Key Areas/Tasks To undertake clerical duties, this includes providing and receiving routine information requiring tact or persuasive skills. To contact patients via the telephone to book their outpatient appointment in accordance to Trust targets. To set up ad hoc outpatient clinics as and when this is required. Communicate tactfully with patients to book an appointment, answering any non medical questions the patient may have regarding their outpatient appointment. Monitoring the number referrals received and escalating any capacity issues to the Directotate to comply with waiting time targets. Ensure that the patient Access Policy is adhered to at all times and communicate any issues accordingly. Emailing any appropropriate information to patients regarding their appointment Taking direction from the Directotate when booking patient appointments. Manaully uploading the patients referral and any other relevant information from E-referral onto Iportal in readiness for the patients outpatient appointment. Communicate with patients and their relatives who are telephoning to update their personal details and change or cancel appointments. Deal with enquiries where there may be barriers to understanding. Should have GCSE Maths and English (grade C or above), or equivalent qualifications, or have at least 1 years experience in a customer service environment. The post holder will search for patients using Careflow to ensure the correct patient is identified and correcting patient demographics when incorrect. The post holder is expected to employ tact and diplomacy with all patients and staff when dealing with problems encountered and to refer to a supervisor if necessary to resolve issues beyond their capabilities. The post holder will be sitting for data input for prolonged periods of time. Requires concentration with regular interruptions during the working day. You will be expected to: Work as part of multidisciplinary teams providing care to children and young people with mental health needs, across Redbridge. Provide medical support to the team, and direct input on clinical cases. To undertake through each patients individual care plan, those duties delegated by the Team Leader and Case Manager, which the Health Care Assistant has been trained and is competent and confident to perform. The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. Our aim is to work together in partnership to join up health and care services, improving our populations health and wellbeing and reducing health inequalities. One such example of this collaborative work is the launch of our HCA Academy, an ICS initiative between Essex Partnership University NHS Foundation Trust (EPUT) and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care assistant (HCA) workforce. Undertake a patient facing and patient supporting role to ensure effective medicines use, through shared decision-making conversations with patients. Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews, processing prescription queries and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively. Support, as determined by the GP Lead and Clinical Pharmacist, medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, reviewing medication upon discharge / admittance. Take a central role in the clinical aspects of complex drugs and shared care protocols and liaising with specialist pharmacists for more complex patients. Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing. Assist in the delivery of medicines optimisation, management incentive schemes and patient safety audits. Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. This will be achieved through undertaking clinical audits (e.g. use of antibiotics). Supporting quality improvement measures and contributing to the Quality and Outcomes Framework and locally commissioned enhanced services. Supervise practice reception teams in sorting and streaming general prescription requests, so as to allow GPs and Clinical Pharmacists to review the more clinically complex requests. Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS) to the administration team. Perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the GP or Clinical Pharmacist. Support the Clinical Pharmacists in Structured Medication Reviews (e.g. organise necessary monitoring tests before SMR). Support medicine reconciliation of patients whose care is transferred back into primary care in a timely and effective manner liaising with patients and other providers to ensure patients receive appropriate medication post discharge. Assist Clinical Pharmacists with any searches, data collection and medicine safety tasks. Implement changes to medication that result from CAS/MHRA alerts, product withdrawal and other local/national guidance. Work collaboratively with the Practice team. Develop relationships and work closely with community, hospital and MMT pharmacy colleagues and the wider health system. Participate in practice meetings, patient participation groups and other local meetings to improve engagement with the role of pharmacy technician.
