We are looking for an experienced Dietitian to provide maternity cover on a part-time/flexible basis to our Paediatric Home Enteral Feeding team.

The Community Paediatric Home Enteral Feeding team provides a high-quality service to children and families in Luton and South Bedfordshire. Successful applicants will be well supported in their role by our supportive team of experienced Band 7 Paediatric Dietitians, Band 6 Paediatric Dietitians and Dietetic Support Workers!

This role can be fulfilled remotely, so we welcome applications from professionals across the country.

We would be willing to offer a short-term contract for 12 months if required.

To provide a highly specialist dietetic service to a caseload of complex paediatric patients – home enteral tube feeding (HETF), learning disabilities, autistic spectrum disorder (ASD).

To work collaboratively with acute dietetic and medical teams to effectively manage enterally fed patients who are stepping down from acute to community care.

To work closely with acute dietetic colleagues to facilitate safe and appropriate discharge planning from acute services to community. This will include regular attendance to the wards or meetings at Luton and Dunstable Hospital for patients on the caseload.

To work closely alongside the multidisciplinary teams who look after these patients, across both acute and community services.

To act as a source of expertise to the wider dietetic team and other health professionals on HETF and paediatric dietetics.

Main Duties and Responsibilities:

To manage a caseload of complex paediatric patients – includes home enteral tube feeding, learning disabilities, autistic spectrum disorder (ASD). Providing dietetic reviews in community clinic, day centres, home visit, special needs schools and jointly with consultants/other healthcare professionals.

To undertake comprehensive assessments for patients, including anthropometrics, biochemistry, clinical condition, diet history and social circumstances and to use advanced clinical reasoning skills to interpret data.

To provide appropriate evidence based intervention and evaluate outcomes.

To prioritise and plan the day-to-day provision of dietetic services within your own caseload.

To contribute to the development and implementation of protocols and guidelines for your specialist areas within the service as well as nutrition-related Trust-wide policies.

To work autonomously with little day to day supervision.

To make timely and appropriate direct referrals to other healthcare professionals and specialist services

To liaise with dietetic colleagues locally and nationally and other experts to ensure nutritional care, standards, advice and information are consistent with the current evidence base.

To work jointly with the other home enteral feeding dietitians and provide cover for the community paediatric dietitians or home enteral feeding team lead in their absence, as required.

To work collaboratively and liaise with a wide range of professions across a variety of locations and agencies.

To challenge professional and organisational boundaries in the interest of patients.

To organise and co-ordinate the process of setting up a home enterally fed patient on the home delivery service following discharge from hospital.

Contribute to department continuous professional development (CPD) sessions, home enteral tube feeding meetings and MDT caseload meetings.

To give expert opinion on complex nutrition related issues where a number of social, ethical, psychological and medical factors have to be considered and where there may be conflicting ideas within the multidisciplinary team

To be responsible for the provision and development of dietetic services in your specialist areas across the Trust, and to be responsible for service change in those specialist areas in response to service requirement.

To be professionally and legally accountable and responsible for all aspects of your own work, including the management of patients in your care.

To maintain accurate and comprehensive patient records in line with the British Dietetic Association (BDA), HCPC and trust guidelines.

We are looking to cover 8am until 6pm (shifts are 8am-4pm and 10am-6pm) Monday to Friday, shifts will be allocated as the demand of service dictates.



Our aim is to find the right support, the right resources and the right services for our young people.

This offers the right candidate an opportunity to hone skills in the triage of mental health referrals, explore and link with other statutory and non-statutory services, help to identify advice and guidance for young people and their parents and work with a dedicated dynamic and supportive team. We are looking for individuals with experience of working within Mental Health services and with children/ young people’s mental health.

The post holder will be a registered Practitioner and provide clinical expertise to allow the review, information gathering and triaging of mental health referrals for children, adolescents, and young people (0-25) within Norfolk and Waveney at the Single Point of Access – Just One Number (JON). Main Duties and Responsibilities

Ensure the Named Professional/safeguarding team specialist nurses are aware of serious cases involving CCS and Datix system is utilised to consider learning within the Trust services.

Participate in MASH management meetings to ensure continued health contribution in the development of the MASH function.

To highlight concerns regarding practice to the appropriate named professional within the provider organisation or directly with the ICB Designated Nurse in the case of primary care.

Ensure the health team has access and share health records for children as per MASH Information Sharing Agreement and act as a conduit for information sharing between the health economy and MASH in the case of children and as appropriate parents, or carers.

Contribute to the evaluation of MASH processes from a health perspective to ensure safe and appropriate information sharing within the MASH and across the wider health economy.

To contribute to regular audit of referrals to Children’s Social Care from the Health System.

To maintain high standards of record keeping and documentation on cases where information has been shared.

To ensure regular safeguarding supervision is accessed as per the CCS Safeguarding Supervision Policy

Communication

To establish good working relationships and work collaboratively with other agencies including social care and other health care providers.

Participate in case and strategy discussions and inform decision making regarding thresholds and risk.

Contribute to the safe and effective running of the health contribution to the MASH service in a timely manner, utilising resources efficiently.

To promote effective safe communication networks between health agencies and social care.

To maintain a clear focus of health’s role and offer professional challenge to support resolution of professional disagreements and disputes as needed.

Undertake Audit where required to assist the Lead Nurse in providing appropriate data to contribute to quality assurance of MASH activity.

Support the lead nurse to provide appropriate data as required by the MASH Operational group.

In return we will provide regular supervision, mentorship, and training under the preceptorship programme so that you enjoy your job and develop your professional skills within Children’s Services.

Our fully integrated OT service has been combining health and social care roles for many years. This community based role will involve working at your clinic base (Cambridge area), local special schools, family homes, mainstream and local pre-school settings. Typical caseloads have a mixture of children with physical/developmental needs with conditions such as cerebral palsy and autism. Although desirable, experience of working with children is not essential.

The post holder will maintain strong links with their Band 6 supervisor, a newly appointed Band 5 colleague, plus supportive community OT colleagues and at times work along-side the specialist housing “team-within-a-team” OT colleagues within our service, who support children and families with major housing adaptations.

The post holder will require excellent communication, organisation and time management skills to support provision of effective interventions.



The post holder will be given a thorough induction to the specialist area.

We also work closely with our physiotherapy colleagues as well as other members of the multi-disciplinary team such as speech and language therapists and paediatricians.

Nature of work

To provide children’s occupational therapy assessment and treatment for children. This will include the assessment and prescription of specialist equipment for home and school to support function and participation (e.g. positioning, beds, toilet and bathing, hoists and slings, safety) and link with the housing specialists within the service to ensure a fully integrated service. This will be with support from senior colleague(s).

To work in a variety of settings such as clinics, mainstream schools/nurseries, special schools/nurseries, client’s homes, and other community settings

To appropriately delegate work/tasks to therapy assistants, maintaining oversight of their input with children and young people.

To formulate and deliver an individual occupational therapy programme based on a sound knowledge of evidence based practice and treatment approaches using clinical assessment, reasoning skills and knowledge of treatment modalities available, e.g. CO-OP approach, COPM, neurodevelopmental therapy, sensory integration therapy (in line with strongest evidence) and other alternatives (with appropriate support from senior colleague)

To participate in training/lecturing programmes, alongside senior colleague(s), for teachers and learning support assistants in schools and parents/carers and other members of the multi-disciplinary team.

To participate, alongside senior colleague(s) in the planning, development and evaluation of the children and young people’s (CYP) Occupational Therapy service.

Where appropriate, to act as a key worker for CYP on their caseload, ensuring that there is effective communication between all members of the team and that a coordinated approach is in place.

The nature of children’s practice will require the post holder to bend, stretch, lift children’s limbs, crawl, kneel, work in confined spaces and at times work from the floor employing light physical effort on a day to day basis.

Communication

To provide excellent communication with Children and Young People, Parents, carers, schools, local education authority, social services, acute hospitals, paediatricians, physiotherapists, speech and language therapists, pre-school teachers, psychologists (clinical and educational) and child and family nurses.

Often information handled will be sensitive and highly complex. Occasionally children are seen with extremely sensitive rare and complex medical conditions this requires communication at a high level with specialist doctors and national centres of excellence.

To communicate concerns, incidents, and complaints to more senior colleagues and participate in resolution, where appropriate.

To provide clear, concise written reports for other agencies for statutory processes to meet legal requirements, e.g. Education, Health and Care Plan, Tribunal cases.

People Management

To be involved in the supervision of occupational therapy students in their clinical practice.

Undertake holistic health assessments of patients with complex palliative and end of life care needs in community settings.

To proactively plan, co-ordinate, deliver and evaluate the provision of a high quality specialist palliative and end of life care within the community and across the primary and secondary care interface ensuing onward referrals are made as appropriate.

1. To independently manage a caseload of pre-school and school age children who have speech, and language communication difficulties, working in East Cambridgeshire.
2. To undertake a range of assessments, both standardised and non-standardised, taking account of medical, social and educational factors, which combined with highly specialist theoretical knowledge will lead to differential diagnosis.
3. To plan, deliver and evaluate intervention programmes following professional guidelines and agreed service standards.

Nature of work

To provide children’s occupational therapy assessment and treatment for children. This will include the assessment and prescription of specialist equipment for home and school to support function and participation (e.g. positioning, beds, toilet and bathing, hoists and slings, safety) and link with the housing specialists within the service to ensure a fully integrated service. This will be with support from senior colleague(s).

To work in a variety of settings such as clinics, mainstream schools/nurseries, special schools/nurseries, client’s homes, and other community settings

To appropriately delegate work/tasks to therapy assistants, maintaining oversight of their input with children and young people.

To formulate and deliver an individual occupational therapy programme based on a sound knowledge of evidence based practice and treatment approaches using clinical assessment, reasoning skills and knowledge of treatment modalities available, e.g. CO-OP approach, COPM, neurodevelopmental therapy, sensory integration therapy (in line with strongest evidence) and other alternatives (with appropriate support from senior colleague)

To participate in training/lecturing programmes, alongside senior colleague(s), for teachers and learning support assistants in schools and parents/carers and other members of the multi-disciplinary team.

To participate, alongside senior colleague(s) in the planning, development and evaluation of the children and young people’s (CYP) Occupational Therapy service.

Where appropriate, to act as a key worker for CYP on their caseload, ensuring that there is effective communication between all members of the team and that a coordinated approach is in place.

The nature of children’s practice will require the post holder to bend, stretch, lift children’s limbs, crawl, kneel, work in confined spaces and at times work from the floor employing light physical effort on a day to day basis.

Often information handled will be sensitive and highly complex. Occasionally children are seen with extremely sensitive rare and complex medical conditions this requires communication at a high level with specialist doctors and national centres of excellence.

To communicate concerns, incidents, and complaints to more senior colleagues and participate in resolution, where appropriate.

To provide clear, concise written reports for other agencies for statutory processes to meet legal requirements, e.g. Education, Health and Care Plan, Tribunal cases.

An exciting opportunity has arisen for a highly motivated band 5 physiotherapist to join our Musculoskeletal outpatient team at Brookfields Hospital, Cambridge, within the Cambridge and Ely locality.

We are looking for a clinician with an innovative approach and excellent interpersonal and communication skills. Recent post graduate experience including musculoskeletal outpatients and working within a health service sector, either acute or community is preferred.

The post holder will be required to perform a physiotherapeutic assessment of adults with diverse MSK conditions, to provide a diagnosis and to develop and deliver an individual treatment program.

Supervision takes the form or regular formal training, case review sessions, peer review and observed sessions.

Clinical
• To be professionally and legally accountable and responsible for all aspects of your own work, including the management of patients in your care.
• To ensure a high standard of clinical care for the patients under your management.
• To undertake a comprehensive assessment of patients making use of clinical reasoning skills and manual assessment techniques
• To be responsible for the selection, implementation and evaluation of physiotherapy programmes, either individually, or in a group setting, based on a sound knowledge of evidence based practice and treatment skills.
• To work within trust guidelines and CSP guidelines, and to have a good working knowledge of national and local standards monitoring the quality of own and others practice as appropriate.
• To be responsible for maintaining accurate and comprehensive patient treatment records in line with CSP standards of practice.
• To accept clinical responsibility for a designated caseload of patients and to organise this effectively with regard to clinical priorities and use of time.
• To undertake clinical duties in a number of ways which may include face to face consultations, Physio Advice Line telephone consultations and group settings.
• To work along side band 4 Rehabilitation Instructors to deliver appropriate treatment and rehabilitation progression.

Professional
• To maintain own clinical professional development by keeping abreast of new developments and incorporate them as necessary into your own work.
• To demonstrate a sound understanding of Clinical Governance and Risk Management and apply to the work situation.
• To be an active member of the in-service training programme by the attendance and presentation at staff meetings, tutorials, training sessions, external courses and reflective practice.
• To participate in department/trust audits and evaluation of best practice as required.
• To participate in the staff appraisal scheme and Personal development plan as anappraisee.
• To undertake the measurement and evaluation of your work and current practices through the use and application of evidence-based practice, clinical audit, research and outcome measures.
• To maintain own professional development and requirement to take part in appraisal and KSF process

Organisational
• To plan, manage and prioritise own caseload and be responsible for assisting other team members their workload.
• To decide priorities for own work area, balancing patient-related and professional demands.

• To communicate assessment and treatment results to the appropriate disciplines in the form of reports and letters.

Patient and Public Engagement

To contribute to ensuring thatCambridgeshire Community Services NHS Trustresponsibilities to engage and consult with patients and the public are met in line with statutory requirements

Communication
• To communicate clinical information in an understandable way to patients and to assess patient understanding of treatment proposals, gain co-operation and valid informed consent for treatment. Effective communication requires use of verbal and non-verbal tools to maximise rehabilitation potential, often with unmotivated patients. This can include patients where there are barriers to effective communication e.g. loss of hearing, altered perception, pain, fear etc.
• To communicate effectively with consultants, doctors, nurse practitioners, physiotherapists from other trusts and other Allied Health Professionals (AHPs) who have referred patients or are involved with the patient’s care. This can be direct communication or in the form of discharge / progress reports.
• Contribute to CCS’ internal communication strategies ensuring effective two-way communication and engagement within the organisation.
• Contribute to the external communication strategies forCambridgeshire Community Services NHS Trust, working with the Head of Communications to raise awareness and promote the reputation ofCambridgeshire Community Services NHS Trust.

People Management
• To act as Clinical Supervisor for undergraduate physiotherapists and/or non clinical staff (Rehabilitation Instructor). This involves teaching, organisation, supervision, role modelling, learning contracts, evaluation and assessment of their abilities by grading according to the learning institute’s specifications.
• To partake in clinical supervision according to Trust guidelines.
• To maintainown professional development and requirement to take part in appraisal and KSF process

Clinical and Practice Governance
• Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.
• Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to Cambridgeshire

• It is a condition of your employment that you are currently registered with Health and Care Professionals Council, and it is your responsibility to maintain your professional registration.
