An exciting opportunity for a Outreach Health check worker to join the Essex Wellbeing Service.
The role involves conducting NHS Health checks in a range of settings, including our own clinics, at workplaces, promotional events, on our mobile unit and at GP practices.
Internal candidates will be offered a secondment opportunity.
MAIN DUTIES AND RESPONSIBILITIES:
• Engage with identified clients to deliver the NHS Health Check.
• Provide agreed health improvement advice and signpost information.
• Drive the NHS health check mobile unit when required to various locations in Essex.
• Promote the equality and value the diversity of individuals.
CLINICAL/PROFESSIONAL DUTIES:
• On successful completion of training to perform NHS health check on clients and give feedback according to agreed protocols.
• Signpost and support individuals in accessing services and organisations to enable them to change their behaviour to improve their health.
• Engage with individuals in local communities which have identified health inequalities.
• Help individuals identify how their behaviour and context might affect their health and wellbeing.
COMMUNICATION SKILLS:
• Communicate verbally with individuals about the results of their NHS Health Check.
• Communicate with colleagues and other organisations using the telephone and email.
• Provide information to individuals about the relationship between behaviours and health.
• Enable individuals to develop their knowledge and skills about health and wellbeing.
• Signpost individuals to other agencies for information, support and resources.
PLANNING AND ORGANISATION:
• Manage time effectively to deliver NHS health check in the agreed time allocated.

• Alert manager to any issues of concern relating to work (including concerns about individuals or work in the community).

• Signpost and support individuals in accessing services and organisations to enable them to change their behaviour to improve their health.

POLICY AND SERVICE DEVELOPMENT:
• Contribute to the planning and development of health improvement services.
PLANNING AND ORGANISATION:
• Manage time effectively to deliver NHS health check in the agreed time allocated.
FINANCIAL AND PHYSICAL:
• Order and maintain supplies of health promotion literature and resources.
• Awareness of the need to deliver service within budget. Contribute to discussions around budget planning.
INFORMATION MANAGEMENT:
• Build up and maintain knowledge of contacts and organisations within the community.
• Respond effectively to referrals for health improvement services.
• Inform team of ways in which local services can be improved to improve the health and wellbeing of the local community / barriers that individuals are experiencing in changing their behaviour.
EDUCATION AND RESEARCH:
• Provide evidence-based information to individuals about health and wellbeing.
• Help individuals to access and use local services.
• Take an active part in developing own knowledge and skills.
• Support service managers with the collection of data to monitor service performance.
FREEDOM TO ACT:
• Will be able to deliver health checks independently
PHYSICAL EFFORT:
• Transporting, lifting and handling of health improvement resources and equipment
• Moving tables and chairs in venues.
