GENERAL OVERVIEW:

This job provides direct care to patients and assists with non-direct patient care unit coordination. The incumbent in this position is expected to perform basic nursing procedures related to care and comfort of patients in stable condition, and have knowledge of patient population needs in the assigned work area. In addition, the incumbent is expected to assist the RN in the care of acutely ill patients. They is expected to practice medical asepsis, respect the dignity and privacy with every patient and family, and to maintain objectivity during patient care. They are expected to know and adhere to all basic hospital policies and procedures, and respond appropriately during emergency situations. They is expected to maintain the patient/family environment and perform general unit upkeep. They is expected to use accurate verbal and written communication, and to document patient care appropriately in the electronic medical record. The incumbent works under the general supervision of the Registered Nurse for patients in stable condition. Will work under the direct supervision of the Registered Nurse in the care of the acutely ill and/or complex patient. The incumbent receives specific work assignments from the Charge Nurse and/or Care Team Leader which are both written & verbal and is expected to demonstrate evidence of efficient time management by completing the work within the scheduled time. In addition to clinical responsibilities, the incumbent is expected to perform a variety of duties related to the coordination of activities on the inpatient unit. This includes activities related to patient admissions, discharges, and transfers.

ESSENTIAL RESPONSIBILITIES:

• Provides, obtains or performs and documents direct patient care and activities such as: activities of daily living, skin survey, vital signs, ambulation, specimen collection, intake and output, meal consumption, height and weight, blood glucose testing/monitoring, point of care testing, bladder scanning, transporting of patients and their medical record, documentation of O2, early mobilization and ambulation protocols, assistance with exam, treatments and procedures. (30%)
• Participates in quality and patient satisfaction initiatives which may include bedside shift report, purposeful hourly rounding promoting safety such as falls prevention, hand washing, isolation protocols and maintaining a clean and orderly environment. (30%)
• Accurately documents vital signs, weights, I&O, restraints, specimen collection, and all other treatments. Report changes in patient's physical, mental and emotional conditions, as well as any issues or concerns identified to nursing staff. (20%)
• Participates in unit activity such as supply stocking and equipment/appliance cleaning, checks and retrieval. (20%)
• Performs other duties as assigned or required.

QUALIFICATIONS:

Minimum

• High School Diploma or GED
• CPR Certification
• Limited or no experience: Must be able to attend 2 weeks of training
• Ability to read, write and follow oral and written instructions
• Ability to perform basic mathematics
• Basic computer skills

Preferred

• Acute care experience with in the last 3 years
• Currently enrolled in ASN or BSN program. Preference given to those enrolled in Associates or Bachelor’s degree Nursing programs and completed at least one clinical rotation

As a Transitional Care Associate, you will facilitate the safe and timely transition from acute care to alternative levels of care such as skilled nursing facility, long-term acute care, inpatient rehabilitation, home infusion therapy, hospice and/or home care program. You will also facilitate discharge plan for the transition of care and services into the designated setting or service. You should be willing to work collaboratively with care coordination team members to identify any barriers to a safe discharge and works with patient/family to provide clear communication regarding discharge plan.  

Schedule: 4 Ten-hour shifts, no on-call, 7am-5:30pm, with rotating weekend. This position offers a weekend schedule which is eligible for a flat rate $3/hour weekend shift differential. 

POSITION SUMMARY

CORE FUNCTIONS
1. Processes and facilitates the timely discharge/transfer of clients from hospital care to identified post-acute setting. Notifies care coordination team member(s) if patient or caregiver demonstrate or verbalize any inability/concern to be able to manage their post-acute plan or responsibilities.

2. Facilitates/ implements the care plan with proposed interventions in collaboration with healthcare team. Collaborates with all members of the healthcare team to implement, manage and communicate the transition of care arrangements.

3. Participates in performance improvement projects, Banner initiatives and performs data collection for measurement of projects as assigned.

4. Documents all interventions in the patient medical record both timely and accurately including all elements of the discharge plan. Performs transfer of accurate, pertinent patient information between all appropriate entities of the post-acute care continuum.

5. Assist and support patients and families in making appropriate arrangements for the post-acute plan. Performs follow-up calls to patients and providers as indicated and report any concerns to leadership.

6. Serves as an intermediary when providing community resources to patients, caregiver, and families. Discusses with patient, caregiver, and/or family maintaining clear communication regarding anticipated discharge date and potential care settings.

7. Maintains knowledge of Medicare, Medicaid and other program benefits to assist patients with transition of care planning and choices.

8. Employee has freedom to determine how to best accomplish functions within established procedures and implements the discharge plan under the delegated authority of a provider, licensed MSW, registered nurse or other licensed healthcare professional. Confers with supervisor/manager on any unusual situations and communicates plans and activities for patient discharge across the care continuum. Internal customers: Post-acute services team members and all levels of nursing management and staff, medical staff, and all other members of assigned facility interdisciplinary health care team. External customers: home health agencies, nursing homes, insurance providers, group homes, assisted living facilities, hospice, long-term acute care hospitals, inpatient rehabilitation facilities, volunteer agencies, county/governmental agencies and medical supply companies and others as required.

MINIMUM QUALIFICATIONS

A Bachelor’s degree in social work or related degree or a Licensed Practice Nurse, or a Licensed Respiratory Therapist required.

Must have knowledge of government/community agencies and resources, such as Medicare/Medicaid, long term care or other applicable resources/services. Must demonstrate effective communication and customer service skills, human relation skills and time management skills. Must be able to work flexible hours and work weekends on rotation. BLS required. (BLS is not required for employees working in the Insurance Division.)

Employees working at BUMG, BUMCT, or BUMCS in a Behavioral Health clinical setting that serves children must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment. PREFERRED QUALIFICATIONS

Our Care Coordination staff is seeking a Registered Nurse RN Case Manager to work closely with patients, their families, physicians, and other healthcare team members to ensure needed services are received at the correct level of care and in a timely fashion. As a member of our team, you will facilitate a seamless transfer from one level of care to the next. You will provide coordination and arrangement of post-discharge services, including home healthcare, extended care-services, medical equipment, and transportation. 

This is a highly collaborative team and our team members occasionally float from both of our Tucson campuses. Expected shift is 5/8s. Hours are 8AM to 4:30PM. Holidays per department requirement. You will also have the opportunity utilize our self-scheduling tool for shift and weekend coverage. Enjoy a flat rate $3/hour weekend shift differential. 

POSITION SUMMARY
This position provides comprehensive care coordination for patients as assigned. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements. This position is accountable for the clinical quality of Care Coordination services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care. The goal is to empower the patient and the family to participate to the fullest of their abilities in the discharge planning process. This position provides developmentally appropriate care for the population that it serves which includes planning for the safe discharge, continuity of care, the ability to recognize and plan for the unique needs of all ages as well as the physically disabled, mentally ill, chronically ill and terminally ill patient.

CORE FUNCTIONS
1. Manages individual patients across the health care continuum to achieve the optimal clinical care, financial, operational, and satisfaction outcomes.

Positions are entity based with no budgetary responsibility. MINIMUM QUALIFICATIONS

Must possess knowledge of case management or utilization review as normally obtained through the completion of a bachelor's degree in case management or health care.

Requires current Registered Nurse (R.N.) license in state worked. For assignments in an acute care setting, Basic Life Support (BLS) certification is also required.
 

Requires a proficiency level typically achieved with 2-3 years clinical experience. Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, and time management skills. Must demonstrate ability to work effectively in an interdisciplinary team format. May have to take rotating call based on the acute facility need. Banner Registry and Travel positions require a minimum of one year Case Manager experience in an acute care hospital.

PREFERRED QUALIFICATIONS

Certification for CCM (Certified Case Manager) preferred.

POSITION SUMMARY: 

The Store Consultant consistently delivers a positive customer experience to all customers, utilizing consultative skills to anticipate customer needs, suggest alternatives and provide solutions. GENERAL OVERVIEW:

This job provides direct care to patients and assists with non-direct patient care unit coordination. The Progressive Care Unit (PCU) is a step down from Critical Care/ICU and serves a high acuity patient.   This unit is a great gateway for RNs who seek challenge, love to learn, and would like to build a solid foundation of skills across many areas.  Especially suited for RNs with the long term goal of working within specialty areas.   

The Banner Baywood Progressive Care Unit is an 80-bed inpatient high acuity, cardiac monitored unit focusing on providing exceptional patient-centered care for adults and geriatric patients. The PCU provides care for diverse patients with acute and chronic disease processes and multiple co-morbidities. Our leadership team prides itself on a collaborative teamwork approach, while ensuring a strong focus on continued growth and mentoring for our RNs.  Nurses on our unit are EKG and ACLS certified, and receive education beyond basic level learning.  Our vision is to provide our team with an environment where our RNs can thrive and create an exceptional patient experience!

As a Registered Nurse RN at Banner you will have the opportunity utilize our self-scheduling tool for shift and weekend coverage. We also offer 18% night shift differential and an additional flat rate $3/hour weekend shift differential for eligible shifts!

POSITION SUMMARY
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.

CORE FUNCTIONS
1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.

2. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.

3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.

4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.

5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient’s response to treatment. Notes physician orders accurately and in a timely manner.

6. Provides care based on the best evidence available and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.

7. Contributes to society through activities that lead to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participates in the improvement of national nursing quality indicators and outcomes. Such activities may include participating in professional organizations.

8. In some roles, this position may supervise staff and work flow of the department.

9. Interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Interacts with other health care providers in numerous settings in order to report and ask for or clarify information. Also works with clergy to provide spiritual support. Synthesizes and prioritizes data from multiple sources to provide support for the human response of the patient and family to changes in health status.

MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.

Relates throughout the interview process the experience, training and education needed to perform the job. Experience in the clinical area for which he or she is applying is desired. Must maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area. Banner Registry and Travel physician practice positions require a minimum of one year experience as a Registered Nurse in a physician practice or an acute care setting.

PREFERRED QUALIFICATIONS
Bachelor’s degree preferred. Professional certification preferred.
