The Acute Physical Deterioration Implementation Network is an essential national vehicle for joint working between key stakeholders including NHS Wales, Welsh Government and the third sector to improve the outcomes and experience for those affected by acute physical deterioration in Wales. Building on the consensus recommendations for the management of acute physical deterioration across Wales paper, the next phase of service improvement for patients and their families must be to drive forward best practice in the management of acute physical deterioration in all settings. Providing effective systems for detection, monitoring and escalation of care that can minimise risks for the population of Wales, based upon current evidence and lived and learnt experience. This is an exciting and significant national leadership role within the NHS. As managerial lead of the Acute Physical Deterioration Implementation Network and working closely with the National Clinical Lead for Acute Physical Deterioration the post holder will be responsible for the design, planning, implementation and management of the existing and future work programmes of the Network together with the infrastructure available to deliver the programme. The post holder will shape the work programme to successfully deliver transformational change across the landscape, providing leadership and direction across the interfaces, joining up the constituent work programmes and other agencies to ensure alignment and a shared vision. The post holder will carry out the following duties for the PCN: - oversee the day to day running of the Enhanced Health in Care Home Team, PCN Cancer Services and the Digital Clinical Hub, together with ad hoc support to the PCN Pharmacy Team Lead to ensure services delivered are of high quality, safe and caring and value for money. - collation and preparation of performance reports, KPI data (using EMIS), outcomes, briefing papers for each of the teams in preparation for all relevant review meetings with Network members and / or external stakeholders, ensuring professional standards are maintained. - standard operating procedure compliance, preparing and distribution policies for the above teams. - meet regularly with the PCN Manager and PCN Project Manager to keep them updated on service delivery, identifying any areas of risk, mitigating factors and any areas for quality improvement. - attend regular service operational meetings with Southport and Formby Health Ltd (SFHL) to share updates, learning and keep PCN teams up to date with wider developments across the PCN and Southport and Formby Health. - attend regular Governance committee meetings with SFHL to ensure PCN services are compliant with governance requirements for the specific PCN teams. - assisting with line management of staff such as agreeing weekly work plan for the PCN staff where applicable. - support the HR Manager with inductions and onboarding of new starters and identify training and development needs across the teams and escalate to the PCN leadership team for consideration. - co-ordinate record keeping of PCN initiatives including mechanisms to record and monitor progress, identify risk areas and action planning. - support HR Manager and Team leads to manage rota systems for staff within PCN to ensure staffing levels are sufficient to meet the service needs on a daily basis. The Head of Emergency Planning, Resilience and Response (EPRR) and System Resilience will be responsible for leading the organisations work in respect of Emergency Planning Resilience and Response (EPRR) and System Resilience working with the Deputy Director of EPRR, the Deputy Director of System Resilience and the wider EPRR and System Resilience Teams. The Safeguard/Ulysses System Manager will manage the systems configurations and developments, including the upkeep of defined data fields within the Trust application in line with learning from Patient SAfety Events (LFPSE) specifications.
