What you will be doing

As a qualified Mental Health Nurse (RMN) or Learning Disability Nurse (RNLD), you will ensure adults with mental health conditions in this Mental Health service receive high-quality care, while also supporting your colleagues and promoting positive teamwork.

The wards are busy, with plenty to keep you engaged and motivated, and a good amount of routine and variety. Your day-to-day will include:

• Providing high-quality, effective and compassionate nursing care to service users
• Being responsible for the assessment, planning, implementation and evaluation of care
• Providing mentorship and support to team members
• Observing, recording and reporting all service user changes, and maintaining accurate records of care provided

You will be working at Chadwick Lodge and Eaglestone View, a medium and low secure and locked services providing specialist treatment programmes for service users who may have been detained under the Mental Health Act (1983) and have a history of offending behaviour.

The service offer a broad spectrum of evidence-based therapeutic interventions, involving service users at every stage. The hospital’s consultant-led multidisciplinary team has a wealth of clinical and research experience in forensic psychiatry. All programmes are designed to reduce the risk of re-offending and to maximise the potential and enhance the quality of life of each service user.

Chadwick Lodge secure services care for service users with complex and enduring mental illness who need to achieve stability in order to benefit from multidisciplinary interventions, helping them move on to community settings whenever possible.

The post holder will be a member of the Primary Care Plus team. .This post has the responsibility for providing effective day to day mental health care and management to clients with mental health problems presenting in Primary Care. The PCP team provides a single pathway for assessment, referral management and care coordination on behalf of patients with more complex mental health problems presenting in Primary Care. The post holder will offer short term mental health interventions to support this client group within primary care settings and will work at the primary secondary care interface to facilitate movement of clients between primary and secondary care services. PCP provides a dedicated service package to support those newly discharged from secondary care and their GP practice. An innovative and exciting service continues to be developed in Milton Keynes to support individuals with wellbeing and mental health needs in GP surgeries. We are seeking an enthusiastic, dynamic and motivated Band 6 mental health professional to join the team.

The role will be Full Time and will involve assessing and providing support to clients whom have a range of mild to moderate mental health needs, you with work extensively alongside GPs and practice staff with support from the local mental health service.

You will need to have excellent assessment skills, be able to work autonomously and be able to forge positive working relationship with primary care staff and clients. You will have excellent working knowledge of wellbeing and mental health care and the mental health needs of dynamic and changing population.

The post holder will be based and embedded within a Primary Care Network (PCN) working with GPs, practice nurses and other Additional Roles Reimbursement Scheme (ARRS) workers e.g. social prescribers, health and wellbeing coaches, care co-ordinators, as part of the primary care Multidisciplinary Team (MDT).

They will work with primary care GPs to manage the mental health needs of their population at the point of contact. This includes those with lower level support requirements that can be met within primary care, working with the Primary Care Plus (PCP) team and GP practices. The role will act as interface between primary care and wider mental health services.
