About the job:
As a Social Worker in the Mental Health Community team at Bournemouth, Christchurch and Poole Council, your primary responsibility is to provide support and intervention to individuals with mental health needs in the community. This role involves assessment, care planning, and collaborative working with healthcare professionals and community resources to enhance the well-being of individuals experiencing mental health challenges.
Key Responsibilities:
1. Assessment and Care Planning:
• Conduct comprehensive assessments of the mental health needs of individuals in the community.
• Develop and implement person-centred care plans that address identified needs and promote recovery.
2. Collaborative Working:
• Work closely with mental health professionals, primary care services, and community resources to ensure a coordinated and integrated approach to mental health support.
• Participate in multidisciplinary meetings and contribute to care reviews.
3. Crisis Intervention:
• Provide timely and effective crisis intervention to individuals experiencing mental health crises in the community.
• Collaborate with crisis teams and emergency services to ensure appropriate support and safety.
4. Advocacy and Support:
• Advocate for the rights and preferences of individuals with mental health needs.
• Provide emotional support and practical assistance to help individuals achieve their goals and aspirations.
5. Risk Assessment and Management:
• Identify and assess risks to the safety and well-being of individuals with mental health needs.
• Develop and implement risk management plans in collaboration with the individual and relevant professionals.
6. Community Engagement:
• Facilitate community integration and participation for individuals with mental health needs.
• Collaborate with local organisations and community groups to enhance the support network available.
7. Record Keeping and Reporting:
• Maintain accurate and up-to-date case records in compliance with organisational standards.
• Prepare reports for case conferences, reviews, and other relevant forums.
Requirements:
· 1 year of qualified Social Work experience
· DBS on the Update Service or happy to apply for one
· 5 years of satisfactory references
· Driving License with access to a vehicle
· SWE Registered
