Community Nursing Sister/Charge Nurse
Location: Wokingham.
Working Hours: Monday to Friday, 8.30am to 4.30pm (part-time may be considered). With approximately one weekend every 4 weeks, bank holidays in rotation and one late shift covering 4:30pm to 7.00pm shift per week on a rota basis.
Salary: £39,205 to £47,084

We seek an enthusiastic and experienced nurse with excellent interpersonal skills and a compassionate nature. You should be open to relevant training opportunities and have proven leadership experience, with a strong ability to implement innovative practices.
With strong management skills, you will oversee the caseload management of housebound patients with complex nursing needs in the community. As well as supporting more junior staff to grow and develop.
Your role will be crucial in ensuring the delivery of high-quality, evidence-based care to patients, their families, and carers in the community.

• Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.
• Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.
• Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients.
• Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmers.
• Accountable for the standards of care and for determining the activity of your team. This will include acting as a role model for staff, sharing learning and developing practice.

• Adult nursing qualification along with NMC registration
