The Manchester community response services offer a seven day service, therefore this entails working some weekends, bank holidays and shift work on a rota basis. The post involves working across community services on a rotational basis, delivering holistic assessments & interventions to patients in their own homes/ place of residence. Independent travel within the local geographical area to deliver the role is essential.
The rotationswillinclude:
· Discharge to Assess- delivered over 7 days 08:00-20:00
· Home pathway- delivered over 7 days 08:00- 20:00
· Hospital at home- delivered over 7 days 08:00- 20:00
· CrumpsallValeIntermediateCare-deliveredover7days shift patterns including earlies (07:30-15:30) and lates (11:00-19:00)
With the above in mind, you must have experience of working in the NHS and in community services. The successful candidate will be able to demonstrate an appropriate level of clinical skills in occupational therapy, will be experienced in administering person-centred assessments, as well as implementing and evaluating occupational therapy interventions in a fast paces working environment. You must have a recognised Occupational Therapy qualification and current HCPC registration.
The individual will manage a clinical caseload of clients, including those withcomplex and highly complex needs with support from the band 7 therapists forhighlycomplexpatientsandsupervisetheband4/3supportstaffwithcaseloadmanagement.
The post holder will work without direct supervision as a lone worker in thecommunityassessing,planning,implementing,evaluatingandmodifyingindividualised treatment programmes in a variety of care settings e.g. patientshome,clinicsetting,residentialhomesandintermediatecaresettingforpatientswithlowtohighly complexneeds.
The post holder will work as a team member collaborating with colleagues toensure patient needs and goals are met using approaches which promotechoice,participationand quality oflife.
The post holder will ensure patients are triaged and managed on appropriateand timely pathways of care by liaising closely and working in partnership withotherhealthandsocialcareprofessionalsincludingGPs,Consultants,Specialistequipmentagencies,SocialServicepartnersandVoluntaryagenciesandsupportthetimelydischargeofpatientsfromtheacutesetting.
The post holder will provide complex professional and clinical advice to clients,family members, carers and health and social care professionals includingGPs,therapycolleagues and caremanagers.
