Management of Care: To provide care and interventions, risk and safety management and safeguarding of all patients on the Collaborative Care team caseload and in reaching to Castleberg Hospital. To be responsible for efficient caseload management and work without direct supervision practice, to work within the HCPC Standard and OT Code of Professional Conduct To provide clinical education and training to occupational therapy students to graduate level and other members of the team to allow them to gain occupational therapy competencies. To be an active member of the senior team within the Collaborative Care Teams, participating in service development, to be shift co-ordinator in the team managing referrals, day to day management of the team and identified managerial responsibilities. To undertake a comprehensive assessment of patients with frailty, complicated physical and mental health presentation, and multiple pathologies. To include patient counselling on the storage, correct dosage/use, side effects and other relevant information about the drugs supplied. Accurately handle payments of prescription charges. Support the service to the outside hospitals, clinics and nursing homes. Provide drugs and medications requested by the agreed time to the correct location. Supply the service to patients taking Clozaril. Under the supervision of a pharmacist, the dispensing of Controlled drugs Assist with the Extemporaneous Preparation of Medicines and over labelling/pre-packing procedures. To carry out reception duties including the answering of telephones and their appropriate transfer to other staff. To provide administrative support to the anticoagulant service, ensuring that any calls on the answer machine are dealt with in a timely manner and all patient appointments are actioned efficiently. Homecare To receive and raise orders using the Homecare EMIS module to forward to Homecare Companies along with prescriptions prioritising as appropriate. To support the delivery of training on Homecare Medicines and the Homecare EMIS module for relevant members of the multi-disciplinary team. To maintain accurate and up to date records, including patient data and financial information. To input patient and detailed drug information accurately into the Homecare EMIS Module. To ensure compliance with current Homecare medicines management guidance. Minimise delays in delivery of medication to patients by ensuring that orders are placed promptly and prescriptions are made available to the Homecare company at the appropriate time. To be a point of contact regarding the medicines home delivery service for patients, Homecare providers and the AHNSFT staff. To interpret and follow up prescription queries. Our Care Home service, known as Community Houses, has rapidly expanded with 4 sites across Keighley that offer a step-down service to adults with complex mental health needs.

We are a unique service which offers enhanced care to our Service Users to continue with their rehabilitation journey, including physical health monitoring with support from local medical centres.

Each service is a large residential house with an office and is where you will help our Service Users to carry out routine daily tasks - everything from help with cooking, cleaning, activities in the community and encouraging independent living in their future. You will even have day trip visits to the seaside with service users as part of this role.

What you will be doing

You will join the qualified and experienced team that supports people with mental health conditions. As a step-down service between Three Valleys Hospital and Independent Living, service users who are ready to leave the hospital setting but do not quite have the full life skills to live on their own in the community are supported.

Whether you have been a Healthcare Assistant, Support Worker or a Care Worker, you can utilise your experience to support people to lead a valued and fulfilled life.

We have a 4-tier working structure and all Senior Support Workers are on one level. You will report to a Team Leader with the Registered Manager and Deputy on-site daily and can be approached for support, questions and anything else you will need. The last staff survey identified that at our staff answered yes to the question ‘I feel well supported by my line manager’.

You will work across all four houses and encourage our Service Users to lead a full and valued life, helping them to live more independently, so a positive attitude, compassion, resilience and vigilance are important. It can at times be a challenging environment, but regular feedback from staff is that it is hugely rewarding to see people improve; being able to do something today that they couldn’t do last week because of you.

As a 24/7 service the standard shift pattern is on a rolling two-week basis covering days, nights and weekends. Overtime is also available and occasional flexibility can also be offered.

Senior Support Worker duties include:

• Assist Service Users with their day-to-day activities i.e. Cooking, cleaning, washing, some personal care
• Contributing to individual recovery journeys
• Promoting independence and empowerment
• Writing and updating care plans
• Recording Service User electronic notes
• Promoting activities and supporting Service Users with in-house and community-based rehabilitation

The role in some of our community houses involves independent lone working, medication administration and contributing to CPA’s, therefore some previous knowledge and experience in these areas are required.
