The Lead Nurse for the Thames Valley and Wessex Paediatric Critical Care ODN will provide nursing leadership across the Network, acting as a source of expertise, advice and knowledge for providers, commissioning and ICBs. You will work alongside the national PCC ODN lead nurses to deliver on nationally agreed programmes of work and to support regional surge planning for PCC capacity.

You would work with the Network Manager, Education team and Lead Clinicians as part of a multi-disciplinary leadership team to lead the strategic and operational development of coherent and effective network arrangements, and delivery of a programme of work that addresses national and local priorities, leading on user engagement and risk management. some of the key programmes of work for the upcoming 18 months are: implementation of the PCC services specifications for Level 2, Level 3 and Transport services, working in collaboration with LTV providers to develop services alongside the NCEPOD recommendations and implementation of national Paediatric Early Warning scores.

You will also, in partnership with the ODN Education team, PIER and SORT play a key role in delivering education and training for Paediatric Critical Care across the ODN.

This role is primarily a home working role however the ability and flexibility to travel to regional and national meetings is essential.

Clinical Focus
• Maintain clinical experience and provide expertise in the areas of Paediatric Critical Care (PCC).

• Ensure that the network manager is kept informed of significant clinical issues associated with the delivery of PCC cross region alongside the PCC surge plans

• Under the direction of the network manager, engage with a range of stakeholders to support national reviews, regional initiatives, cost and value improvements, whilst keeping abreast with new clinical innovations.

Patient Experience and Quality of Care
• Work with parent/patient representatives and other health personnel to improve patients and family experience throughout their healthcare journey
• Work with the other ODNs to develop a patient and carer strategy
• Provide regular monitoring and reporting updates on the implementation of a “patient and parental engagement strategy” to relevant stakeholders.
• Provide proactive support to the patient support groups that are established within the network, ensuring appropriate links with national charities and third sector organisations.
• Adhere to the network clinical governance procedures whilst providing advice and clinical support during investigations into patient complaints where the care is across the ODN footprint..

• Lead on the TVW PCC ODN Morbidity and Mortality process ensuring learning is shared in a timely manner and any issues escalated via appropriate channels.
• Work with the network manager on quality and effectiveness PCC services through benchmarking and auditing the regional services against national standards and producing a plan to implement the required changes

• Participate in national/international events in order to share best practice and update your knowledge of key areas of healthcare developments
• Contribute to the TVW CYP ODN annual report providing statistics, key performance indicators, service and practice developments in collaboration with network manager and data analyst





Leadership
• Work autonomously across multiple professional, organisational and system boundaries and proactively plan, develop and sustain new partnerships and networks to influence and improve health, outcomes and healthcare delivery system
• Leads on research projects within the network to benefit PCC pathways.
• Work collaboratively with the other network nurses nationally to achieve national goals and to share learning
• Working with the ODN Management Team, the post holder will help to deliver the service specifications and GIRFT recommendations across the Network through collaborative working with L1, L2 and L3 provider organisations and commissioners.
• Coordinate, lead and manage the projects within the PCC workstream and support other workstreams as appropriate.
• Build strong relationships with key stakeholders in all acute trusts, ICB, and Specialised Commissioning across the region.
• Provide an over-arching nurse leadership and co-ordination role within the network for the effective delivery of nursing care, ensuring it is responsive and flexible to patient needs for PCC.
• To work with local trusts to build on the nursing network, providing support, advice, leadership and learning across all bands of the registered and unregistered nursing workforce.
• Provide support for the resolution and investigation of complaints and serious incident investigations, identify risks to patient safety, and establish mitigations for issues that are of cross site concern and where joint learning could be delivered
• To be visible and accessible and to provide forward thinking, clinical professional leadership to paediatric critical care staff at all levels across the region.

Outpatient Pathway

· Administer all new referrals received via the E referral service, ensuring all are electronically triaged by the appropriate clinical staff member and action asrequired.

· Process and log all non-Choose and Book referrals on EPR, including registering new patients in addition to updating existing patient details.

· Book new and follow-up outpatient and diagnostic appointments, ensuring that capacity is proactively and efficiently used. This includes booking patients into the correct clinic to ensure that they are seen by the most appropriateclinician.

· Reschedule outpatient appointments as a result of patients calling the service, in line with the 18-week RTT / cancer / screening targets and agreed local processes inrespectto patient cancellations.

Your core role in Clinical Trials Pharmacy would be to:

You will be expected to work as part of a team in order to assist the doctors with diagnostic procedures and facilitate the efficient running of clinics. You will required to assist in the running of specialised clinics such a our 2 week wait hysteroscopy, vulval and pelvic mass clinics, our colposcopy clinics supporting clinicians carry out treatments following abnormal smears and our general gynaecology clinics that include urogynaecology and endometriosis clinics. This role covers a wide variety of services within women’s health. You will be part of the nursing team to support the department sister in the running of the unit. You will assist in the maintenance of the patient environment in terms of cleanliness, tidiness, auditing, and the restocking and ordering of supplies. You will be part of the team also supporting our Pre op assessment nurses to carry out assessments on our patients that fit certain criteria following out patient hysteroscopy being booked for general anaesthetic.

Responsibilities 1. Use laboratory computer systems to record patient specimens and provide clinical advice on sample selection and necessary laboratory tests for patient diagnosis and management. 2. Perform all sample processing procedures required in the laboratory. 3. Conduct HLA and HNA antibody screening and specification. 4. Carry out flow cytometric crossmatching. 5. Perform genotyping using molecular methodologies and assist in the maintenance and development of in-house genotyping as required. 6. Ensure the accuracy of patient test results entered into hospital, regional, and national computer systems by laboratory staff. 7. Validate and verify relevant laboratory tests. 8. Independently perform a wide range of specialised tests according to standard operation procedures (SOPs), analysing, interpreting, and reporting results. 9. Assimilate highly complex information to produce accurate reports. 10. Participate in internal and external quality assurance schemes (e.g. UK NEQAS for H&I) and collaborate with senior staff to act on results. 11. Monitor consumable supplies and equipment to ensure laboratory function. 12. Oversee day-to-day performance and ensure tasks are performed to a high standard. 13. Independently perform emergency out-of-hours on-call duties for solid organ and islet transplantation, including advising surgeons on transplant rejection risks based on complex data. 14. Evaluate patient-related data to assist in therapeutic decision-making. 15. Maintain flexibility with working hours and participation in the 24/7 on-call rota. 16. Respond to on-call notifications within one hour at any time. 17. Ensure all duties are performed to the highest standard, following relevant SOPs. 21. Order consumables and laboratory equipment in consultation with senior staff. 22. Maintain accurate records of all work and general laboratory activities. Prepare workload and audit data as required. 23. Create, format, and maintain databases for clinical audits and patient records. 24. Liaise with senior staff to address any problems in the laboratory. 25. Train and provide practical instruction to colleagues and visiting research scientists, ensuring efficient and cost-effective resource use. 26. Develop and maintain relevant SOPs in consultation with senior staff. 27. Adhere to safety policies for handling infectious tissue samples and hazardous chemicals, maintaining relevant COSHH and risk assessment documentation. 28. Actively participate in laboratory meetings. 29. Contribute to research projects and publish findings in peer-reviewed journals, as directed by senior staff. 30. Engage in appropriate CPD schemes, present articles at the laboratory journal club, attend local and national meetings, and maintain the required level of professional attainment. 31. Perform any reasonable duties related to the position, as requested by senior staff. Special Duties 32. Design and implement internal quality assurance schemes to ensure service compliance with national and international accreditation standards in consultation with senior staff. 33. Communicate effectively and accurately with users, including attending Multidisciplinary Team (MDT) meetings and acting as a point of contact for users. 34. Organise and plan workload to ensure efficient service delivery. 35. Initiate service development by reviewing and costing laboratory tests and planning laboratory procedure improvements in consultation with senior staff. 36. Evaluate costs and implement new technology in collaboration with senior colleagues.

Outpatient Pathway Administer all new referrals received via the E referral service, ensuring all are electronically triaged by the appropriate clinical staff member and action as required. Process and log all non-Choose and Book referrals on EPR, including registering new patients in addition to updating existing patient details. Book new and follow-up outpatient and diagnostic appointments, ensuring that capacity is proactively and efficiently used. This includes booking patients into the correct clinic to ensure that they are seen by the most appropriate clinician. Reschedule outpatient appointments as a result of patients calling the service, in line with the 18-week RTT / cancer / screening targets and agreed local processes in respect to patient cancellations. Actively review patient DNAs, liaising with the appropriate clinical staff and reschedule patient appointments in line with agreed local processes. As required, complete clinic cancellation forms and ensure appropriately authorised (minimum 6 weeks' notice required) before processing the clinic cancellation on EPR and rescheduling of patient appointments. Adding additional clinic capacity on EPR as directed by management, to include the use of adding appointment slots on EPR that can be booked via Choose and Book. To be responsible for actively managing the OP PTL and Incomplete lists. General Demonstrate high levels of customer care and be an ambassador for customer care within the Trust. Provide a robust administration function that underpins the delivery of a high quality service and maintain effective working relationships with clinical, nursing and administrative staff. Answer all telephone calls in a timely manner and action as appropriate. Maintain patient confidentiality at all times. Be flexible in your approach to work, such as covering other job roles at an appropriate grade or site, and to include varying working hours to ensure the service maintains a minimum level of cover during its core hours of 08:00 to 17:00. Retrieve notes and ensure all patient information is available in a timely manner for all appropriate outpatient appointments, diagnostic appointments and inpatient / daycase procedures, checking demographics at every stage and locating lost notes where appropriate. Support the Team Leader, Patient Pathway Manager and Service Manager in the investigation and resolution of patient queries and complaints received direct to the service or via PALS / Complaints Department. Book interpreters for patients prior to appointments and admissions as and when required. Liaise with Clinical coders to ensure patient notes are available so that all patient episodes are coded accurately and promptly. To support and contribute to on-going and future service development projects supporting continuous improvement of the services we provide. Take on alternating roles alongside other members of the PD team to: Provide advice, education and support for patients and staff in the renal unit / other departments. Deal with any outpatient telephone queries and assess patients in the unit. Provide nurse consultations in the clinic or at the patients home. Liaise closely with the Home Therapies/ PD nurses in Network units, providing peer support. Promote a culture that supports and enables self-care. Support the development of patient focussed education, including training to self-manage on-going consequences of treatment. 2. Liaise with primary care teams regarding the care of home therapy patients, providing education and information when required. Visit patients in other hospitals as required, ensure patients have access and able to do PD. 3. Contribute to the on call rota 4. Oversee the care and treatment of a caseload of patients working autonomously as their named nurse, (including a number, of pre-dialysis patients) and: Ensure tests / blood tests are done as per protocol. Ensure patients receive optimal dialysis treatment, taking quality of life into account, making necessary changes to patients regime as required. Ensure refresher training is carried out as protocol. Conduct a root cause analysis after every new episode of PD peritonitis. 5. Involvement in the day-to-day administration of the PD/home HD programme by: Following up new patient referrals and planning home visits. Co-ordinating operation dates / training dates. Organising the delivery / collection of supplies and equipment. Ensuring patients clinical waste collections are arranged. Ensuring companies have up to date information on patients regimen. Liaising with companies re: patients holiday deliveries. Recording infection episodes, admission rates, reasons for patients coming off PD etc. for audit purposes / clinical governance on appropriate activity sheet. Monitor in patient activity for therapy costing purposes. Maintaining accurate EPR patient records. 6. Use current research / best evidence to underpin clinical practice. Involvement in any research studies underway in the unit. 7. Ensure that all equipment used by staff or patients is maintained in a safe working condition and that any faults are reported to technical staff. Ensure patients / relatives are taught how to use specific machines in a structured teaching session and their competency is documented and reviewed as appropriate. 8. PGDs Ensure assessment and competency achieved to allow use of PD PGDs, be involved in audit yearly. 9. To manage complaints from relatives/patients, to prevent further escalation whilst promoting support to the manager. Education 1. Supervise, teach and support nursing and non-nursing staff involved in direct patient care. 2. Use a variety of teaching resources and aids to provide a structured patient training programme, which can be adjusted to meet the learning, needs of each individual. 3. Participate in formal teaching sessions on renal unit study days. Provide clinical support on informal basis for ward staff. 4. Mentor undergraduate / post registration renal course students. 5. Act as a role model in expert clinical evidence-based practice. 6. Be responsible for ones own professional development and continuing education in both nephrology and RRT Nursing, and also professional issues in nursing. 7. Contribute to protocols, documentation systems, standards, policies and clinical guidelines for others to use in practice. 8. Take available opportunities to submit written work for conferences or journals. Professional 1. Discuss performance and future development needs using the appraisal framework. 2. Develop personal and professional knowledge through critical self-appraisal, clinical supervision and networking with other health care professionals locally and nationally including project work. 3. Acquire skills and knowledge to properly implement the computer system and any other systems, which may be implemented as resource management tools. 4. This post is for 37.5 hours per week generally Monday Friday working hours but there will be an expectation to work occasional weekend and evening clinics.
