ function.

In addition patients who have had an ‘acute’ episode but are not yet ready to return home will come to us for a short period of rehabilitation to ensure they are safe to return to their home environment and their abilities are optimised.

We are a team of RGNs, ACP ,Healthcare assistants, Physiotherapists, Occupational Therapists and Rehabilitation support workers who aim to maximise our patients’ potential to enable them to return home. When that is not possible, we work closely with our adult social care colleagues to plan best outcomes.

We are looking for an RGN who enjoys working as part of a friendly and supportive team. It is hard work but very rewarding – planning the patient pathway, enabling them to progress and supporting them through setbacks is what we do best.

We are looking for an enthusiastic, experienced Radiographer to join our friendly Imaging Team at Bogner Regis War Memorial Hospital on a fixed term contract for 12 months starting in December 2024 to cover maternity leave.

Our department consists of a two well-equipped digital X-ray rooms and brand new Ultrasound and Bone Densitometry suites. The department performs a wide range of X-ray examinations from a variety of referral sources including Primary Care, Musculoskeletal services and the Minor Injury Unit on-site. We provide an X-ray and Ultrasound service for the local population and Bone Densitometry scanning for a wider population within West Sussex.

Data development

Design and develop major information systems to meet specification of others Responsible for the planning, development, review, update, upgrade and introduction of major new IM&T systems to meet user requirements. e.g. Redevelopment of aspects of patient care system, business systems, infrastructure, information systems.

Data innovation

You will seek out opportunities for innovation with new tools and the use of data and plan and organise a broad range of complex activities; formulate, adjust plans or strategies, plan specialist projects which impact across clinical and non clinical areas. E.g. Plan training delivery for a team of trainers or for a new system.

Data integration design

You will deliver data solutions in accordance with agreed organisational standards that ensure services are resilient, scalable and future proof.

Data modelling

With your understanding of the concepts and principles of data modelling, you will produce, maintain and update relevant data models for specific business needs. You will know how to reverse-engineer data models from a live system.

Metadata management

You will work with metadata repositories to complete complex tasks such as data and systems integration impact analysis. You will assist in maintaining a repository to ensure information remains accurate and up to date.

Problem resolution (data)

Being aware about the types of problems in databases, data processes, data products and services, you will debug and unpack errors raised within our manual and automated processes.

Programming and build (data engineering)

You will design, code, test, correct and document simple programs or scripts under the direction of others using T-SQL, Store Procedures, Basic C# and or low-code tooling.

You will have developed physical skills; advanced keyboard use; highly developed physical skills, accuracy important; manipulation of fine tools, materials. Inputting and manipulating data, information into computer databases, system; uses fine tools when working on IM&T systems

Testing

You will regularly design, code and execute test scripts to support ETL processes defined within our platforms

In addition to the day to day aspects of your role you will also have a responsibility to:

- Contribute to the development, migration and implementation of our Data Warehousing Solutions as they evolve and modernise over time.

- Participate in the planning sessions of business as usual work and in-flight projects

- Assist Senior and Lead Engineers in meeting our requirements for commissioner and national submissions within agreed timescales

- Attendance of meetings as and when required in relation to departmental modernisation, business critical system changes / issues, as-well as providing technical and non-technical advice on request.

- On occasion, to commit to out of hours work for key migrations, and or manual modifications to systems / datasets to minimise critical business disruption.

- Monitor scheduled jobs for failures, escalate and work with Senior or Lead Data Engineers to resolve.

RES provides clinical support to the pupils of Chailey Heritage School and a large number of outpatients referred from local services who cannot meet these clients’ needs. The Rehabilitation Engineering Service has a large in-house manufacturing workshop.

This post is situated within a specialist pre-dominantly paediatric rehabilitation engineering service (RES). The client group currently served, have severe and complex physical disabilities and therefore require bespoke assistive technology (AT) solutions in order to undertake activities of daily living. The equipment has a clinical role in the care of the child helping to prevent deformity in particular that associated with seating standing and lying.

This post is key in the successful provision of this equipment through excellent stores management.

To identify and work with the Management team in the development of processes relating to the stores and to take responsibility for ensuring the stock levels within stores are maintained at an operational level and are of an acceptable quality.

To work as part of the departmental team and, on occasion, provide technical assistance to the engineers.

To implement existing process for stock control, stock inspection, ordering and receipt of goods.

We are a multi-agency team which provide health and wellbeing education and promotion to adults and older people in their own homes, over the telephone and via other routes i.e. group sessions and community events. We also help deliver the NHS Health Checks program in the community and at outreach venues across West Sussex.

This will be a band 5 - 6 development post. This is a great role for a nurse with an enthusiasm for Health Promotion and a good knowledge of local health and social care. A band 5 nurse who wants to further develop their skills in early health intervention. It's a job that comes with flexibility and the scope for you to bring in your ideas on how to reduce health inequalities across West Sussex.

• To deliver NHS Health Checks to the public in outreach venues after appropriate training. This requires a holistic approach assisting people to access services designed to improve their health & wellbeing through behaviour change. This is part of our duties as providers to West Sussex County Council’s Public Health Department.
• To work closely with other PAT colleagues, primary care teams, social & caring services and the voluntary sector in the development and delivery of integrated services to avoid duplication and overlap.
• To contribute to covering the Duty Line along with other PAT colleagues.
• To contribute to scoping local services/initiatives and, along with other PAT colleagues, contribute to the maintenance of a database/directory to ensure accurate signposting and outcomes.
• With reference to routine data and other information available from West Sussex County Council’s Public Health Department and other sources, ensure that services and resources are targeted towards those populations where need is the greatest, thereby tackling health inequalities. This is carried out with guidance from the multi-agency PAT Board.
• The post holder is not responsible for leading or managing other members of the team however, will occasionally be responsible for pre-registration student nurses who are on a community placement within the PATs i.e. student shadows NHS Nurse Advisor on a visit or when carrying out an NHS Health Check.

Provide a holistic assessment of health, social and environmental needs of adults and older people living in the community.

Provide advice, signposting and onward referral to a range of statutory, voluntary and community services to enhance the health and wellbeing of adults and older people by utilising up to date knowledge of relevant national & local policy developments.

Work as part of a multi-agency team which includes health, social and caring services and voluntary sector staff;
