About the job:
We are currently seeking a dedicated and experienced Qualified Social Worker to join our Adult Social Care team in Blackburn. As a key member of our team, you will play a vital role in assessing and supporting the needs of adults in our community, promoting their independence and overall well-being.
Key Responsibilities:
1. Assessment and Care Planning:
• Conduct thorough assessments of adults requiring social care services to identify their specific needs and circumstances.
• Develop and implement personalized care plans in collaboration with individuals, their families, and other relevant stakeholders.
2. Support and Intervention:
• Provide direct support and intervention to adults, addressing issues related to physical health, mental well-being, and social inclusion.
• Advocate for individuals to access appropriate services and resources that enhance their quality of life.
3. Collaboration and Networking:
• Work closely with other professionals, including healthcare providers, community organizations, and support services, to ensure a coordinated and holistic approach to care.
4. Safeguarding:
• Actively participate in safeguarding processes, identifying and responding to concerns about the well-being of vulnerable adults.
• Implement measures to protect individuals from harm and promote a safe living environment.
5. Documentation and Record Keeping:
• Maintain accurate and up-to-date records of assessments, interventions, and ongoing progress.
• Ensure compliance with documentation standards and regulatory requirements.
6. Continuous Professional Development:
• Stay informed about current legislation, policies, and best practices in adult social care.
• Participate in training and development activities to enhance and update professional skills.
Requirements:
· 1 year of qualified Social Work experience
· DBS on the Update Service or happy to apply for one
· 5 years of satisfactory references
· Driving License with access to a vehicle
· SWE Registered
