The LPN may contribute to the plan of care by collaborating with interdisciplinary team members, the patient and the patient's family. The LPN collects data and information, recognizes changes in conditions and reports this to the RN supervisor or another appropriate clinical supervisor to assist in the identification of problems and formulation of goals, outcomes, and patient-centered plans of care that are developed in collaboration with patients, their families, and the interdisciplinary health care team. The LPN participates in the nursing process by appraising the individual patients status or situation at hand. The LPN can participate in implementing established teaching plans for patients and their families with common health problems and well-defined health learning needs and suggest any modifications to the plan of care that may be necessary. The PACT LPN Clinical Associate facilitates continuity of care by making the appropriate referrals.

Demonstrates competence in delivering and improving holistic care through collaborative strategies with others. The LPN is proficient in initiating, performing, and completing assigned duties in providing care to variable Veteran populations under the general review of a Registered Nurse (RN). Collaborates with PACT staff to develop comprehensive health care plans and care management plans for patients assigned to patient panels. Employs continuous improvement strategies and active learning to improve the team's function, increase efficiency, encourage standardization, improve health outcomes and optimize the quality of care they deliver. These positions are located at the Orlando VA Medical center, 13800 Veterans Way, Orlando, FL 32827.The Acute Care Nursing Assistant (NA) responsibilities include all activities of daily living and general patient care duties to help the healthcare team restore the Veteran to the highest level of independence or remain comfortable toward the end of life. The NA will provide support for diagnostic procedures, nursing treatments, patient charting, and reinforcement of patient education. The performance of these tasks involves direct working relationships with Veterans and participation as a member of the nursing care team in direct service to Veterans.

Nursing Assistant (NA) Acute care is a member of the nursing care team and assist licensed nursing staff in the care of patients/residents receiving outpatient care, home, or community living care, or other patients/residents who are in acute, sub-acute or chronic states of illness. The distinguishing factor is that patient/resident assignments typically involve more complex nursing needs which can vary within a range of predictable to unpredictable requirements. NAs at this level also assume more of a peer education/mentorship role by assisting with the coordination of new NA orientation and overseeing/assessing their practical experience while in a clinical setting. They will have the ability to recognize and react to emergent patient/resident care situations and intervene while waiting for assistance. Completed work may require routine or detailed higher-level review depending on the complexity of the duties.

The duties for the Acute Care Nursing Assistant include but not limited to:

• Plans, organizes, and prioritizes assigned workload.
• Measures and records intake and output.
• Demonstrates ability to properly use equipment, materials, and supplies in simple diagnostic and treatment procedures such as glucometer test for blood glucose level, EKGs, bladder scanning, collection of urine samples, sputum, and stool. Delivers samples to lab and picks up blood products from the blood bank.
• Records basic data and patient responses, reporting significant changes to the RN.
• Assists by positioning and draping as well as emotionally supporting patients/residents during a variety of diagnostic and treatment procedures.
• Reports the condition of wounds and skin and any reaction to or results of treatment.
• Able to observe Veterans' physical or emotional status and refer deviations and/or problems to supervisor in a timely manner.
• Performs a wide variety of duties aimed at increasing the comfort and spirit of the Veteran.
• Assists in orienting patients/residents to the outpatient and/or inpatient hospital environment.

• Follows established SOPs and checklists in the use, set up, pre-cleaning, storage, reprocessing, and maintenance of all Reusable Medical Devices, as applicable.

This position independently performs duties that require proficiency in special procedures and broad knowledge of service operations, such as food and beverage preparation, modified diets, computer proficiency, dishwashing, dry and refrigerated storage, and safety and sanitation.

Major duties and responsibilities include but are not limited to:

• Sets up and breaks down assigned station with the correct supplies and food items;
• Portions food items into standard serving sizes using the proper utensils and specified dishware;
• Decides what food items to serve for the most common diets when the diet card or tray ticket identifies only the kind of diet called for;
• Provides patients with basic information about modified diets and the nutrition services that are available;
• Identifies obvious discrepancies between the prescribed diets and the food items designated by the menu;
• Follows directions in assembling, measuring, weighing, or mixing ingredients for basic formulas and supplemental feedings following standardized recipes and using sanitary techniques;
• Delivers meal trays to the patients' bedsides; reporting the patients' comments and complaints to the supervisor or dietitian;
• Provides assistance to cooks in the food preparation area including assembling, labeling and arranging completed recipe items in preparation for use by cooks;
• Prepares fresh fruit/cold salads/dressings/sandwich fillings and cold sandwiches and simple cold desserts along with individual and bulk nourishments and supplement foods and beverages for patients;
• Makes final check of diet trays assembled by lower grade workers for completeness, correct food temperatures;
• Works in one or more functional areas of the kitchen such as food preparation, dish and pot washing, dry and refrigerated storage and receiving, and the serving line;
• Performs heavy-duty cleaning tasks throughout the food service and related areas;

The Advanced Medical Support Assistant must possess a thorough working knowledge of clinic processes and services to function in and interact with various clinics throughout the hospital. Advanced Medical Assistant duties consist of but are not limited to the following:
