The Single Point of Access (SPA) Administrator is crucial to the whole service as it determines the timely response to referrals, guaranteeing the safe, appropriate management of patients throughout Brent Unplanned Care services.

The SPA is an administrative service whose purpose is to streamline and coordinate all referrals to BRR, providing a central base through which referrers can access the service. The Administrator coordinates the whole service pathway and monitors the process.

The Administrator is an integral team member and is vital to the smooth day-to-day running of the service. Must maintain professional practice standards, ensuring statutory requirements are met within the framework of the Community Care Act and Council Policies and those of the Acute Trust. Be an active member of the SPA, ensuring there are safe and appropriate pathways between all agencies.

Please note this is a fixed term contract for 14 Months
• The Postholder will have excellent communication skills and be able to take oral referrals, mainly by telephone, from GPs and other organisations. This will require exceptional listening skills and interpreting accurate, sometimes complex information.


• The Post Holder will be able to triage referrals, distinguishing between urgent and less urgent referrals within the service pathways and ensuring that every referral is directed to the appropriate pathway. This is a crucial element of SPA as it determines the safety and appropriate, timely care of all patients. Training for this will be competency-based.
• The Postholder will lead in liaising with all health and social organisations and updating them on any potential changes in the care management plans.
• Provide sometimes complex information to include patients and carers. Also, request assistance, especially when there are barriers to language and understanding. This may include gaining informed consent in sensitive circumstances and demonstrating empathy with patients, carers, and families.
• Be proficient with IT systems, competent in sending relevant information and documents to other professionals/disciplines, and able to communicate by phone, writing, email, etc., often in the context of frequent interruption.

· Maintain confidentiality of information consistent with legislation and Trust policies.

CLCH BRENT RAPID RESPONSE - SENIOR PHYSIOTHERAPIST

We are seeking an enthusiastic Specialist Physiotherapist to join our Unplanned Care team based at Sudbury Primary Care at Wembley, London delivering care to patients closer to home and preventing unnecessary hospital admission in Brent.

This role will primarily work under the Rapid Response pathway, but may require the candidate to work across the other pathways when required.

All pathways deliver nursing and therapy care in a patient’s usual home environment.



RR focuses on preventing avoidable hospital admissions for patients with acute ill-health conditions by providing holistic assessment within 2 hours of referral and treatments over a 5 day pathway.

The Brent Health Matters Clinical Team, are looking to recruit a team of enthusiastic and forward thinking GP’s to join our dedicated and friendly team, to help us deliveran integrated approach to thecare, tovulnerable patients that do not normally engage with health services that have beenreferred by local GP’s or identified through staff interventionsby BHM team.

The Brent Health Matters Clinical Service delivers bespoke, holistic, health and social care assessments, to the identified patient cohorts and require GPs to provide their clinical knowledge, expertise and support, to ensure the provision of its high quality, effective and safe patient care.

The aim of the Brent Health Matters programme is to reduce the variations in health inequalities, which in turn will support our communities to better manage long term conditions.

This will include seeing these patients in a clinic based setting, at their home or other setting in the community.


• Respond to medical problems presented by patient’s or the clinical team, including history taking, examination, supporting diagnosis and requests for referrals, where appropriate, to the registered GP
• To provide clinical oversight to the team including reviews of the services Quantitative and Qualitative KPI’s.
• To assist and support relationship building with key stakeholders that may use or refer into the service
• To provide clinical GP support, feedback and strategies at agreed BHM community events.

An opportunity has arisen to work within the Brent Health Matters Clinical Services at Central London Community Healthcare NHS Trust. The service works directly with local communities in Brent, to engage with people having difficulty accessing health services, improve health education, healthcare needs and some of the social determinants of health inequalities.



Working alongside our senior nurses, AHP’s, GP’s and admin members, support patient’s to regain and sustain their levels of function and self-management, with respect to their long term conditions. The Brent Rapid Response team is an unique service that aims at providing a high quality clinical service in the patient's own home and preventing unwarranted admissions to hospitals. Brent Children and Young People’s Health Inequalities Team

Central London Community Healthcare Trust have an exciting opportunity for 2 x Senior Community Nurses for CYP within the Brent Health Matters clinical service.

The Brent Health Matters (BHM) programme reduces health inequalities in the community by targeting hard to serve and less engaged members of our communities in Brent. The programme has been successfully supporting adults in the community and funding has now been made available to develop a new Children’s health inequalities team within the programme to work alongside the very successful service for adults.

The Brent Health Matters team works with a wide range of system partners to provide holistic health and social care assessments to identified cohorts of patients in the community. The MDT clinical service will support hard to serve children and young people (CYP) to access a wide range of community services, additionally promoting self-management of their health conditions.

The key priorities of the service are aligned with the ICP priorities for CYP and these are currently to promote and increase uptake of immunisations, proactively manage long term conditions eg: asthma and mental health and signpost hard to serve children and young to appropriate community services to promote health and wellbeing and reduce avoidable hospital admissions.

As the BHM service expands to support CYP, it is an exciting time to join the team in Brent. We are seeking enthusiastic and skilled colleagues to join us as we build on existing models of care and introduce new ways of working.
