GENERAL OVERVIEW:

This job provides direct care to patients and assists with non-direct patient care unit coordination. The incumbent in this position is expected to perform basic nursing procedures related to care and comfort of patients in stable condition, and have knowledge of patient population needs in the assigned work area. The incumbent works under the general supervision of the Registered Nurse for patients in stable condition. Will work under the direct supervision of the Registered Nurse in the care of the acutely ill and/or complex patient. The incumbent receives specific work assignments from the Charge Nurse and/or Care Team Leader which are both written & verbal and is expected to demonstrate evidence of efficient time management by completing the work within the scheduled time. In addition to clinical responsibilities, the incumbent is expected to perform a variety of duties related to the coordination of activities on the inpatient unit. ESSENTIAL RESPONSIBILITIES:

• Provides, obtains or performs and documents direct patient care and activities such as: activities of daily living, skin survey, vital signs, ambulation, specimen collection, intake and output, meal consumption, height and weight, blood glucose testing/monitoring, point of care testing, bladder scanning, transporting of patients and their medical record, documentation of O2, early mobilization and ambulation protocols, assistance with exam, treatments and procedures. (30%)
• Participates in quality and patient satisfaction initiatives which may include bedside shift report, purposeful hourly rounding promoting safety such as falls prevention, hand washing, isolation protocols and maintaining a clean and orderly environment. (30%)
• Accurately documents vital signs, weights, I&O, restraints, specimen collection, and all other treatments. Report changes in patient's physical, mental and emotional conditions, as well as any issues or concerns identified to nursing staff. (20%)
• Participates in unit activity such as supply stocking and equipment/appliance cleaning, checks and retrieval. (20%)
• Performs other duties as assigned or required.

QUALIFICATIONS:

Minimum

• High School Diploma or GED
• CPR Certification
• Limited or no experience: Must be able to attend 2 weeks of training
• Ability to read, write and follow oral and written instructions
• Ability to perform basic mathematics
• Basic computer skills

Preferred

• Acute care experience with in the last 3 years
• Currently enrolled in ASN or BSN program. Preference given to those enrolled in Associates or Bachelor’s degree Nursing programs and completed at least one clinical rotation

Job Description:

Responsible for the day-to-day clinical and administrative operation of Centerstone's FACT Team. Responsible for compiling and submitting all required reports necessary to satisfy internal and regulatory requirements. Will also function as a practicing clinician.  Will ensure that the program functions within established regulatory operating procedures. Team Leader-FACT Team, Ft Myers, Licensed Clinician Required.

Minimum Qualifications

Must possess a Master's Degree in an appropriate social service discipline. Must be licensed in the state of Florida as a Registered Intern, LMHC, LMFT, or LCSW, and have experience with mental health and/or co-occurring issues. Valid Florida Driver's License with a clear driving record and current auto insurance. 

Preferred Qualification:

Previous experience working in an assertive community treatment team setting preferred. 1 year of supervisory experience preferred.

Special Skills, Knowledge and Abilities:

Knowledge and understanding of the characteristics of severe and persistent mental illnesses, the FACT model, legal system, and mental health and community resources. Highly motivated, self-directed, excellent supervisory skills with program management experience desirable.

Essential Functions: 

• Participate in staff recruiting, interviewing, hiring and ensuring that hired staff receive the orientation and training required by the policies and regulations of Centerstone.

• Direct and coordinate, with the psychiatrist, the admission process. 

• Schedule the initial screening, admission interview, develop and coordinate the initial assessment and treatment plan, assign the most appropriate staff for the client's individual treatment team.

• Coordinate the comprehensive assessment for each client and provide clinical supervision of the development of the comprehensive assessment and individualized treatment plan.

• Direct the day-to-day clinical operations of the team including staff work hours to assure coverage, lead the daily organizational staff meetings, treatment team meetings; maintain ongoing evaluation of client's status in order to do appropriate planning and coordination of treatment activities to meet client's changing needs.

• Develop and administer the FACT program budget line items to purchase necessary supplies and to cover travel and transportation costs.

• Supervise medical records management to assure compliance with all regulatory agencies and corporate policies and procedures, train staff on medical records requirements.

• Conduct clinical assessments and provide direct clinical treatment, rehabilitation, and support services to clients. 

• Regularly review client records and use reviews as part of clinical supervision and training.

• Carry out and document performance improvement activities and review use of program services.

• Develop and maintain program policies and procedures and revise as needed and/or as required by corporate policy. 

• Develop and maintain relationships with other staff, law-enforcement, human service agencies and community resources. Performs on-call duty for crisis intervention and for staff supervision and consultation.

Work Environment/Physical Demands:   

This position is located in Ft Myers. This position operates in a professional office environment and in community based setting.  Sits, stands, bends, lifts and moves intermittently during work hours.  Is subject to frequent interruptions.  Is involved with clients, residents, family members, personnel, visitors, government agencies/personnel, etc., in a behavioral health setting.

Position Type, Expected Hours of Work and Travel:  

This is a full-time position, and the expected work hours are 40 hours per week, Monday - Friday with evening, weekend, and holiday hours required.  On call rotation required. Local travel is required.

Our Med Surg Neuro Team strives for excellence in this fast-paced, dynamic setting. We are looking for a Patient Care Assistants to join our dedicated, multidisciplinary team. This position will require transporting patients during admissions and discharges along with handling patient information. POSITION SUMMARY
This position provides routine patient care to promote patient comfort and contribute to the overall patient plan of care. This position is responsible for on-going patient data collection and documentation in the medical records and provides a variety of unit support functions which contribute to the overall efficient operation of the nursing unit/department.

CORE FUNCTIONS
1. Provides assistance in activities of daily living and completes necessary tasks to promote patient health, including assisting patients with basic hygiene needs, maintaining an optimum level of mobility and providing assistance with nutritional intake as needed.

2. Collects patient specimens and data, including vital signs, input/output and other delegated measurements in accordance with unit/facility policy and standards of care. Documents objective data and routine aspects of patient care. Assists in the transportation of patient specimens.

3. Recognizes and reports abnormalities and/or changes in the patient’s health status and responses to treatment to a licensed professional. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for which care is provided.

4. Performs procedures and uses equipment as delegated by a licensed professional, consistent with demonstrated competencies and according to state scope of practice; procedures may include blood draws, foley catheters removal, EKGs, and EKG monitoring. Provides necessary supplies, support and assistance to medical staff and patients for unit specific procedures, under the direct supervision of a licensed professional.

5. Performs receptionist/clerical duties to support overall operating efficiency and effective flow of communication and information throughout the department. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution.

6. Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient’s record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. Assists in the ordering of patient specimens. May transcribe and process physician orders effectively using the electronic medical record.

7. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery. Monitors and tracks the use and whereabouts of electronic equipment such as patient ID scanners and lap top computers. Ensures that all are accounted for and in working order.

8. Based on area of assignment may assist with transporting patients, including pre-operative and immediate post-operative patients to critical care unit. Participates in OR room turnover.

9. This position works under supervision and has responsibility for assigned unit only. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Interacts with staff at all levels throughout the facility, as well as patients, families and physicians

• Gathers and formats data into regular and ad-hoc reports and dashboards.

  Qualifications:

• Typically between 3 - 5 years of relevant experience and post-secondary degree in related field of study or an equivalent combination of education and experience.
• Basic knowledge of portfolio management and related credit qualification standards, policies, and procedures.
• In-depth understanding of the mortgage lending process and applicable regulatory requirements.
• Strong interpersonal, sales, customer service, and negotiation skills.
• Expert knowledge of self-management and teamwork, personal lending, relationship sales and management.

Summary

Provides direct student care under the delegation of the school Nurse. Is responsible for providing first aid, medication administration and emergency response for school children through the School Health Advocacy Program. Works with the school nurse, school staff, other health care providers, community program and others to improve the health and well-being of the child. Responsible for assisting high-risk families access available community resources. Provides education to clients and the community related to focus of community program.

Health aides in this role will be working with moderately cognitively impaired children, assisting with children's daily medical and school needs: daily meds, tube feedings, potentially straight catheters, toileting, and feeding.

Qualifications

• High School Diploma or equivalent, required.
• 2 years of relevant experience Required.
• CRT-First Aid - AHA American Heart Association OR CRT-CPR - AHA American Heart Association, required.

Essential Functions

• Serves as the school health office primary contact for health-related needs.
• Provides direct student health care as delegated by the RN according to school district policies. Collects information, such as vital signs and history of illness or injury and communicates as needed with RN to determine student’s disposition. Documents direct and indirect services provided and other pertinent program data.
• Administers scheduled, as needed and emergency medications under the delegation of the RN according to Corewell Health and school district policies.
• Provides treatments such as tube feedings, diabetic management (administration of insulin) and catheterization under the delegation of the RN according to Corewell Health and school district policies.
• Participates in gathering pertinent information regarding student health concerns and coordinates these needs to school staff, prior to any school field trip.
• Communicates to the RN information received from students, family members, physicians, and other members of the educational team, following FERPA guidelines.
• Continuously collects and documents data regarding school immunization status, school communicable disease reports, school hearing, vision, and lice screenings as well as student health concerns. Acts as a role model and resource to students, their families, and the school community to promote a healthy and safe school environment.
• Partners with other health providers to provide services to students. Completes prep work, documentation, and administers results and follow up of partnering with providers.
• Works collaboratively with student and their families by referring community agencies and volunteers to schedule and provide optimal coordination of services, to facilitate a supportive relationship and to evaluate effectiveness of services provided and plan for future needs.
• Acts as a role model and resource to students, their families, and the school community to promote a healthy and safe school environment.
• Mentors, empowers, and advocates for students and their families to help them gain independence with their health care needs.
• Participates in social and physical evaluations that could affect the student at home or in school. Acts as a resource to program staff to discuss barriers about life/situational/cultural factors impacting client, family, and environment.
• Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.
• Performs other duties as assigned.
