In addition, the HUD-VASH Social Work Supervisor will provide high quality social work case management, care and services to a small caseload of assigned Veteran patients enrolled in the HUD-VASH Program. This includes active case management and care coordination, clinical assessment and treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers/agencies as needed by the client. The HUD-VASH Social Worker Supervisor will function as an integral member of the HCHV/HUD-VASH treatment team, actively participating in patient assessment, treatment planning, case management, and provision of a full range of treatment and services to all assigned Veteran patients.

The HUD-VASH Social Work Supervisor will provide administrative, technical and clinical supervision necessary for accomplishing the work of the organizational unit for assigned social work staff within the HUD-VASH Program.

Supervisory and administrative responsibilities will comprise approximately 70% of the HUD-VASH Social Work Supervisor's assigned duties.

The Supervisory Social Worker actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.

The Supervisory Social Worker takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested.

The Supervisory Social Worker performs accountability functions: 1) document all contacts with patients/families in the medical charts; 2) completes statistical and other reports in accordance with VA Medical Center and respective professional discipline service policies.

The Homeless Grant & Per Diem Program is an essential part of VHA and is vital for providing safe transitional housing and supportive services for homeless veterans. The incumbent supervises a group of professional social workers and support staff, and provides administrative, technical, and clinical supervision necessary for accomplishing the work of the unit. You will need to communicate with our services users and their next of kin effectively and appropriately, whilst also giving then the opportunity to communicate their needs as well.

Activities will include taking part with clinical tasks, as well as aspects of Personal Care and Moving & Handling.
