We are currently recruiting for a Home Support Scheduler to join our Home Assessment Team – Out of Hours in North Lanarkshire.
This is an exciting opportunity to join a team whose focus is on putting people first, making sure they get access to the right
discharge for those individuals who are ready to return home from hospital?
As part of our new Home First Team you will work closely with Health & Social Care and Acute colleagues whose focus is prevention of delays in hospital discharge arrangements and ensuring individuals and their families receive the best possible support in those first days after returning home. Working closely with the Home Support Team Leader, you will use a dynamic scheduling tool to ensure the people we support receive a prompt and efficient person-centred support which promotes greater independence and choice.
To undertake this role, you will need a positive approach and be able to communicate effectively with a range of individuals including service users, their families, and other partners/agencies. We are looking for someone with excellent organisation skills, is resourceful and works well under pressure. Previous experience working in a social work or home support related environment is essential. Possession of a SQV Level 3 or equivalent qualification is desirable however successful applicants who do not hold this qualification must demonstrate a willingness to work towards achieving this within a reasonable timescale.
Work Pattern-
Two Week Rota:
Week 1
Mon Tue Fri Sat Sun - 12:30-22:00 (with 30min break)
Week 2
Tue - 12:30-20:00 (with 30min break) Wed Thur - 12:30-22:00 (with 30min break)

*For all casual/sessional roles, you will receive 12.1% of your hourly rate for every hour worked in total recompense for annual leave, including public holidays. These rates will be indicated separately on your payslip. You will only be paid for the hours that you work.
