This is an exciting opportunity for a senior mental health practitioner to join the Fylde Coast Home Treatment Team.

The Fylde Coast Home Treatment Team provides 24 hour support to service users who are suffering from mental health concerns that would otherwise require admission to an acute in patient environment. There is a strong focus on a recovery model and effective risk management to provide high quality care in a community setting.

An exciting opportunity has arisen to join the Children's Community Health Service as a Safeguarding and Children in Care Lead 0-19.

The Post Holder will be responsible for leading on the delivery of safeguarding to the Universal health service and children’s community health services (0 – 19 yrs), including Health Visiting, Family Nurse Practitioners and School Nursing. The post holder will act as the lead practitioner during duty shifts, providing leadership and supervision and providing specialist knowledge and support to the teams on a daily basis. The Role;

As an Activities Coordinator you will take a key role in delivering the provision of activities, focussing on functional daily living skills whilst reducing risk issues. You will deliver activities on a one to one basis as well as group sessions, comprising of meaningful therapeutic activities, to promote independence and wellbeing.
