• Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings using all elements of the nursing process.

Data Collection and Tracking
Performs work involving the collection, compilation, and/or tracking of data and statistical information in support of all the Extended Care programs which includes; Hospice and Palliative Care, Community Care, HBPC, to include Rural Health and Medical Foster Home, Social Work support as well as supporting the operations and staff in the Community Living Center (CLC).

The incumbent maintains databases and spreadsheets, tracks consults and their outcomes. Assists in the planning, review and reporting of data/statistical results of program/project studies. Establishes protocols for and reporting of data/statistical results of program/project studies. Establishes protocols for incoming data, organizes computerized data sets, and retrieves computerized data.

Performs a basic analysis of data and generates a variety of reports. Evaluates documents and data collection forms. Coordinates incoming data from a variety of sources. Review documents, reports, and/or applications for omissions and inconsistencies, and ensures data entry is complete and accurate.

Supports quality improvement activities through data collection and completes administrative requirements, reports, and event planning. Incumbent also performs data collection and reporting functions for other programs in the Extended Care Service line.

EmployeeEducationCoordination
Incumbent monitors staff completion of all mandatory education requirements. Incumbent works with Extended Care staff to remind them of education opportunities and find appropriate training. Monitors and enters staff training into the TMS system. Enters training requests into the SF-$182 share folder for the service and serves as the liaison with Human Resource and Education Service to facilitate employee applications for funding. Troubleshoots electronic training problems and trains staff on use of the online education modules.

Human Resources: Recruitment, Orientation, Boarding and Licensing
Incumbent is responsible for collecting resumes and scheduling interviews for openings in Extended Care. Once applicants are offered positions, incumbent works with Human Resources to schedule start dates and serves as a liaison between the new hire and Human Resources, ensuring that the hiring process proceeds smoothly and all requirements are met prior to start date. Incumbent completes SF-52 for new hires as well as for any changes to existing staff.

Medical Coverage Coordination
Incumbent serves as the coordinator for the off-tour coverage of the VA NJ HealthCare System Lyons Campus. VA NJ policy MS-04 requires a medical physician to be on station 24 hours a day, 7 days a week. Incumbent coordinates the hiring, scheduling, and time and attendance for the Medical Officers Of the Day (MODs). Incumbent serves as a liaison with the Medical Administrative Assistants (MAAs), Medical Administrative Service, to coordinate on-call schedules.

ADPAC
Incumbent deals with all computer and technology related issues for Extended Care. This includes troubleshooting technical problems with hardware and software, serving as Extended Care representative to Information Management Service on various committees and workgroups, and participating in the rollout of any new information management initiatives.

Correspondence and Minutes
Incumbent completes clerical and correspondence duties as assigned. Incumbent is specifically responsible for twice-weekly Nursing Home Screening Committee minutes which must be completed and circulated immediately following Screening Committee meetings. Incumbent also works on letters responding to patients, families, and outside stakeholders such as Congress and Veterans Service Organizations.

Program Resources. Services and Support
Performs work related to the acquisition and/or development of program information and resource materials/personnel to support the policy development and/or technical activities of Extended Care. Incumbent is responsible for assisting the AO in creation and/or implementation and enforcement of new Standard Operating Procedures/Protocols, as well as 'for adherence to these SOPs. Incumbent works with all Extended Care staff towards compliance with SOPs, and is responsible for educating MODs on these procedures and protocols.

Customer Service
Answers telephone calls throughout the day and to cover all phone calls when the secretary is out. Deal with internal and external customers to include patient and family concerns. Contact with patients and/or family is largely done through phone contact, writing letters and/or in person.

The incumbent processes all incoming requests to the facility for Release of Information (ROI) along with information required by the VA Regional Office through the Automated Medical Information Exchange (AMIE).

• Processes medical legal releases of information to attorneys and courts pertaining to Tort Feasor (Third Party Liability), Workman's Compensation, Labor and Industries and subpoenas.
• Screens and follows through on all incoming telephone calls from attorneys and insurance representatives regarding legal cases and requests under the Privacy Act, referring only problem cases to the Supervisor.
• Reviews and analyzes written and telephone requests to ascertain type of case to be developed.
• Develops and prepares cases for release to Regional Counsel and other appropriate parties.
• Contacts attorneys when necessary to obtain additional information to develop the case.
• Maintains files for all Privacy Act requests and subpoena cases for all cases being developed, processed or complete.
• Compiles tort cases for the Regional Office. This requires extensive investigations to track down x-rays and health records to compile a complete health record. Contacts all VA's to have them make a CD of all of the photocopied/printed/written to electronic media information, including digital x-rays.
• All paper health records are arranged in date order, numbered and several exact copies made for the Regional Office.
• All computer health records are downloaded to a CD including documents in VistA Imaging. For cases going to trial, this may include additional updates and coordination with the US Attorney's Office.
• Interviews veterans and insurance agents requesting information for Third Party Liability, Workman's Compensation, and Privacy Act cases for referral to Medical Cost Recovery and/or the Regional Counsel's Office.
• Evaluates validity of each request. Determines which information is to be released in compliance with existing laws (Privacy Act of 1974, Freedom of Information Act, and Health Insurance Portability and Accountability Act).
• Ensures that proper authorization exists before release is made. Processes the request to the requesting agency or individual.
• Inputs all requests into the ROI computer package for logging and tracking of these requests.
• Receives and directs callers and visitors. Receives and/or gives out forms and assists visitors and/or callers with the completion of forms or documents.
• Responds to questions from patients concerning services. Provides advisory and technical assistance to patients, administrative staff and professional staff regarding release of information.

The MSA coordinates with the Clinical Team to review the clinic utilization by using various reports (i.e. Clinic Utilization Statistical Summary), ensure that the clinic setup is closely monitored to effectively support the needs of the clinic, and make any necessary adjustments. The MSA develops/maintains effective and efficient communication with the patient, interdisciplinary team, VA Medical Centers, and other agencies (e.g. assist with communications during the inpatient to outpatient discharge; communicate with non-VA medical facilities by fax; facilitate/process secure messaging with the patient and team; develop and manage a tracking system for follow up care as such consults, tests, etc.)

Assist and/or stand in for the section chief with the following duties:

• Running daily section huddles, attending daily section chief huddle, and reporting section status.

• Pulls stock inventory and material for delivery; replenishes stock inventory; interacts with clinical and administrative customers and maintains records on stock levels;
• Communicates to the Lead or Supervisor any concerns related to supplies, equipment and procedures.
• Provides information to customers regarding medical supplies;
• Monitors expiration dates, rotates stock to limit outdates and removes items from use as required by outdates;
• Monitors and controls critical supplies;
• Performs daily inventory using bar coding equipment on a predetermined schedule that will allow for timely replenishment;
• Resolves differences between records and physical count of stock;
• Distributes items according to established or ad hoc requirements in computerized inventory management systems;
• Delivers supplies in a timely manner and ensures supplies and material delivered are in sterile and operational condition;
• Completes specialty cart preparation;
• Cleans assigned secondary areas in accordance with aseptic principles; and
• Receives supplies and material into the medical center's catchment area.

• Duties include transportation, decontamination, sterile preparation, and storage of Dental Instrumentation following all guidelines.
• Training conducted at Hampton VAMC SPS.
• Responsible for sterile preparation which is accomplished through inspection, assembly, packaging, and terminal sterilization of all trays, sets, and packs for use throughout the facility.
• Assemblies range from simple sets of four or five instruments to very complex sets of up to ninety instruments such as Total joint sets, femoral vascular, radical nephrectomy, abdominal vascular, chest sets, thoracic, orthopedic implant sets, carotid vascular, general surgery sets, etc.
• Assembles delicate micro-surgical sets such as micro-surgical ear set. ENT plastic set for procedures requiring plastic surgery, micro-surgical eye instruments, and other surgical specialties. These sets contain stainless steel instruments, titanium staples, screws, plates, and other prostheses which require expert care and careful attention.
• The incumbent assembles and selects the correct sterilizing method for a variety of costly and delicate equipment such as flexible resectoscopes and other flexible and rigid endoscopes.
• Tests instruments for functionality and assembles instruments in accordance with manufacturer's guidelines.
• Selects appropriate methods for sterilizing, balancing time constraints with possible detrimental effects of quicker methods.
• Independently prepares the full range of trays and sets necessary to be used in the wards, clinic areas, dental clinic and surgery.
• Updates trays and count sheets as needed.
• Considers surgery schedules and Clinic schedules along with considering time factors for processing order to determine how many items to stock based on the turnaround time for processing, preparation and frequency of use.
• Utilize computers to perform record keeping, tracking operations. Processes emergent requests, prepares tray lists, and updates files as required.
• Conducts inventory of all sterilized trays checking package integrity and expiration dates.

Develops plans, procedures, goals and objectives in support of the Aerospace Medicine and Force Health Management Programs using an extensive knowledge of Department of Defense (DOD), Occupational Safety Health Administration (OSHA), United States Air Force (USAF) and Air National Guard (ANG) directives, policies and standards. Programs include physical examinations and standards, flight medicine and related aero medical and force health management concerns. Interprets directives and applies standards to determine worldwide deployability of personnel. Advises and makes recommendations to senior officials on force health issues related to physical exams, standards and flight medicine activities to ensure compliance. Determines requirements, requests funding and administers accounts for procurement of supplies, equipment, and medical assessment in support of the physical exam process, flight medicine activities and related programs. Implements and maintains the information management systems utilized for aerospace medicine and force health protection programs. Systems include preventive health assessment (PHA) and individual medical readiness (PIMR), Aero medical Information Management Waiver Tracking System (AIMWTS), Physical Exam Processing Program (PEPP), Grounding Management Information System (GMIS) and other related programs. Compiles and completes various special and recurring reports utilizing these systems. Functions as the health systems program administrator. Identifies, develops and implements training programs which support mission essential aerospace medicine and force health management requirements. Administers the Physical Examinations and Standards (PES) program and serves as a medical expert on related issues. Develops procedures to evaluate the effectiveness of the PES program and implements processes to ensure individual medical readiness and deployability. Reviews and resolves concerns or conflicts made by the member, supervisor, and/or commander. Identifies reports, formulates and recommends solutions to command personnel for resolution of compliance and delinquency trends and patterns which could negatively impact medical readiness. Addresses complex medical issues with members, supervisors, commanders, state staff, and higher headquarters. Identifies and analyzes abnormal findings through research using medical references and professional consultations. Using established guidelines, advises member of findings, determines additional requirements and refers to medical professionals as needed. Edits, reviews and validates physical examinations to determine qualifications for initial or continued military service. Conducts paraprofessional evaluations (an in-depth screening or interview for the purpose of gathering data for the healthcare provider) and interprets results. Manages the Reserve Component Periodic Health Assessment (RCPHA) program. Reviews military unique individual readiness requirements. Identifies documents and ensures member is notified of abnormal laboratory or diagnostic results. Tracks PIMR and reports noncompliance information to leadership. Provides training for unit RCPHA monitors. Acts as the Medical Group customer service expert. Addresses complex issues using independent judgment. Provides technical medical assistance to medical inquiries and responds to inquiry by researching, compiling information, generating response and providing information through written correspondence, telephone, electronic or in-person response. Monitors the medical status of military personnel. Identifies and profiles personnel with medical conditions impacting duty performance or assignment restriction. Reviews and validates profiles disqualified for worldwide duty. Notifies and advises commanders, supervisors and unit deployment managers when a member's medical/dental condition or duty restriction affects deployable status. Determines the need for and initiates Medical Evaluation Board (MEB) actions for duty related medical conditions. Coordinates, prepares and/or processes MEB with active duty facility. Serves as the primary local interface with the Military Personnel Flight (MPF) for MEB/Physical Evaluation Board (PEB) process, issues, and concerns. Advises individuals and commanders on appropriate procedures and responsibilities. Coordinates with military and civilian providers for required medical tests, studies and medical evaluation board proceedings. Uses applicable medical references to code medical conditions and treatments; consolidates medical information and forwards to higher headquarters as required. Determines the need for and initiates worldwide duty evaluations for non-duty related actions. Coordinates, prepares and/or processes actions for higher headquarters review. Performs additional duties as assigned.
