This autonomous role within skin cancer services provides support/key worker status to skin cancer patients and their families/carers from diagnosis and throughout their pathway. About the Team

The Surrey SLT service is a large team qualified therapists and non-qualified therapy assistants working in mainstream and specialist provisions through primary, secondary and colleges, with some opportunities for a preschool caseload (deafness and complex specialist schools).

We aim to deliver the right support at the right time, through assessment and intervention; equipping families and professionals with the skills and resources to work together to support children with SLCN. We offer universal, targeted and specialist support for children, families, and the school workforce; working with children and young people with EHCPs and those at SEN support following Surrey's Graduated Response. We also have a specialist dysphagia service and specialist deaf service.

The Specialist Service for deaf children

A team of Specialist SLTs provide an innovative Surrey wide service for school-aged deaf children and young people in mainstream schools and 5 Specialist Resource Bases. The team is also commissioned to support preschool deaf children who meet criteria. We have locally defined specialist competencies in relation to deafness with in-house clinical supervision, team meetings and shadowing to share knowledge and expertise of working in this field. Team members are encouraged to be part of service wide initiatives and to contribute to ongoing research and publications.

About the Role

Your main duties in the role of Specialist Speech and Language Therapist will include:

• Clinical decision making in conjunction with school/college staff
• Working with assistants, setting targets for the children and young people they are supporting, and monitoring of the delivery and outcomes of their work
• Assessment, advice and intervention for children with a range of needs
• Supporting junior members of the team, line managing others and taking on students

The post-holder will be responsible for delivering a high standard of outpatient care to patients living with human immunodeficiency deficiency virus. This will include delivering nurse-led clinics for Sexual Health and HIV investigations, vaccines and results management.



To take charge of the clinic shift when required according to rota, as well as supporting health care support workers and medical staff.



The post holder will facilitate the learning experience and environment for nurses (both pre and post registration) and other health professionals undergoing training and education in the department. This will include supervising and mentoring students undertaking HIV/ sexual health courses.

CLINICAL PRACTICE
▪ To maintain a high standard of nursing care and service provision, testing, treatment
and management of sexually transmitted infections (STIs), and managing care for
people living with HIV and other blood borne infections (BBVs) attending the clinic.
▪ To obtain clear and thorough relevant histories from patients including medications,
smoking, alcohol and sexual history.
▪ To examine patients and obtain appropriate clinical specimens according to clinic
protocols.
▪ To manage a range of symptomatic and asymptomatic sexual health presentations
according to local policy
▪ To initiate Cryotherapy according to Cryotherapy protocol
▪ To recognise signs and symptoms of complex and unstable health problems requiring
medical review and refer on to appropriate health professionals
▪ To treat patients within the confines of patient group directions (PGDs) and arrange
prescriptions of treatments not covered by PGD and safely administer and supply
these.
▪ To perform microscopy according to protocol (relevance dependent on site of work)
▪ To carry out partner notification processes.
▪ To promote sexual health and wellbeing through advice and information giving.
▪ To discuss HIV transmission, including U=U
▪ To offer advice on HIV medication and side effects.
▪ To discuss and provide appropriate vaccination including sign posting to primary care.
▪ To discuss HIV results and refer for senior advice when required.
▪ To take part in initiatives to engage users in the development of the services depending on site of work
▪ To liaise with team to engage patients in care and ensuring the DNA lost to follow-up pathways are adhered to.
▪ To maintain clinical case notes including electronic patients records to a high standard.
▪ To observe and adhere to clinic protocols and working guidelines.
▪ To liaise with nursing colleagues, other members of the multidisciplinary team and external agencies.
▪ To assist the HCSW (health care support worker) to set up clinics, maintain equipment and order supplies if required.
▪ To take part in initiatives to engage users in the development of the services depending on site of work.

Participate in the ward based Medicines Management (MM) service to medical and surgical wards within the trust to ensure all patients receive the best care regarding the management of their medication. Participate in the development of MM services within the trust in align with good clinical practice. Order and supply medication to the individual patients as appropriate to ensure the patients receives all required medication in a timely manner. To provide a technical role alongside the clinical pharmacist and other healthcare professionals in a ward based environment to support patients in the management of their medication to include: Medicines Reconciliation. Assessing patients own drugs. Transcribing medication for the individual patients. Checking medication on discharge. Counselling patients on their medication.
