Social Worker (Program Coordinator) Rehabilitation Treatment Program: The Social Worker Coordinator Lead Transition Coordinator (LTC) is a Mental Health Residential Rehabilitation Treatment Program (MH RRTP) Lead whose primary responsibilities are to provide clinical evidence-based services and interventions; program development; caregiver, Veteran, and staff education; community outreach; resource development; continuous quality improvement activities; and evaluation/consultation. In addition, the incumbent must be a skilled communicator capable of developing effective partnerships with local, state, and federal agencies as well as nongovernmental organizations. The incumbent is administratively responsible for the clinical and daily operations of the MH RRTP, including the screening and admissions process and clinical functions, including discharge planning, care coordination, and daily programming. The LTC coordinates admissions for Veterans requesting admission into the MH RRTP and collaborates with the MH RRTP Screening Board regarding pending consults. A thorough knowledge of the scheduling and registration software packages is essential for the successful performance of assigned duties. Duty 1: Plans, coordinates, and assesses activities in specific areas of safety and occupational health necessary to the overall execution of the installation occupational safety program and integration with other specialties to ensure safe, secure, effective nuclear operations.

Duty 2: Plans and conducts investigations and analysis of mishaps and hazardous conditions to determine origin, causes, and contributing elements and prepares written evaluation reports concerning findings and proposed recommendations.

Duty 3: Plans, coordinates and evaluates activities necessary to the overall execution of a broad and diverse range safety program.

Duty 4: Develops and provides technical safety training/instruction to supervisors, employees, and/or unit safety representatives on a wide range of work operations areas, such as development and maintenance of job safety training outlines/plans (JSTOs), traffic safety techniques, motorcycle safety, proper storage of hazardous materials, common laboratory hazards, and building evacuation procedures.
