The Person:

We are looking for a motivated and ambitious finance professional with relevant skills and experience to lead our client function which supports the end-to-end budget management process for our three Local Authority Clients.

Reporting to the Chief Finance Officer, this is a chance to help redevelop and shape the financial support provided to our client councils as we respond to the exciting challenges that are presented by recent formation of the South East Lincolnshire Councils Partnership.

You should be a CCAB qualified accountant and have experience of local government finance. You will possess the ability to lead and motivate a team as well as manage workloads within tight timescales.

We are looking for a Senior Product Manager to help lead our Builder Service Experience focus within our Platform and Data Services subdivision.   The Platform and Data Services subdivision primarily builds tools, frameworks, and products for use by internal athenahealth development teams within our athenaOne flagship product.  Additionally, we also have a suite of technical products such as APIs, Eventing, CDS Hooks, SSO, etc., that can be leveraged by external customers who want to build on top of the athenaOne ecosystem.  These products may be used by existing athenaOne customers who want to build their own apps or by third-party health IT companies who will innovate specific workflows leveraging these tools.

POSITION SUMMARY:

The Transitions of Care (TOC) Operations Manager will provide operational support to transitions of care programs at Boston Medical Center (BMC). The Manager will work with inpatient, outpatient and community-based staff to ensure that programs have operational and programmatic support and will directly supervise CWA (community wellness advocates) across multiple programs and interventions. 

One of the programs that the Manager will support is the HARP program at BMC, which follows high risk patients who have been discharged from BMC for 30-60 days post discharge. The program’s aim is reduce hospital readmissions. The team consists of nurse practitioners, CWAs and program leadership. Team members provide care management through home and community-based visits and phone calls. They also support patients in meeting medical and behavioral health needs as well as social determinants of health needs.

In this role the Manager will provide supervision, professional development and support of HARP CWAs and, in the future, CWAs in other roles that also support transitions of care or high-risk patients with chronic medical conditions. Specific responsibilities in these areas include day-to-day oversight, performance management, recruitment, onboarding and participation in educational program planning. The Manager may also provide more intensive hands-on support for individual CWAs working with challenging cases, helping the CWA address social barriers and/or partner more effectively with a community partners. Collaboratively with the Value Based Care Medical Director, HARP Medical Director, HARP team and other stakeholders as necessary, the TOC Operations Manager will participate in program improvement initiatives, bringing their unique expertise in social determinants of health and the CWA role to program design and implementation strategy. Additionally, they will be responsible for supporting HARP team operations such as making sure the team has any necessary supplies and equipment, keeping track of team schedules (including calendar and time off), providing support for team and program paper materials (e.g. brochures) and other operational tasks as needed.

In the future, the Manager will be responsible to help design and operationalize new interventions across BMC that involve CWAs who support additional chronic disease management and TOC programs. The Manager responsibilities will include day-to-day oversight, performance management, recruitment, onboarding and participation in educational program planning. The Manager may also provide more intensive hands-on support for individual CWAs working with challenging cases, helping the CWA address social barriers and/or partner more effectively with a community partners.

Candidates for this role must have excellent interpersonal skills, operational experience and experience identifying and address social determinants of health among vulnerable patients. Must have prior social work and/or community health or advocacy experience.

• Provide operational support to the HARP program including managing documents and supplies for team, coordinating meetings and trainings, providing support for team meetings
• Supervise a cohort of CWAs, including systematically reviewing their patient panels and the quality of their work and documentation, providing feedback and coaching, and conducting formal performance appraisals. 
• Responsible for recruitment and onboarding of CWAs. 
• In collaboration with other BMC leaders, identify HARP and other TOC CWA learning needs and work to meet educational needs of CWAs through educational programming, coaching, and mentoring. 
• Serves as an expert resource to CWAs, HARP and TOC interdisciplinary teams and other leaders for consultation, problem solving and ongoing program development. 
• Participates in updating CWA performance standards and competencies insuring practice keeps abreast of local and national advances in CHW practice.
• Partners with other HARP, VBC and BMC leaders build strong relationships with community agencies and resources that support social determinants of health for BMC patients. 
• Participates with other staff in activities that include community outreach, presentations to community organizations, development of materials, and phone calls. 
• As an expert in the CWA role and in social determinants of health, is actively engaged with the broader HARP and VBC inter-disciplinary leadership team, in discussions, decisions and initiatives to improve VBC lead programs
• This role will be in person on the BMC campus and intermittently community based for CWA supervision 
• Follow established hospital infection control and safety procedures.
• Perform other duties as requested or needed.
