Service Care Solutions has a new and exciting opportunity which has become available working as a Leaving Care Personal Advisor with Cheshire East Council

Cheshire East Council are currently looking for someone who is happy to work 37 hours per week.

Job Purpose

As a Personal Advisor, you'll act as a corporate parent for young people preparing to leave the care of the local authority. You will provide qualitative assessments, ongoing support, and guidance to empower care leavers in developing the necessary skills for independent living. This role aims to support care leavers in realizing their full potential, contributing to their emotional, social, and financial independence.

Key Responsibilities

• Relationship Building: Establish and maintain meaningful relationships with care leavers, ensuring their views are heard and their needs are central to all support efforts.
• Pathway Planning: Participate in the assessment, preparation, and review of pathway plans to equip young people with essential life skills for independence.
• Service Coordination: Facilitate access to relevant services, working collaboratively with partner agencies to support the progress and well-being of care leavers.
• Documentation: Maintain comprehensive, up-to-date records of interactions and services provided using the Council's electronic systems.
• Crisis Response: Provide flexible, rapid response support in crisis situations, which may include work outside of typical office hours.
• Equal Opportunities: Deliver services with a caring approach that respects diversity, including race, culture, disability, gender, and sexuality.
• Advocacy and Safeguarding: Promote the best interests of care leavers, advocate for their rights, and take necessary measures to ensure their well-being.

Requirements

Essential

• Qualifications: NVQ Level 3 in Advice and Guidance, Health and Social Care, or equivalent education.
• Knowledge & Skills: Understanding of young people's issues, assessment methods, and education/employment trends. Ability to work effectively within a team and build partnerships across diverse groups. Proficient in IT applications and effective time management.
• Personal Competencies: Excellent communication skills, flexibility, adaptability, and commitment to equal opportunities.
• Other Requirements: Willingness to travel across Cheshire East and flexibility to work evenings and weekends when required.

Main Responsibilities

• Be vigilant and proactive in reporting any safeguarding concerns you may have about vulnerable individuals in your care
• Proactively assist in seeking out new opportunities for growth and development of the service whilst protecting and safeguarding the Company’s values and culture
• Ensure that all new team members are appropriately recruited into the Company and analyse initial training and development needs
• Work in conjunction with the training team to ensure all staff receive the correct training
• In conjunction with the management team, recruit and select new staff taking an active part in the interview process
• Lead staff in enabling the service users to live fulfilling lives based around their individual needs and aspirations
• Attend interviews for potential new packages of care and support as required with the senior management team
• Ensure appropriate risk assessments are in place and implemented to safeguard the wellbeing of service users
• Implement induction/assessment programmes for all new service users, in conjunction with the wider management team
• Understand and promote a person centred and needs led approach to deliver a quality service which fully complies with the Eden philosophy, values, policies and procedures
• Understand the wider implications of the business in relation to the commissioning process, audit and inspection (CQC etc.)
• Always promote a professional image of the service and the Company.
• Establish and maintain positive working relationships with key stakeholders i.e. Adult Social Care & Health, CCG’s etc
• Ensure effective communication is established and maintained throughout the service and with external agencies, service users and their circle of support
• Ensure effective partnership working with external training and provider / agencies (i.e. NAS, SaLT and psychology services)

The Ideal Candidate

• Strong supervisory skills with the ability to support and develop a team
• Be qualified to a minimum standard of Level 2 in Health and Social Care
• Ideally you will have previous experience in a Supported Living or Residential service
• Experience of working with adults with Learning Disabilities or Mental Health conditions is preferable
• Be willing to undertake further training or development (Health and Social Care Level 3 or 4 and Diploma in Management Level 5 if applicable)
• Ability to work effectively in a fast paced, demanding role

The post holder will take a lead role in a comprehensive review of Community Mental Health with a view to delivering evidence based integrated care within Western and Eastern Cheshire Secondary Care Mental Health services.
