We have a fabulous opportunity for you to join our fantastically skilled North Tyneside Transitions team as a Support Coordinator! You’ll help our customers lead meaningful lives with person-centered support for daily activities, appointments, and more. Our customers, all of whom are supported under Section 117 aftercare from the Mental Health Act, rely on us to live independently in the community.
You’ll work closely with social workers and treatment teams to ensure wrap-around support. You’ll assist customers transitioning from hospital into our fully-furnished properties or through floating support for those living in their own homes. For Homegroup residents, you’ll also handle tenancy management, benefits, and property checks (with full training). Based at Railway Terrace, you’ll cover North Tyneside alongside an inspiring, supportive team ready to welcome you!
Typical day as a support coordinator
- Helping customers with daily living activities such as making meals, medication prompts, shopping, budgeting, maintaining their tenancy.
- Carrying out risk assessments, support planning, goal setting and regular reviews.
- Attending MDT meetings for customers and working collaboratively with other professionals involved in customers care.
- Use technology to complete admin tasks (customer notes, safeguarding, incident reporting, support planning)
- Providing tenancy management including ensuring customers have all correct benefits as well as monthly property checks (Which can be added into a support session) as well as reporting any repairs.
- Cuppas, chats and catch ups, Plenty of time to collaborate with colleagues too.

You bring
- Passion to support our customers to live their best life, working collaboratively with an eye for detail.
- Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
- Experience of coordinating and assessing customer referrals.
- The ability to work on your own initiative, remain calm under pressure and have a resilient approach

Job details
- Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
- You’ll need an Enhanced DBS check done and we pay for that.
- Covering the North Tyneside area, primarily working in the community, our office base is Railway Terrace, North Shields and will work closely with the wider teams.
