Responsibilities
Conduct comprehensive assessments of individuals and families to identify needs and risks.
Develop, implement, and review care plans in collaboration with service users and relevant stakeholders.
Provide direct support and interventions to individuals and families, ensuring their rights and needs are prioritized.
Maintain accurate and up-to-date case records and documentation in accordance with policies and procedures.
Collaborate effectively with other professionals, agencies, and community resources to provide holistic support.
Participate in multi-agency meetings and case reviews to share insights and contribute to the development of best practices.
Engage in continuous professional development and training to enhance skills and knowledge in social work practices.
Requirements
Qualified Social Worker with valid registration with the appropriate regulatory body.
Minimum of 2 years post-ASYE experience in a relevant social work setting.
Demonstrated experience in conducting assessments and developing care plans.
Strong understanding of social work legislation, policies, and best practices.
Excellent communication and interpersonal skills to engage with service users and colleagues.
Ability to manage a caseload effectively, prioritizing tasks and meeting deadlines.
Commitment to professional ethics and the values of social work.
Hours Per Week: 36.50
Start Time: 08:45
End Time: 17:15
Pay Per Hour: £18.92
