Role Purpose:

Cyfle Cymru is a Peer Mentoring project which helps people develop confidence to make positive life changes, and provides support to access training, education, volunteering, and employment. Our peer mentors draw on their own lived experience to support people affected by substance misuse and/or mental health conditions. Cyfle Cymru will support people aged 16 to 24 (Not in education, employment, or training) and those 25+ that are economically inactive or long term unemployed.

Our combined expertise in the fields of substance use, mental health, housing, criminal justice, and employment support enables Adferiad Recovery to meet the needs of our most vulnerable with a single, unified, and comprehensive approach.

Adferiad in partnership with St Giles Wise have launched a finance, debt and welfare benefit service for people on probation (Male 18+). Funded by the Ministry of Justice, this exciting service provides exclusive financial wellbeing support to individuals who have been referred to the service by the Probation Service.

Vetting process will be in accordance with HM Probation and Prison Service Enhanced Level 1 check.

Role Purpose:
· To provide a new way of working to support individuals in the community clusters who would benefit from social and wellbeing support as an alternative or in addition to health care provision.
· To maximise the utilisation of community assets as an alternative to traditional health care, the service will achieve sustainable change in the way that people choose to manage their own health and wellbeing at a community level.
· To work with the GP practices within each of the 3 clusters to receive GP and self-referrals (from patients of the GP practices), to support and signpost identified patients to community support and community networks specific to their wellbeing and support needs.
· To act as a resource to practice based staff on the availability of local community and third sector services and support groups etc.
· To reduce ongoing requirements upon primary healthcare services by supporting patients to access appropriate community-based services and support.
· To ensure that the service received by the patients is person centred and is culturally competent in consideration of the individual’s cultural and socio-economic needs.

Job Purpose:

The Dyfodol Administrator and Data Officer role is a pivotal role in contributing to the development of quality of the Dyfodol Service in the custodial estate.

Based at: HMP Cardiff

Job Purpose:

Leading and supporting the Substance Misuse Team at HMP Cardiff to deliver quality substance misuse services in accordance with service specifications and contract requirements. Provide support and guidance to caseworkers involved in assessment and care planning to identify and deliver the appropriate support and interventions to reduce risk of harm and support offender rehabilitation. Monitor caseworkers’ performance to ensure that accurate and comprehensive records/data collection is maintained. Take a lead on the ongoing delivery and improvement of the recovery programme and contribute towards integrated treatment pathways and the identification of gaps in the system. Ensure that caseworkers are competent practitioners, able to provide accurate advice and information regarding substance use and associated lifestyle behaviours that reduces the risk of overdose, reduces the spread of Blood Borne Viruses, promotes healthy living, and minimises the potential risks and impact of drug related crime and disorder on the wider community.

The Dyfodol Service is an integrated partnership provision for South Wales and for the public sector prisons of HMP Cardiff, HMP Swansea, HMP Usk/Prescoed and including HMP Parc. The aim of the service is to address the causes of drug and/or alcohol related offending and risk of harm to individuals, families, peers and their communities through assessment, engagement and diverting individual offenders out of crime and into appropriate levels of interventions, support the reduction of drug and alcohol related misuse and reduce levels of offending, demonstrating evidence based best practice in all the Services. The Service will include developing and sustaining close working relationships with a number of organisations to align to and support the work of the three Area Planning Boards (Western Bay, Cardiff the Vale and Cwm Taf). It will also provide support for adult offenders and prisoners who have a drug and/or alcohol dependency who reside within the 7 unitary local authority areas of South Wales (Swansea, Neath Port Talbot, Bridgend, Cardiff, Vale of Glamorgan, Rhondda Cynon Taf and Merthyr Tydfil).
