The RN Care Manager works as a responsible member of the nursing team and works cohesively with Veterans, family members and an interdisciplinary team of professionals. The RN Care Manager must be knowledgeable of care coordination guidelines, documentation requirements and ensure accuracy in the electronic health record. The RN Care Manager must provide the Veteran/family members with Veteran centered services which will allow the Veteran to make the best-informed decision, increase Veteran satisfaction and provide a reliable process to provide quality care in a timely manner. Collaborates to promote positive clinical, financial, and psychosocial outcomes for a caseload of Veterans with a variety of chronic and acute diagnoses. Assists staff, patients, and families / significant others with post-hospital planning by determining appropriate community resources and evaluating patients' nursing care needs. Focuses on the patient in the context of family/community by integrating an assessment of living conditions, individual and family dynamics and cultural background into the patient's plan of care. Provides post discharge follow-up contacts for post hospital patients to ensure their understanding and compliance with medications, discharge instructions, follow-up appointments and appropriate level of care. For Primary Care, contact is made within two days post-discharge. Serves as an advocate in the care management process by supporting decisions, clarifying options, and problem solving with the interdisciplinary team. Complies with health, safety and environmental rules and procedures. Performs work in a manner that enhances the safety of the work environment. Ensures strict adherence to federal, state, and local codes concerning health, safety, and the environment as applicable within the performance of his/her duties. • Tracking and reporting on Veterans who have attempted suicide or are at high risk for suicide
• Responding to referrals from the Veterans Crisis Line (VCL) and self-referrals from other sources
• Ensuring that all VA staff members have received discipline-appropriate suicide prevention training (VA S.A.V.E. training or equivalent)
• Collaborating with community organizations and partners to increase awareness of suicide prevention best practices and providing training to individuals who have contact with Veterans
• Providing subject matter consultation to providers on available resources for suicidal individuals, as well as expertise and direction in the areas of system design to prevent deaths by suicide within their local VA medical centers
• Working with providers to ensure that monitoring and treatment is intensified for high-risk patients while making sure that those who are at elevated risk receive education and support about approaches to reduce risks
• Reporting to OMHSP on Veterans who attempted or died by suicide and sending OMHSP requested data, which is used to determine characteristics and risks associated with these groups of Veterans
• Ensuring that medical centers have established a High-Risk for Suicide List (HRSL) of Veterans and a process for establishing Patient Record Flag Category 1 - High Risk for Suicide (HRS-PRFs).
• Thorough knowledge of the program objectives to ensure that actions taken are appropriate.
• Disseminate resources and implement effective, evidence based strategies for reducing incidence of suicide and suicide behavior.

This position involves a multi-grade career ladder. The major duties listed below represent the full performance level of GS-9. • Provides orientation and situational technical advice to physicians regarding VA policies and procedures.
• Assists medical center staff with medically related problems, including involuntary commitments, orders of protective custody, and management of unruly or severely disturbed patients.
• Performs for administrative matters as the MOD does for clinical matters, working as a team to carry out top management responsibilities during other than regular working hours.

Provides clinical case management and therapeutic intervention services to assigned Veterans.

Provides evidence based therapeutic interventions in encounters with Veterans to promote recovery and successful transition to community living and care. Interventions will be specifically delivered in treatment modalities for addictions, substance abuse and dependence, complex mental health conditions, and PTSD.

Designs, plans and conducts various psycho-social educational classes and process groups related to helping the Veterans understand mental health and their needs and the impact on them and their whole life system of support.

Provides clinical services to Veterans admitted to the RRTP, which includes DCHV (Domiciliary Care for Homeless Veterans) programming, addictions treatment, mental health treatment, and PTSD treatment.

The incumbent is responsible for completing the following duties/responsibilities:

• Conducts psychosocial assessments to determine needs of Veterans and family members.
• Participates in the treatment planning process with other disciplines.
• Coordinates and refers care with other disciplines.
• Provides individual, family and group counseling to support treatment and recovery of Veterans experiencing addictions, complex mental health conditions, and PTSD.
• Provides consultation/education to Veterans and families on community resources, advance directives and VA benefits.
• Acts as liaison between VA and community resources, to include marketing and public relation duties as needed.
• Provides case management to Veterans. Engages family and other supports to promote recovery for Veterans.
• Assists the Veteran and family in understanding contributing factors to problems, discusses solutions, and encourages lasting changes to reduce stressors.

This position is located in the Military Duty (AGR) Management Branch of the Support Personnel Management Office. The purpose of this position is to serve as the full-time health systems representative on all AGR Medical Department (AMEDD) related matters, and to function as a key assistant to the Military Personnel Management Specialist(MPMS) in administering the AGR medical program of the state on a day-to-day basis.

Performs work to manage health services activities, including plans and operations, managed care, human resource management, logistics management, patient administration, budgetary and fiscal management, medical manpower, medical facility management, biometrics, medical recruiting, and aeromedical evacuation. Work may include directing the hospital accreditation program and management improvement studies. Provides support to health care management officials by analyzing, evaluating, advising on and/or coordinating health care delivery systems and operations. Manages and performs public health activities and programs. Provides administrative oversight to the Preventive Health Assessment and Individual Medical Readiness Program and the related Occupational Health programs. The duties vary with the type of assignment and grade of the position.

Duties include but are not limited to the following:

• Initiate and carry out a variety of administrative, laboratory and echocardiogram duties in support of patient examinations in the Primary Care Clinic and CBOCs.
• Provide and perform administrative, venipuncture and echocardiograms in coordination of patient examinations and advise the professional staff on administrative order request for venipuncture.
• Ensure all orders for venipuncture and EKG;s are completed.
• Perform standard diagnostic tests or procedures such as electrocardiograms, attaching electrodes to chest area of patient for testing results transmission, venipuncture for collection of blood specimen, according to written protocol or as directed by the physician or other professional staff.
• Responsible for accurate labeling and packaging of samples for transport to facility lab.
• Relay telephonic, e-mail, and face to face messages to team members.
• Analyze situations to determine what decision can be made or if advice of a higher authority is needed, especially for problems that may involve public relations.
• Receive and review new lab and EKG orders.
• Obtain all key information needed to initiate administrative processing of physician orders in a timely manner.
• Assure that patients are comfortable and treated courteously, enthusiastically and in a timely manner.

• Comply with all OSHA safety/fire prevention rules/regulations, using protective equipment when required.
• Promptly report all accidents and notifies supervisor of unsafe or unhealthy conditions in the workplace.
• Contact Social Worker on behalf of patient's personal needs as needed or requested and the travel office to arrange necessary transportation following established guidelines.
