Detailed job description and main responsibilities Understands the importance of the woman as the centre of her care. Demonstrates that he/ she consult and involves women in decisions about their care and wellbeing. Demonstrates understanding of the boundaries of the Maternity Support Worker role. Confident and competent in all intrapartum related band 3 competencies. Works flexibly across the intrapartum areas to provide excellence in labour care during the latent phase of labour, and support the woman, family and midwife during active labour if appropriate/required. For example, precipitate delivery, woman labouring alone. Is able to assist the midwife in emergencies such as neonatal resuscitation, precipitate birth/haemorrhage. Recognises and respond to emergencies appropriately such as adult and neonatal basic life support. Is able to initiate fundamental care during an emergency to ensure safety. Acts as 2ndperson at birth to support the midwife in essential tasks at the time of birth supporting maintaining a safe birth environment. Is able to undertake after training, clinical skills such as venepuncture, removal of urinary catheters, cannulas, maternal and neonatal observations Is able to support the midwife in the immediate postnatal period with suturing (including gaining competence in swab counts and related records), maternal well-being assessments and can support initiating safe skin to skin. Is able to support women with initiating first infant feed, responsive infant feeding, helping women to gain skills and confidence with attachment and positioning and immediate care of newborn babies. Displays skills, within own sphere of practice, related to promoting womens comfort, mobility, support needs. Uses initiative when responding to changes in the woman or babys condition. Assists the midwives and bereavement team in caring for and supporting women and their families who are experiencing the loss of a baby. Supports with memory making for families. Helps maintain the bereavement rooms and annex to expected standards. Displays excellent two-way communication with women, families and colleagues. Is able to overcome barriers to understanding when the womans first language is not English or has a learning or hearing difficulty. Demonstrates appropriate assertiveness and ability to challenge others when the rights of women and others may be infringed. They will work as part of a multi-disciplinary team in a clinical role and will have senior leadership responsibilities. They will manage a team of Clinical Pharmacists and Pharmacy Technicians across the 3 practices. They will provide clinical supervision, training, support and mentorship. They will set objectives within the team that match both the professional development of each team member and the priorities of the three surgeries. They will develop the clinical skills of the Pharmacy Team within their scope of practice and allocate work accordingly, with direction from the 3 practices. Theywill take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, especially for older people, people in residential care homes and those with multiple co-morbidities. They will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, medicine reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practices. They will provide clinical leadership on medicines optimisation and quality improvement, manage some aspects of the quality and outcomes framework and enhanced services and contribute to the delivery of the local medicines optimisation strategy. They will ensure that the practices liaise with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. Primary Duties and Areas of Responsibility Long-term condition patients Telephone consultations with patients with multimorbidities and, in partnership with primary healthcare colleagues, implement improvements to patients medicines, including de-prescribing. Development of long-term condition patient facing clinics including participation in group consultations. Review the on-going need for each medicine to ensure they get the best use of their medicines. Support achievement of relevant targets. Clinical Medication Review Undertake clinical medication reviews with patients with multimorbidity and polypharmacy and care home residents. Implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests. Support and develop the clinical pharmacists and pharmacy technicians to be able to carry out medication reviews within their scope of practice. Patient facing medicines support Develop patient facing clinics for the Pharmacy Team for patients with questions, queries and concerns about their medicines in the practice. Telephone medicines support Provide telephone support for patients with questions, queries and concerns about their medicines. Medicine information to practice staff and patients Act as a source of medicines information for all the practice team and patients. Suggest and recommend solutions. Management of medicines at discharge from hospital To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Set up and manage systems to ensure continuity of medicines supply to highrisk groups of patients Work in partnership with hospital and local provider colleagues to proactively manage patients at high risk of medicine related problems before they are discharged to ensure continuity of care. Signposting Ensuring that patients are referred to the appropriate healthcare professional for the right care, with an emphasis on training reception teams to help with pathways and liaising with external facilitators where necessary to improve processes. Ensuring that the addition of the pharmacist to the clinical team results in a reduction in workload for other clinicians. Unplanned Hospital Admissions Work with case managers, multidisciplinary review teams, hospital colleagues and virtual ward teams to manage medicines-related risk for readmission and patient harm. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups. Repeat prescribing Produce and implement a practice repeat prescribing policy. Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review. Ensure patients have appropriate monitoring tests in place when required. Medicines Safety Identify national and local policy and guidance that affects patient safety through the use of medicines, including MHRA alerts, product withdrawals and emerging evidence from clinical trials. Manage the process of implementing changes to medicines and guidance for practitioners. Medicines Quality Improvement Programme Working with colleagues, identify and provide leadership on areas of prescribing and medicines optimisation. Conduct clinical audits and medicine optimisation projects or work with colleagues such as GP registrars, practice managers etc., feedback the results and implement changes in conjunction with the practice team. Present results and provide leadership on suggested changes. Contribute to national and local research initiatives. Frailty Take a leading role in the contractual requirements to support frail patients with their medication including medication review and patient identification. Implementation of local and national guidelines and formulary recommendations Work with the ICB to implement local and national guidelines including NICE etc. Liaise with hospital colleagues where prescribing needs to be returned to specialists. Education and Training Provide education and training to primary healthcare team on therapeutics and medicines optimisation. Provide training to visiting medical, nursing, pharmacy and other healthcare students where appropriate. Supervision Provide support and mentorship to clinical pharmacists and pharmacy technicians. Care Quality Commission Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved. Undertake risk assessment and ensure compliance with medicines legislation. Public Health To support public health campaigns. To provide specialist knowledge on all public health programmes available to the general public. Momentum Instore have an exciting opportunity for you to join us as a Beauty Merchandiser on a flexible basis, working 8-hour days Monday to Friday as and when we have projects (2025).

We have a busy 6 weeks from the start of January 2025 (schedules of work to be given in December) and are looking for key people in your location. Paid training days will take place in December and paid before Christmas.

Work throughout 2025 on a project-by-project basis.

Role and Responsibilities


Visiting retailers and updating the stand
Moving parts around and resequencing stock according to a new planogram
Changing over the visuals
Maintaining the stand, keeping it looking fresh for customers that visit the store
Travelling to your depot* for kit collection, which is paid time.

The Probation Service works with around 30,000 offenders a year to support their rehabilitation. Residential Workers work in Approved Premises (AP) to secure homes for offenders who are on release from the prison service and making the transition back into the community. You will be required to keep the AP safe and secure for other members of staff and those currently resident there. Previous experience working within the security industry is required.

Your new role
You will be part of a team providing 24-hour cover within the Approved Premises. This will include both security and care duties. You will be expected to assist in the well-being and supervision of residents, maintaining discipline and adherence to AP rules, whilst remaining social and understanding towards those housed in the AP. This post will involve substantial out of hours work. Working with a 37 hour shift based rota across 7 weeks. Each shift will be 12 hours, so you will not be working everyday of the week.
Duties will include:

* Undertake regular inspections of the building, including cellars and grounds throughout the period of duty, recording any occurrences/damage or defects, and monitor CCTV equipment
* Maintain an active presence and regularly engage with residents in a pro-social manner.

* Contribute towards the protection of residents, such as assisting with risk assessments.
* Ensure that the AP is locked and secure during the curfew period. Confirm the presence and well-being of all residents overnight.
* Supervise residents' meals, follow prescribed medication procedures including the issuing of medication to residents and both weekly and nightly audit of medication sheets, maintain hostel records and files as required, including data entry.

* Deal with visitors and telephone calls, and monitor the work of visiting contractors in accordance with health and safety.
* You will work 37 hours per week, including unsocial hours and weekends.

Hourly rate: 18.32 p/h (inclusive of holiday)
Weekday overtime: 20. 29 p/h (inclusive of holiday)
Weekend overtime: 26.70 p/h (inclusive of holiday)


What you'll need to succeed
Previous experience in a similar field, security and care work will be valuable. You will need to be a strong communicator, level-headed and security conscious.
