Bury - Qualified Social Worker – Neighbourhood Team
£30 per hour umbrella
About the job:
We're seeking a compassionate and dedicated Qualified Social Worker to join our Neighbourhood Team in Bury. In this role, you'll be responsible for providing high-quality social care services to individuals and families within the community. Your primary focus will be on assessing needs, developing care plans, and implementing interventions to enhance the well-being of service users.
Key Responsibilities:
1. Conduct comprehensive assessments of individuals and families to identify their social care needs
2. Develop and implement effective care plans in collaboration with service users, their families, and relevant stakeholders
3. Provide support and guidance to individuals to promote independence and improve their quality of life
4. Work closely with other professionals, such as healthcare providers, educators, and community organizations, to coordinate holistic care for service users
5. Monitor and review care plans regularly, making adjustments as necessary to ensure they meet the evolving needs of individuals and families
6. Actively participate in multi-disciplinary meetings and case conferences to contribute valuable insights and ensure coordinated care
7. Collaborate with safeguarding teams to address and respond to concerns of vulnerable individuals, prioritizing their safety and well-being
8. Maintain accurate and up-to-date records, documenting assessments, interventions, and progress made by service users
9. Stay abreast of relevant legislation, policies, and best practices in social work, ensuring compliance with professional standards
10. Participate in ongoing professional development activities to enhance and update your skills and knowledge
Requirements:
· 1 year of qualified Social Work experience
· DBS on the Update Service or happy to apply for one
· 5 years of satisfactory references
· Driving License with access to a vehicle
· SWE Registered

Key Responsibilities:
1. Permanency Planning:
• Conduct thorough assessments of children and their families to determine the most appropriate permanency options.
• Develop and implement permanency plans in collaboration with children, their families, and relevant professionals.
2. Adoption and Fostering:
• Facilitate the adoption and long-term fostering process, ensuring that children's needs and well-being are central to decision-making.
