Our Knowsley intermediate care bed-based team is seeking a committed Band 6 Occupational Therapist to join our service, supporting the patients through shared care within their localities. We are seeking a highly skilled, enthusiastic, and proactive therapist to join our team. The service responds to the rehabilitation needs of adults 18+, who have multiple complex co-morbidities. Services are provided within bed-based units and patient's own homes. In addition to bed-based services, the team provide urgent care therapy response to prevent a hospital admission with an MDT approach where it is safe to do so.

We are looking to appoint a highly motivated, innovative and enthusiastic Senior Pharmacy Technician (Clinical Services).

Your role will involve working with the Chief Pharmacy Technician (Clinical Services) to manage and develop our technical ward services team.

This role requires excellent organisational and interpersonal skills, alongside sound knowledge of clinical ward services. Relevant experience and good written and verbal communication skills are also essential.

KEY DUTIES
• To work under the supervision of the Chief Pharmacy Technician Clinical Services to manage the operational provision of pharmacy services to wards and provide cover in their absence.
• Plan and manage own workload working independently.
• To work with the Chief Pharmacy Technician Clinical Services to performance manage band 5 pharmacy technicians and other staff allocated to ward services within the department including:

- The recruitment and selection process of pharmacy technicians and assistants

- Ensure all direct line reports have an annual appraisal and appropriate personal development plan which is in line with the strategic objectives of the department, ensure this is reviewed and updated in regular 1:1’s

- The conducting and recording of ‘return to work’ interviews and managing sickness absence for band 5 technicians according to the Trust sickness absence policy.

- To organise and conduct disciplinary meetings with ward services staff in accordance with Trust procedures.
• Provide pastoral and emotional support to the team when necessary.

• To perform final accuracy checking on prescriptions and inpatient requisitions which have been clinically verified by a pharmacist in accordance with Trust procedures and ensure dispensing errors are corrected by the appropriate member of staff.
• Labelling and dispensing of medication as required including the accurate input of data into the pharmacy JAC computer system, extemporaneous dispensing, and blister packs.
• To provide a specialist clinical technical service to a designated ward, in accordance with professional, departmental and Trust policies. This will include:

- the taking of a comprehensive medication history for new patients as soon as possible after admission using multiple sources of information

- Close liaison with external organisations e.g. nursing homes/community pharmacies, other hospitals, and GP practices

- Assessment of patients own drugs for reuse.

- Accurately transcribe medicines from EPMA to ICE discharge identifying any discrepancies to the ward doctor and ward pharmacist

- Processing of TTO’s at ward level in a timely manner to maintain patient flows in the Trust.

- Ordering medication in a timely manner to avoid missed doses, particularly of critical medicines.

- Monitoring for adverse drug reactions and submit ‘yellow cards’ to the CSM where appropriate.

- Counselling and educating patients on how to correctly take their medicines including advice on side effects and appropriate storage.

- Identify unintentional nonadherence to medicines and intervene where appropriate.

- Follow up patients to ensure interventions put in place have been successful.

- Monitoring for adverse drug reactions and submit ‘yellow cards’ to the CSM where appropriate.

- To participate in patient counselling and communicate complex information effectively to other grades of staff and patients and if appropriate use demonstrational techniques in the use of medicines to patients, carers, and other health care professionals, ensuring they understand fully any advice or instructions provided.

KEY DUTIES To work under the supervision of the Chief Pharmacy Technician Clinical Services to manage the operational provision of pharmacy services to wards and provide cover in their absence. Provide pastoral and emotional support to the team when necessary. To collate performance data for the Chief Pharmacy Technician Clinical Services to present to the Pharmacy Leadership Team. To write procedures for pharmacy ward technical services Assist the Chief Pharmacy Technician in the investigation of complaints including providing suitable responses and ensuring remedial action is taken where appropriate. CLINICAL & PROFESSIONAL RESPONSIBILITIES To perform final accuracy checking on prescriptions and inpatient requisitions which have been clinically verified by a pharmacist in accordance with Trust procedures and ensure dispensing errors are corrected by the appropriate member of staff. Labelling and dispensing of medication as required including the accurate input of data into the pharmacy JAC computer system, extemporaneous dispensing, and blister packs. To provide a specialist clinical technical service to a designated ward, in accordance with professional, departmental and Trust policies. Our inpatient unit at Newton Community Hospital is where patients needing acute hospital beds can continue rehabilitation, freeing up space for more unwell patients. We also provide urgent care at our Urgent Treatment Centre located in St Helens town centre.

We provide the Mersey Regional Burns & Plastic Surgery Unit at Whiston Hospital and the Spinal Injuries Unit at Southport Hospital to more than 4 million people across the whole of Merseyside, West Lancashire, Cheshire, Isle of Man and North Wales.

• To ensure families experience an excellent service during their time in the Paediatric Department dealing with any issues, matters arising, informal and formal complaints immediately and effectively, according to Trust policy.
• To be flexible and be prepared to support other Paediatric Clinical areas as required working closely with the Paediatric Senior Nurse Bleep holder, prioritising patient need accordingly at all times.
• To be competent at Safeguarding children and implementing the Child Protection procedures as appropriate.

CLINICAL & PROFESSIONAL RESPONSIBILITIES
• Maintain Professional Registration
• Adhere to relevant Code of Practice of Professional body.
• Optimum use of resources so that standards of care and safety are not compromised.
• Maintain effective communication systems, so that all staff can practice in an informed and professional manner.
• Maintain effective communication with patients, relatives, visitors and staff as required, i.e., Medical Coordinator, Senior Manager for ED, Directorate Manager.
• To investigate any untoward incident/complaint thoroughly.
• Liaise promptly with the Emergency Care Manager, Lead Nurse, or Directorate Manager
• To liaise with the Medical Coordinator around the needs and demands of departmental activity.
• Deputise for Senior Nursing staff as directed.
• Be fully conversant with the Major Incident Policy and be prepared to support its activation as required.
• Ensure effective infection prevention and control methods are followed.
• Maintain effective allocation of staff utilising appropriate skill mix.
• To delivery training and teaching packages of a competency-based nature to families, non-parent’s, carers, and staff in educational, social and voluntary settings.



SAFEGUARDING RESPONSIBILITIES

To be competent at safeguarding children and implementing the Child in Need and Child Protection procedures as appropriate includes the following skills, competencies and knowledge: -
• Be able to recognise child abuse.
• Be able to document accurately safeguarding concerns according to Laming recommendations, differentiating fact from opinion.
• Know who to contact / inform / seek advice from; and how to escalate safeguarding concerns according to Trust and Local Safeguarding Children’s Boards policies and procedures.
• Understand the steps in the child protection process.
• Know when and how to act, when and how to refer and the importance of following up a referral.
• Understand which groups of children are at risk of harm or neglect.
• Know how to share information – in writing, by telephone, electronically or in person.
• Know who to share information with and when, understanding the difference between information sharing on individual, organisational and professional levels.
• Work collaboratively with existing health services, social services and other agencies to ensure partnership working and sharing information to support children and families.
• Have an understanding of your own (and others’) professional roles and boundaries, including the Named professionals, trust managers and social workers.
• Have a working knowledge of the implications of key national documents / reports.

• To ensure that the recording of patient care is comprehensive, concise and in line with the NMC Record Keeping guidance.
• Ensure Emergency Department Patient Tracking IT systems are current and reflective of department activity.



TEACHING & TRAINING RESPONSBILITIES
• To maintain an effective learning environment for all staff, contributing to the training and assessment of staff
• To act as a role model and expert practitioner, providing support and guidance to nursing staff
• To participate in the annual performance review of identified staff Band 5 and below to identify their training and development needs.
• To participate in an annual performance review to identify the post-holder’s personal development needs
• To personally ensure fulfilment of the requirements for professional updating and to maintain a personal record for future registration purposes.



LINE MANAGEMENT/SUPERVISORY RESPONSIBILITIES
• Responsibility for designated nursing staff within team including appraisals, HR management and support in clinical practice in conjunction with the Practice Development Nurse and Emergency Care Manager
• To assist the Lead Nurse in planning, recruiting, retaining and developing staff to meet the service needs as required.
• To take an active role in facilitating staff to complete annual mandatory training, including safeguarding training, in accordance with Trust and departmental policy.
• To manage situations as they arise and report to the appropriate person as soon as is possible.
• To participate and support the induction and orientation of new starters.
• To be responsible for the annual appraisals of a team of nurses and health care assistants in agreement with the Lead Nurse, and to ensure their agreed personal development plans (PDPs) are operational.
• To manage the sickness and absence of a team of nurses and health care assistants working closely with the Lead Nurse and to escalate and manage any issues alongside the human resources/absence support team.
• To monitor and develop clinical competencies of a team of nurses providing supervision, support, mentorship and preceptorship is a key component of the role.



FINANCIAL RESPONSIBILITY
• Managing nursing team and skill mix appropriately, utilising bank and agency staff when appropriate within ED budget.



RESEARCH & AUDIT
• Contribute towards the setting, maintaining, and monitoring of standards of care within the Department by leading and participating in Emergency Department audits.
