Evaluate the Veteran to determine whether acute psychiatric hospitalization is indicated by assessing strengths, limitations, abilities, preferences, risk factors, internal/external supports and service needs. The incumbent demonstrates an understanding of diagnosis and the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions. The treatment plan will be used to direct a course of treatment. Provide appropriate resources, based on assessment and expressed needs, to Veterans being discharged home or transferred to another facility.

Collaborate with other service providers in reassessing the Veteran's needs. The social worker is responsible for educating the Veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the Veteran's preference or that of his surrogate decision-maker.

The social worker is responsible for maintaining and using a current social work resource file of VA and community services and programs and enhancing the content to the benefit of veterans. The social worker will refer the Veteran to needed services.

Social worker understands the intricacies of bureaucracy and will act as an advocate when it serves the best interest of the Veteran/family. When appropriate and feasible, the social worker will educate and encourage the Veteran/family to advocate on own behalf, thus fostering a sense of independence and empowerment. • Assists in filling gaps in the dentist's schedule due cancellations and failed appointments.
• Assists dentist in charting findings and treatment plans in patient records.
• Assist with entering Return to Clinic Orders.

• Maintains patient confidentiality per HIPAA with written and verbal information.
• Follows computer security regulations.

• Modifies care as necessitated by patient's age and abilities (patient transfer and position).
• Provides accurate and understandable instructions to patients.
• Participates in patient identification and time out per JCAHO requirements.
• Monitors patient condition during treatment, being alert for signs of physical distress.
• Monitors patient (BP, pulse) , and condition during invasive procedures and during recovery period following procedure.

• Complies with Employee Education requirements.
• Maintains BLS certification that includes operation of an Automatic External Defibrillator (AED) per Medical Center Policy.

• Takes preliminary impressions for study models.
• Pours impressions and prepares study models.
• Makes final denture impression trays and prepares bite rims.
• Performs other dental laboratory functions as time and skill levels allow.
