This role, based in North West Doncaster is part of the adoption of Additional Roles Re-imbursement Scheme (ARRS) mental health roles in primary care it is an exciting opportunity to build new models of working, with community-based roles situated at the heart of general practice while been employed by RDaSH. A GP integrated mental health service will create better joint working across primary and secondary care mental health systems.
The North West PCN was established on 1 April 2024 and is looking to recruit an experienced Mental Health Practitioner to join its new and expanding Team. This is an exciting time for the PCN as they embark on their journey as a new PCN.
The role will work solely within a practice setting working closely with the PCN and practice teams.
This is an excellent opportunity for someone who enjoys working in a dynamic, fast paced environment and values face to face collaboration.
Provide a lead role in supporting Primary Care Networks in triaging mental health concerns and providing specialist advice.
Support GPs within the network to make appropriate referrals and support bridging a gap between primary care and secondary services while working in partnership across the system.
To act as first point of contact for GPs to gain advice and support regarding people’s mental health.
Where appropriate liaise with the GP team and the PCN regarding complex situations
To be available to participate in primary care practice level meetings, including any meetings involving serious concern, i.e., safeguarding and risk management.
Be effective in multi-professional team working and service transformation.
To provide a comprehensive Primary Mental Health Nursing Service across the whole range of patients, service will be provided mainly via clinics and one-to-one interactions with service users which will include the provision of bio-psycho-social assessment and treatment delivery, including the role of Independent Non-medical Prescribing.
In order to work at this level, the nurse must be able to practice autonomously and preferably hold the Independent Non-medical Prescribing accreditation or be prepared to work towards it immediately when commenced in post.
