Band 8a Lead Nurse for District Nursing and Guided Care Matrons
To work in collaboration with partner organisations providing strong operational nurse leadership to the District Nurse (DN), Guided Care Matrons as part ofthe Care Connection Team (CCT) to ensure the delivery of high standards of patient centred nursing care for the residents of Hillingdon.

Due to staff career progressionan opportunity has arisen for a 1 year secondment to co-lead the Hillingdon District Nurse and Care Connection Teams (CCT) . Alongside three other leads, you will be responsible for the leadership, management and development of the District Nurse Service and the Guided Care Matrons and Care Co-ordinators within the Care Connection Teams (CCT).
The District Nurse Teams provide community-based care to housebound, people over 18 years old that require nursing input. They also support ambulant patients that require complex wound dressings in community-based clinics.
The Care Connection Teams are part of the Hillingdon Health Care Partnership (HHCP) . They proactively support people over 18 years old that have long term conditions and are at risk of hospital admission. The Care Connection Teams are aligned to neighbourhoods, based in GP practices. They are a team comprised of Guided Care matrons, GP’s, Care Co-ordinators, Pharmacists, Health and Well-being Officers and Mental Health Nurses.

The successful candidate will join a friendly, experienced, supportive team of Service Leads. You will be one of a team of four Service Leads based at Beaufort House in Uxbridge for a 1 year secondment.
Service Leads are encouraged to be innovative and committed to service improvements including playing an active role in change management.

You will be responsible for ensuring all aspects of the day to day operational delivery of the District Nurse Service& Guided Care/Care Co-ordinator element of the CCT’s. You will be expected to be visible, providing clinical support when necessary. You will be working alongside senior management, commissioners, the CCG and multi-agency providers, families and carers.
You will be expected to work closely with managers from partner organisations to line manage the Guided Care Matrons and Care Co-ordinators to meet CCT targets. You will need to be forward thinking and able to provide integrated leadership. This role requires excellent leadership, organisational, time management and communication skills. This role provides an opportunity to play an integral role in shaping community services for the future.
The successful candidate's main duties will be:
• Act as a role model- demonstrating CNWL core values
• Day to day operational management of the District Nurse, Guided Care Matron and Care Co-ordinators.
• Act as a point of escalation
• Manage the service to consistently meet targets and KPI's
• Over see the service budgets
• Manage quality Governance to provide a safe effective service
• Promote innovation
• Engage with Patients, relatives and carers to ensure services meet their needs
• Support the Adult Services Manager
