About the job:
As a Qualified Social Worker specialising in Children with Disabilities in Slough, your role is crucial in providing support and advocacy for children and their families facing unique challenges. You will work collaboratively with multidisciplinary teams, community resources, and families to enhance the well-being and inclusion of children with disabilities. Responsibilities:
Assessment and Planning:
Conduct comprehensive assessments of children with disabilities, considering their individual needs, strengths, and family dynamics.
Develop and implement person-centred care plans in collaboration with service users, families, and relevant professionals.
Intervention and Support:
Provide specialised interventions and support services to address the specific needs and challenges faced by children with disabilities and their families.
Collaborate with external agencies, educational institutions, and healthcare providers to ensure a holistic and coordinated approach to care.
Advocacy and Empowerment:
Advocate for the rights and inclusion of children with disabilities, promoting their active participation in decision-making processes.
Provide emotional support and practical guidance to families, empowering them to navigate challenges and access available resources.
Collaboration and Networking:
Work closely with multidisciplinary teams, including therapists, educators, and healthcare professionals, to enhance the overall support network for children with disabilities.
Establish and maintain positive relationships with community organisations to expand resources and opportunities for families.
Record Keeping and Reporting:
Maintain accurate and up-to-date records of assessments, interventions, and progress for children with disabilities.
Prepare detailed reports for case conferences, reviews, and other documentation as required.
Requirements:
· 1 year of qualified Social Work experience
· DBS on the Update Service or happy to apply for one
· 5 years of satisfactory references
· Driving License with access to a vehicle
· SWE Registered
