CLINICAL RESPONSIBILITIES Provision of a high-quality clinical pharmacy service as part of the multidisciplinary team to all patients within the renal unit, to ensure maximum efficacy, safety and economic drug use and to demonstrate professional accountability to enhance patient care. Delivery of a high-quality, comprehensive clinical pharmacy service to the five satellite haemodialysis units and two kidney care centres. Management of the self-medication programme for renal transplant patients on the acute renal wards. Act as a supplementary or independent prescriber within agreed protocols, and prescribe all required immunosuppression medication for transplant patients for delivery by the homecare service. Demonstrate a high level of expert knowledge in the field of nephrology, dialysis and transplantation. Communication of complex pharmaceutical information, according to the level of understanding, direct to patients/carers and healthcare professionals. Make evidence-based decisions, in partnership with patients and other professionals within the specialist area. Development and review of Patient Group Directions Development and review of Shared Care guidelines. Develop and implementation of relevant local and national guidelines within the renal speciality. Identify, initiate and evaluate Drug Usage Reviews and audits and implement changes in prescribing practice accordingly. Develop relevant patient information leaflets which complement licensed product information Provide support on concordance related issues to patients Provide clinical screening and dispensing support to the renal technicians for the accurate and timely dispensing of renal prescriptions. Provide expert medicines information to clinicians and healthcare professionals and ensure that this information is utilised in a systematic and appropriate way to optimise patient care. To liaise and work with other specialist pharmacists to share the best practice, to strengthen patient focused pharmaceutical care. 2. Diamond Ward (Highgate West) is a 17-bed male acute treatment ward. It specialises in the treatment of patients with severe mental illnesses such as schizophrenia, bipolar affective disorder, depression and personality disorder, with associated challenging behaviours/risks. The focus is in detailed clinical and risk assessment with a view to starting early effective treatment. The clinical lead for the team is Dr Kavinda Gunathillaka, Consultant Psychiatrist. The post would involves working closely with other MDT members such as nursing staff, psychiatrists, OTs and psychologists.

Tasks would include participating in daily MDT meetings and ward rounds (clinical reviews), clerking in new patients, reviewing patients’ mental and physical health, ordering relevant investigations, performing physical examinations, blood tests and ECGs and participating in the out of hours core trainee rota. The foundation doctor would be working under the close supervision of Dr Gunathillaka, his specialty doctor and core trainees.

The post may be based full time in Diamond Ward or may be split between Diamond and another treatment ward in Highgate Mental Health Centre. If the post is split it will involve 3 days per week in Diamond and 2 days per week in another ward. The post has arisen because of a gap due to the post not being filled by a Deanery foundation doctor.

South London Regional Research Delivery Network (RRDN) is looking for an enthusiastic Research Nurse to join the Agile Workforce team.

The Research Nurse will play a key role in ensuring that any research undertaken within the Care settings department safeguards the wellbeing of the patients and is conducted according to Good Clinical Practice (GCP) and the Medicines for Human Use (Clinical Trial) Regulations 2004 and Amended Regulations 2006.

Working collaboratively with the clinical trials team and the wider multidisciplinary team in the management of research patients. They will also act as a patient advocate providing support and assistance as required and assist in the coordination and management of a portfolio of clinical research studies.

The post holder will be required to work flexibly across sites within the RDN: South London geography. The employing Trust will be Guy’s and St Thomas’ NHS Foundation Trust.

If applying as a registered nurse you must have appropriate UK professional registration.

The post holder will be responsible for delivering an agreed number of portfolios of clinical trials within the Secondary, Primary Care and Community settings across the South London region. They will work flexibly across the disciplines to ensure the success of the clinical research projects they are assigned to.
