Are currently looking for some experience at band 7 level in a dynamic, supportive team? The role will include working in in-patients, out–patients and the community. We can consider hours on request.

As an autonomous practitioner, the post holder will undertake a complex clinical workload and be an expert in the advanced assessment, clinical diagnosis and treatment of neurological conditions, including physical, functional, and environmental factors in in-patient, out-patient, the individuals’ home, and other community settings.


The post holder will clinically manage and supervise physiotherapists, therapy assistants, and students and support other clinical colleagues in the team as required. This will include supporting learning and other development activities.


The post holder will take a lead role in planning, coordinating, delivering, and evaluating the Neurological pathway within the Community Rehabilitation Team on a day-to-day basis, whilst working closely together with other internal and external stakeholders.

5. Act as a knowledgeable resource regarding physical impairments following Neurological injury to other members of the multi-disciplinary team.
6. Triage clients are referred to the service so that therapy is given to those identified with the highest risk and need.
7. Demonstrate skills in dealing with complex issues to generate appropriate strategies for caseload management.
8. Ensure that clients and relatives are involved with the planning and implementation of their care plans wherever possible.
9. Provide specific goals to be incorporated into a patient’s rehabilitation program.
10. Design and implement individual specialist therapy programs and evaluate outcomes.
11. Develop effective relationships for information sharing and service development with GPs, social services, and other members of the MDT at the base at Epsom and in community hospitals.

Surrey Downs Health & Care

An exciting opportunity for a newly qualified or developing Band 5 therapist wanting to work in the specialist field of community neurorehabilitation.

We are looking for an enthusiastic individual to join our specialist neuro rehab multidisciplinary team working in Surrey Downs. You will be motivated to develop skills in the assessment and management of acquired complex communication and swallowing disorders and will support patients and their families with rehabilitation across community settings, through education, setting of patient-centered goals, and the use of evidence-based practice. You will have good access to the wider team, including regular supervision, and internal/external training opportunities, along with opportunities in the use of videofluoroscopy.

The post holder will be an experienced or newly qualified SLT with relevant qualifications and some expertise in adult-acquired swallowing and communication difficulties. This post will require knowledge, skills and judgment across a broad range of disorders and client groups including stroke, dementia, traumatic brain injury and progressive neurological conditions. The post holder will have their own caseload and would be responsible, with support as needed, for providing a service to adults in the community including domiciliary visits, nursing homes, community hospitals and End of Life Care. Collaborative working occurs within the multi-disciplinary team ensuring patient-driven goal planning and intervention. The post holder may also provide support within the stroke-supported discharge service. S/he will assess, plan and implement individual therapy programs and advise adults presenting with a wide range of communication and swallowing problems. Educating carers, family members, other professionals and supervising students are also an important part of the role.

S/he will participate in research, audit and development activities within the department. Working collaboratively with colleagues, as well as health care and social service personnel s/he will regularly be involved in departmental service development. S/he will work independently within the framework of guidelines and policies, with support provided as appropriate.

The Post holder will:
1. Provide SLT services to adults referred to the community service for communication and swallowing intervention.

2.Provide a multi-disciplinary assessment of adults taking into account necessary factors including premorbid skills and environmental factors.

3. Provide a differential diagnosis of the communication/swallowing impairment, clearly define strengths and needs, identify SMART goals and offer an appropriate therapy program, taking into account the level of understanding, motivation, insight, level of support and self-management potential.

4. Contribute to training within the department and to other professionals as well as to families/carers and SLT students (to degree level), SLT assistants and support workers.

5. Act as a knowledgeable resource regarding communication and swallowing disorders to other members of the multi-disciplinary team.

6. Triage clients referred to the service, so that therapy is given to those identified with the highest risk and need.

7. Demonstrate skills in dealing with complex issues to generate appropriate strategies for caseload management.

8. Ensure that clients and relatives are involved with planning and implementation of their care plans wherever possible.

9. Provide specific goals to be incorporated into a patient’s individual rehabilitation programme.

10. Design and implement individual specialist therapy programmes and evaluate outcomes.

11. Develop effective relationships for information sharing and service development with GPs, social services and other members of the MDT in the department and in community hospitals.

12. Demonstrate clinical effectiveness by use of evidence based practice to measure progress and have informed discharge criteria.

13. Undertake research and development activities.

14. Work as part of the multi-professional team providing integrated services for adults with acquired neurological disorders.

15. Propose changes to services/policies within SLT department.

16. Maintain timely and accurate case notes and provide written reports reflecting specialist knowledge.

17. Develop a working knowledge of relevant procedural and national/local drivers that impact on community SLT including Vulnerable Adults, NSF for older people, National Stroke Strategy etc.

18. Follow Royal College of Speech and Language Therapists and Health Professional Council policies and professional codes of conduct.

19. Supervise the work of assistants and advise and provide feedback about their development.

20. Maintain and be responsible for re-ordering stock/equipment needed for the post.

21. Ensure that relevant data and information is entered onto the recording system and provide statistical information as required.

22. Take responsibility for own CPD by attending in-service training, study days and MDT meetings and by ensuring membership of appropriate local, national specialist interest groups.
