Clinical responsibilities: The post-holder will be required to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness in consultation with patients and in line with current practice disease management protocols, developing care plans for health. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate. Recording clear and concise consultation notes Collecting data for audit purposes. Prescribing in accordance with local formulary and national guidelines The post-holder may be required to visit patients during surgery time in an emergency if clinically indicated. The post-holder may be required to support enhanced access sessions as per the needs of the PCN. Communication Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment. Recognise the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Work as part of the team to seek feedback, continually improve the service and contribute to business planning. Learning and development Take responsibility for own learning and performance through continuous professional development, including participating in supervision, acting as a positive role model and assessing own learning needs. Act as a mentor and positive role model to students and junior members of staff, sharing information and good practice. Contribute to education and training of trainee clinical staff and students. Maintaining confidentiality The GP will, as an integral part of and in the course of carrying out his/her role, have access to confidential and sometimes sensitive information relating to patients, carers and family members, as well as similar information about colleagues. They may also have access to confidential information about the practices and the PCN. All such information, in whatever format and howsoever made available, must be treated with the strictest confidence. All such information held within and outside of the practice will be shared only as necessary, and in accordance with the practices confidentiality / data sharing policies, and in line with data protection legislation and the Freedom of Information Act. Clinical responsibilities: Maintains personal contact with patients and meets their needs on anindividual basis, using the process of listening, assessing, planning, implementing and evaluating care. Can deal with the anxieties of relatives and arranges the support they may need. Reports any untoward incidents. Maintains security. Carries out Midwifery care to the highest possible standard according to agreed policies and procedures, and the Midwives, Rules and Code of Practice. Ensures that the appropriate practitioner is informed promptly of any complication in the mothers or babys condition. Is prepared to work in areas of the midwifery service other than the ward / department / community in which based, if required to do so to meet the needs of the service. Able to manage a caseload in the community; to take 24-hour responsibility for area of practice including participating in an on- call rota; with the further opportunity for some posts to be rotational to the hospital. Maintains the supply and custody of drugs, medicines etc, including keeping of agreed register. Ensures that measuring, administration and recording of all drugs and medicines is carried out in accordance with the rules of the NMC, and as laid down in Trust policy. Ensures accurate and contemporaneous records of all care given and audit same. Complete and monitor other records as required by statue and local policy. Liaise with all other personnel regarding access to the department, promoting relationships to enhance the quality of care to the consumer and ensuring effective communication with health care professionals. To ensure that evidence based practice is implemented and audited on a regular basis, to encourage and promote innovative practice. Please see attached job description for more information.

Patient contact will largely be incidental with the exception of patient requests for access to Health Records, which may be received in person. Help to oversee and support the wellbeing of the team Assist with recruitment and retention coordinating and adjusting the daily workload of the teams Notify the DHROM of areas of pressure that may exist so these can be reviewed; help the DHROM mitigate or resolve departmental pressures using basic problem solving and analytical skills Undertake initial performance management and sickness absence tasks and procedures as per Trust policies. This will occasionally involve distressing circumstances e.g. if a staff member requires informal capability or conduct management. Undertake annual appraisals and ensuring staff are cross-trained in all aspects of the scanning bureau processes whilst ensuring any barriers to understanding are addressed Ensure all new starters are provided with welcome and training packs on their first day and provided with uniform Help to arrange Trust and Local induction for new staff Actively participate in and support the departments maintenance of the BSI 10008 compliance accreditation.

To ensure accurate extraction of complex clinical details relating to diagnoses and operative procedures for each consultant episode from patient medical records To engage and support the internal continuous audit process ensuring that a personal IG level 2 audit result is achieved and maintain To assist in the daily supervision of trainee and less experienced clinical coding staff providing mentorship and cultivating a supportive learning environment To interpret histology and radiology reports to ensure accuracy of the coding process To undertake the coding of extracted information ensuring adherence to National Standards and local policies To accurately enter the clinical codes onto the Trust information system PAS/Medicode To identify insufficient/incorrect clinical data in source documentation and using own initiative to raise the query for resolution with other relevant departments To share and assist other colleagues with personal coding experience and promote a learning environment at workplace To organise daily workload efficiently so that personal coding targets are met, including allocated Consultant backlog list, supporting the coding team in achieving high standards of coding completeness and accuracy at agreed timescales To take responsibility for self-development on a continuous basis by studying for the Auditor and/or Trainer qualification, if required by the Trust. To participate in ongoing coding training and testing to meet National Standards and Trust requirements. This may require travelling to other organisations outside core working hours To liaise with Clinicians and their support staff concerning interpretation of case note documentation affecting coding To investigate and correct coded clinical data rejected or queried by recipient information systems To communicate complex coding rules to clinical coding staff, medical, clinical and administrative staff and external agencies. Implement agreed local policy directives and assist in the development of local coding policies which impact on the various service areas Undertake/Participate in complex audits To work across all specialities
