JOB PURPOSEThe Postholder will report to the Senior Health Improvement Practitioner as part of the healthimprovement, Healthy Knowsley Service. Complete all administration processes relating to the health coaching offer in an accurate and timelymanner, including record keeping, outcome measures, use of the RiO system, spreadsheets andreferral processes.13. Maintain accurate, clear, concise and contemporaneous records as per Trust policy and communicateinformation to members of the team as necessary.14. Take appropriate immediate action following incidents and subsequent reporting and completion ofTrust documentation. Appropriate management supervision is provided to all staff, including direct reports to enhance wellbeing and optimise effective practice in The Service. This will provide a regular opportunity to review performance, wellbeing and facilitate open communication. Complete pre-training stakeholder engagement to ensure that the training offer is utilised. Deliver and oversee training provided by Wellbeing Practitioners, including session evaluation. This will ensure a high quality training offer themed on Making Every Contact Count or Oral Health Promotion. Contribute to service and stakeholder meetings, including Life Rooms meetings and Public Health Steering Groups. Thereby, facilitating a cascade of information between the Healthy Knowsley Service and the wider system. Review relevant NICE guidance for service, completing GAP analysis of recommendations and updating Service pathways to ensure compliance. Any changes to pathways will be done in collaboration with the Health Improvement Delivery Manager with changes cascaded to relevant staff delivering interventions. All changes made to pathways to implement changes will be monitored to ensure their effective introduction to ensure compliance with recommendations and assure the quality of the service delivery. Review, update and cascade Service Standard Operating Procedures (SOP) in collaboration with the Health Improvement Delivery Manager. Monitor the fidelity to the SOP through quality check of delivery and data. Thereby, ensuring an accurate overarching document that influences the quality and safety of service delivery. Lead Communities of Practice sessions to increase awareness of modifiable lifestyle factors. Engage relevant stakeholders and partners to ensure a broad representation at the sessions from agencies across the Borough. In doing so there will be an opportunity to increase the opportunity for health promoting practices and environments. Utilise a range of methods to collate patient feedback including the facilitation of Service User focus groups. Share the feedback with the Health Improvement Delivery Manager and the wider team. Utilise the feedback to drive continuous service development, updating SOPs accordingly. Establish stakeholder relationships to ensure that the Heathy Knowsley Service is well connected to partners in both Mersey Care and the wider system. Foster collaborative relationships that facilitate referral pathways and complementary support to ensure that individuals are able to maximise wellbeing.

PRINCIPAL RESPONSIBILITIES1. To assist in the provision of evidence-based training to upskill staff working in care homes,schools and early years settings in order to promote and deliver good care with confidence.2. Support the Healthy Knowsley Service Senior Health Improvement Practitioner to support promotion of oral health Standards, taking responsibility for own allocated workloads of staff from partnership groups.3. Engages in the facilitation of learning and development interventions with colleagues from the Healthy Knowsley Service team and from the wider organisation.4. Liaise directly with Lead Practitioners and/or subject matter experts in amending sessions asrequired.5. To provide educational practical sessions in oral health training under the supervision of the Senior Health Improvement Practitioner.6. To quality check oral health promotion practices (supervised tooth brushing) in early yearssettings to ensure that practice is completed in line with training provided.7. To maintain competencies relating to oral health promotion.8. To assist in the production of all relevant documentation to support the administrative process required for all oral health programmes.9. To provide advice and guidance acting as a mentor within the work setting where appropriate.10. To assist in training to ensure that it is delivered consistently across staff groups and meets thelearning outcomes established by subject matter experts and /or the Healthy Knowsley Practitioners.11. Duties and responsibilities: Phlebotomy. Processing and management of laboratory samples via GPTR system as requested by GPs/nurses. Perform ABPI on patients when requested by GP as per protocol. Administer influenza, shingles, pneumococcal, B12 vaccines to eligible patients as outlined in practice protocol. Basic wound care (dressings) under guidance of GP/Practice Nurses. ECG recording. Job Purpose

To provide strategic and operational leadership to The Life Rooms health improvement function in The Healthy knowsley Service to ensure an offer that meets the needs of local populations and involves partner organisations in an authentic manner.

8. Ensure that the health improvement team adhere to all agreed data collection processes by regularly scrutinising data to provide up-to-date reports for the Operational Management Team and the Senior Leadership Team on performance against KPIs, and outcome measures.
9. Work proactively with the Quality and Research Lead and the Research Team to identify quality improvement initiatives around health improvement and contribute to Life Rooms service evaluation and research activity.
10. Provide statistical reports for Senior Leadership Team and Commissioners from software used within Service.
11. Be aware of national training programmes to ensure that the health improvement team remain up to date with the most current implementation of health improvement thinking that meets the evolving needs of the local population. This will include managing staff competencies and proactively encouraging and supporting staff development.
12. Work collaboratively with partner and community organisations to develop accessible pathways of referral for service users.
13. Work collaboratively with clinical colleagues to develop accessible referral pathways into the health improvement team which then allow them to access the outcome of their referrals.
14. Undertake the full range of human resource activity for members of the Healthy Knowsley Service including recruitment, induction, training, supervision, sickness management, grievance, and disciplinary management etc.

The post holder taking the lead for the Healthy Families Team will be responsible for the case management, assessment, triaging, planning and delivery of care for parents, children and young people using evidence based interventions. This will be delivered in line with the National andLocal guidelines and public health priorities.
