Care Assessor – Ulverston
We are currently recruiting for a Care Assessor full-time based at our Ulverston branch and surrounding areas.
The role of the Care Assessor is to ensure the continuity of care while actively promoting the services that the business offers, through face to face and telephone communication. You will be responsible for meeting with the clients, their relatives and possibly other healthcare professionals, to develop opportunities to increase the profile of the service across the territory. You will fully assess their requirements and the support they require and develop a person-centred, comprehensive care plan.

Job Description:
• Arrange, attend, and complete a review of care with customers.
• Ensure new care assessments are completed timeously.
• Develop and maintain effective working relationships with customers and managers.
• Ensure care plans are person centered.
• Complete, update and maintain records or care on the Care system.
• General office administration.
• On-call management on a rota basis.
• Qualifications and Skills – NVQ level 3 in Health and Social Care.
